The annual compliance visit was conducted on July 18, 2025. This facility operates with a Five (5) Star License, which was issued October 23, 2017. Upon arrival at the facility, I was greeted by the administrator, Nancy Newcomb. I stated the purpose of my visit and together, we conducted a walkthrough of the facility. I monitored the indoor and outdoor spaces. The children were observed participating in indoor and outdoor free play, routines, circle time through a teacher, directed activity. The last Fire Inspection was conducted on July 17, 2025. The center was approved for daytime care only. The last Sanitation Inspection was conducted on November 21, 2025, and the facility achieved a “Superior” classification and four (4) demerits. A partial monitoring of childcare requirements was assessed this date. The permit is available, restrictions maintained. I monitored supervision, staff/child ratio, general safety, and discipline. I monitored program records, and one new staff file. The last Fire Drill was conducted on February 12, 2026. The last Lock Down/ Shelter-in-Place was on January 5, 2026. Playground Inspection was conducted on February 6, 2026. The following violations were observed, cited and technical assistance was offered. Violation Number Comment Rule 508 Special diet or food allergy information was not posted where they can be seen in food preparation and eating areas. There was no allergies list posted in the kitchen. .0901(g) 807 A safe indoor and outdoor environment was not provided for the children. The kitchen door was not locked. 10A NCAC 09 .0601(a) 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. In space 5 there was a child's medication for "Diphenhydramine" without a permission form signed and dated by parent. 10A NCAC 09 .0803(1)(a & b) 843 A drug or medicine was administered after its expiration date. A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. In space 5 there were three permission to administer medication forms that expired. One was for Nystatin expired 11/26/25, Infant gas relief drops expired on 2/7/26 and Boudreaux's But Paste expired on 1/3/26. 10A NCAC 09 .0803(1)(d) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). A. Moore did not renew her qualification letter before it expired. This person cannot work around children until she has been qualified by criminal records. This was expired on 1/26/26. G.S. 110-90.2(b) & .2703(n)&(o) 1065 Child care providers scheduled to work in the infant room, did not complete ITS-SIDS training within two months of employment or did not complete the training every three years. Child care administrators did not complete the ITS-SIDS training within 90 days of employment and every three years thereafter. D. Nobles ITS-SIDS expired on 1/11/25. .1102(f) 1757 A valid qualification letter was not on file and available to review at the facility. Several files did not have a qualification letter on file. G.S. 110-90.2(b) & (d) & .2703(e) The above violations must be corrected immediately, and a compliance letter must be drafted to demonstrate how and when corrections were made. The letter must reach my office on or before March 6, 2026. You may submit the letter to me by email at rena.mccaffrey@dhhs.nc.gov. If a compliance letter is not received at my office by 5:00 pm March 6, 2026, a return visit may be conducted to ensure the violations cited have been corrected or addressed. Additional violation may be documented if each violation has not been properly corrected or otherwise addressed. You were provided with an opportunity to ask questions regarding child care requirements. We discussed each of the areas of non-compliance during the walkthrough of the facility. TECHNICAL ASSISTANCE: Today we discussed that all medical forms that are no longer current must be kept in the child’s file for 6 months. Al permission to Administer medication form must have a starting date and ending date. They cannot say “Until further notice”. All medication forms must be kept current. Today we discussed how to manage old trainings forms and keeping current documentation on in a staff file. I stated I would send a copy of the binder to help with organizing staff files. This will help your consultant when she comes to monitor them. All criminal background records must be current, and a current qualification letter must be on file for your consultant to review. During today’s visit several letters were not on file. During today’s visit there was no allergies list in the kitchen for the cook to be aware of children with allergies when serving food to children. The kitchen was unlocked. The kitchen has many dangerous items in them and could cause children great harm if a child should walk in there. This door must be kept locked at all times. The last annual compliance visit was conducted on August 6, 2024. This facility operates with a Five (5) Star License, which was issued on October 23, 2017. A check of the Secretary of State’s website shows the Corporation Status as Current-Active. Upon arrival to the facility, I was greeted by the Nancy Newcomb, program administrator. I stated the purpose of my visit. We conducted a walkthrough of the indoor and outdoor learning spaces. Attendance was taken and observations made. The children were observed participating, indoor and outdoor free play preparing for lunch and resting. The last Fire Inspection was conducted on July 30, 2024, and was approved for day time care only. The center's last Sanitation Inspection was conducted on February 14, 2025. A 'Superior' classification was achieved. During today's visit, a full assessment of applicable child care requirements was conducted. A return visit may be conducted to monitor for compliance with transportation requirements. Today, the following violations were observed, cited and technical assistance was provided. I monitored the indoor and outdoor spaces. The children were observed participating in indoor and outdoor free play, routines, circle time through a teacher, directed activity. The last Fire Inspection was conducted on July 30, 2024. The center was approved for daytime care only. The last Sanitation Inspection was conducted on February 14, 2025, and the facility achieved a “Superior” classification. A partial monitoring of childcare requirements was assessed this date. A check of the Secretary of State’s website shows the Corporation Status as Current-Active. Upon arrival to the facility, I was greeted by a staff member. The person in charge today is Christy Beeson. Ms. Newcomb, Center Manager is not present. I stated the purpose of my visit. We conducted a walkthrough of the facility. Attendance was taken and observations made. The children were observed participating in preparing for a field trip, routines, indoor and outdoor free play preparing for lunch. The last Fire Inspection was conducted on July 30, 2024, and was approved for day time care only. The center's last Sanitation Inspection was conducted on March 8, 2024. Infants up to twelve months of age should be placed for sleep in a supine position (wholly on their back) for every nap or sleep time unless an infant’s primary health care provider has completed a signed waiver indicating that the child requires an alternate sleep position; b. Infants should be placed for sleep in safe sleep environments; which include a firm crib mattress covered by a tight-fitting sheet in a safety-approved crib (the crib should meet the standards and guidelines reviewed/approved by the U.S. Consumer Product Safety Commission [CPSC] (3) and ASTM International [ASTM]). No monitors or positioning devices should be used unless required by the child’s primary health care provider, and no other items should be in a crib occupied by an infant except for a pacifier; c. Caregivers/teachers should not administer medication based solely on a parent’s/guardian’s request. Proper labeling of medications is crucial for safety (1). Both the child’s name and the name and dose of the medication should be clear. Medications should never be removed from their original container. All containers should have child resistant packaging. Potential side-effects are usually included on prescription and OTC medications if the packaging is left. A separate consent signed by the parent/guardian for each medication the caregiver/teacher has permission to administer to the child; each consent should include the child’s name, medication, time, dose, how to give the medication, and start and end dates when it should be given; Authorization from the prescribing health professional for each prescription and non-prescription medication; this authorization should also include potential side effects and other warnings about the medication… (exception: non-prescription sunscreen and insect repellent always require parental/guardian consent but do not require instructions from each child’s individual medical provider). The last Annual Compliance visit was conducted on August 11, 2023. The facility operates with a Five Star License, which was issued on October 23, 2017. The center's last Fire Inspection was conducted on August 3, 2023 and was approved for day time care only. The last Sanitation Inspection was conducted on March 8, 2024 and a superior classification was achieved. Upon arrival to the facility I was greeted by the administrator, Nancy Newcomb. I stated the purpose of my visit and was assisted on a walkthrough of the facility. Attendance was taken and observations made. The children were observed resting and completing routines. A partial assessment a child care requirements was conducted this date. I monitored, general requirements, and the he permit was posted, requirements maintained, program records, staff emergency trainings, supervision, staff/child ratio, I monitored medication requirements and general safety. All criminal background checks are current. The following violations were observed, cited and technical assistance offered. Violation Number Comment Rule 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. In the infant classroom, a prescription medication was not maintained in locked storage. 15A NCAC 18A .2820(d) 847 Parent's medication authorization did not include required information. In a classroom with infants and a classroom with children who are two years old, the dates to begin and complete the administration of medication was not completed/provided. 10A NCAC 09 .0803(4)(6-9) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. Permission to administer medication forms were not observed for over the counter medications in a classroom with children who are two years old .0803(12) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff member's training expired in September 2023. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff member's training expired in September 2023. .1102(d) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One staff member's file did not contain documentation of Maltreatment training. .1102(g) The above violations must be corrected immediately, and a letter sent to my office stating how the violations were corrected. TECHNICAL ASSISTANCE: -CPR/FIRST AID TRAINING All staff members involved in providing direct care to children should complete and document training in pediatric first aid and cardiopulmonary resuscitation (CPR). Courses in pediatric first aid and CPR should be taught in person by instructor-led demonstrations and practiced ensuring the technique could be performed in an emergency. Early care and education programs should follow training renewal cycles recommended by the providing organization (eg, American Heart Association [AHA]). Early care and education programs with staff trained in pediatric first aid and CPR can mitigate the consequences of injury and reduce the potential for death from life-threatening conditions and emergencies. Furthermore, knowledge of pediatric first aid and CPR include addressing a blocked airway (choking) as well as rescue breathing. Repetitive training, coupled with the confidence to use these skills, are critically important to the outcome of an emergency. Documentation of current certification of satisfactory completion of pediatric first aid and demonstration of pediatric CPR skills in the facility assists in implementing and monitoring for proof of compliance. -HEALTH AND SAFETY TRAINING REQUIREMENT- 10A NCAC 09 .1102 HEALTH AND SAFETY TRAINING REQUIREMENTS (g) The child care administrator and all staff members shall complete Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. Upon arrival to the facility, I was greeted by Nancy Newcomb, Center Manager. We conducted a walkthrough of the facility. Attendance was taken and observations made. The children were observed participating in routines, outdoor free play, and resting. The last Fire Inspection was conducted on August 3, 2023, and was approved for day time care only. The center's last Sanitation Inspection was conducted on March 28, 2023. A 'Superior' classification was achieved. During today's visit, a full assessment of applicable Child Care Requirements was conducted. These included, supervision, staff/child ratio, permit was posted. I monitored program records, a sampling of children’s records and a sampling of staff files. I was unable to complete the staff file monitoring this date; therefore, a return visit will be conducted to complete the review of the files. The following violations were observed, cited and technical assistance was provided.
