The last Annual Compliance Visit was conducted on September 3, 2024, the facility currently operates with a Five (5) Star License which was issued on December 6, 2018. Upon arrival to the school, I checked in at the front office and proceeded to the classrooms. I was greeted by the new lead teacher Ms. Martinez, who assisted me during the visit. Attendance was taken and observations made. The children were observed having lunch, participating in routines, outdoor free play and resting. There are two classrooms at this location. The one is an EC classroom and the other is a blended EC and NC Pre-K classroom. The facility's last Fire Inspection was conducted on April 30, 2025, and was approved for daytime care only. The center's last Sanitation Inspection was conducted on March 4, 2025, and was classified as ‘Superior'. During today's visit, a full assessment of applicable Child Care Requirements was conducted. I monitored all child care requirements, the license was posted, restrictions were maintained, I monitored supervision, staff/child ratio and adequate approved space. I monitored program records; I reviewed staff files and training and a sampling of children’s records. The following violations were observed, cited and technical assistance was provided. Violation Number Comment Rule 705 Equipment and furnishings were not sturdy, stable and free of hazards. I observed on the outdoor learning space that the wooden boarders encompassing the mulch surrounding the climbers are splintering in several places. I ask if children have gotten splinters. The staff stated that they have and that it was documented and reported. Staff have requested the repair of the boards. .0601(c) 707 Equipment and furnishings not meeting the requirements outlined in child care .0601(b) and (c) were not removed or made inaccessible. **The splintering wooden boarders observed on the outdoor learning space which surround the climbers are not repaired/smooth/replaced or rendered safe for the children immediately.** .0601(d) 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. **One child's medical assessment was not available for Division Representative Review.** GS110-91(1) The above violations must be corrected immediately, and a corrective action letter must be sent to my office on or before June 17, 2025. Please include the name and identification number of your facility, as well as your electronic signature. The letter must be received by my office on or before June 17, 2025. We discussed the violations cited on this date regarding a safe environment. Both the indoor and the outdoor environment must be safe for the children. I observed on the outdoor learning space that the wooden boarders surrounding the climbers are splintering and there are incident reports stating that children have received splinters. These boarders must be rendered safe for the children immediately or they must be removed and replaced. See the rules below. SECTION .0600 - SAFETY REQUIREMENTS FOR CHILD CARE CENTERS 10A NCAC 09 .0601 SAFE ENVIRONMENT All child care centers shall provide a safe indoor and outdoor environment for the children in care. -All equipment and furnishings shall be in good repair. All commercially manufactured equipment and furnishings shall be assembled and installed according to procedures specified by the manufacturer. For equipment and furnishings purchased after September 1, 2017, that include instructions from the manufacturer, those manufacturer's instructions shall be kept on file at the center, unless they are available electronically for review. -Equipment and furnishings shall be sturdy, stable, and free of hazards that may injure children including sharp edges, lead based or peeling paint, rust, loose nails, splinters, protrusions (excluding nuts and bolts on sides of fences), and pinch and crush points. -Staff of the center shall immediately remove all equipment and furnishings that do not meet the requirements of Paragraphs (b) and (c) of this Rule from the premises or make the equipment or furnishings inaccessible to the children. -Each child care center shall provide equipment and furnishings that are child-size or that can be adapted for use by children. Meghan Williamson, Lead teacher, was present and assisted me during the visit. Attendance was taken and observations made. The children were observed participating in morning routines, teacher directed activities and indoor, outdoor free play and resting. The center's last Fire Inspection was conducted on April 16, 2024 and was approved for daytime care only. The center's last Sanitation Inspection was conducted on March 15, 2024 and was classified as ‘Superior'. During today's visit, a full assessment of applicable Child Care Requirements was conducted. I monitored license restrictions, supervision, staff/child ratio and adequate approved space. I monitored program records, criminal background checks, I reviewed staff files and training and a sampling of children’s records. This is the second week of Emergency drills have been conducted. There is a new administrator at this facility. I have collected the preservice requirements for administrators form during today’s visit. The following violations were observed, cited and technical assistance was provided. Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed. This facility has a new administrator. The DCDEE criminal record check was not available to Division Representative review. G.S. 110-90.2(b) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One staff member who was hired in October 2023, did not have documentation of completing Recognizing and Responding to Suspicions of Child Maltreatment. .1102(g) The above violations must be corrected immediately. A corrective action letter must be drafted describing how and when the violation was corrected. Please include the name and identification number of your facility, as well as your electronic signature. The letter must be received by my office on or before September 17, 2024. Any persons responsible for and/or caring for pre-school aged children must follow the steps to acquire this documentation and it must be available for Division Representative review. Protective surfacing shall be either: 1) loose surfacing material, including wood mulch, double shredded bark mulch, uniform wood chips, fine sand, coarse sand, and pea gravel, except that pea gravel shall not be used if the area will be used by children under three years of age. (c) The site-level administrator shall document the progress of any teacher assistants who are working toward the required education. The last Annual Compliance Visit was conducted on September 8, 2022. The facility currently operates with a Five (5) Star License which was issued on December 6, 2018. There are two classrooms this location. They are identified as rooms 102 and 104. Upon arrival to the school, I checked in at the front office and proceeded to the classrooms. Ashley Defeo-Belk, Lead teacher, was present and assisted me during the visit. Attendance was taken and observations made. The children were observed participating in morning routines, teacher directed activities and indoor and outdoor free play. The center's last Fire Inspection was conducted on March 29, 2023 and was approved for daytime care only. The center's last Sanitation Inspection was conducted on December 7, 2022 and a ‘Superior' rating was achieved. During today's visit, a full assessment of applicable Child Care Requirements was conducted. I monitored license restrictions, supervision, staff/child ratio and adequate approved space. I monitored program records, criminal background checks, I reviewed staff files and training and a sampling of children’s records. Emergency drills and playground inspections were current. The following violation was observed, cited and technical assistance was provided. Violation Number Comment Rule 716 All stationary equipment, more than 18 inches high, was not installed over protective surfacing. There were two climbing structures which did not have protective surfacing. Currently, the surfacing beneath the structures is grass. .0605(j) The above violation must be corrected immediately. A Corrective Action Letter must be drafted describing how and when the violation was corrected. Please include the name and identification number of your facility, as well as your electronic signature. The letter must be received by my office on or before September 19, 2023.
