The last annual compliance visit was conducted 8/19/25. Currently this program operates with a GS 110 Notice of Compliance issued 5/26/21. The center director, Mila Fields, was not present. Teresa Williams assisted me today. I conducted an assisted walk-through of the center. The last sanitation inspection was completed 3/3/26 with a ‘Superior’ classification and 2 demerits. The last fire inspection was conducted on 1/21/26. Children were observed playing in activity areas and routine care activities. The files for two new staff members were reviewed. A partial assessment of applicable childcare rules was conducted today. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The fire inspection was due by 1/8/26 and was completed 1/21/26. 10A NCAC 09 .0304(a) 843 A drug or medicine was administered after its expiration date. The inhaler for one child in twos was expired. 10A NCAC 09 .0803(1)(d) 847 Parent's medication authorization did not include required information. Name of Operation: BETHANY STEPPING STONES Facility ID: 29000100 Consultant: ANITA GRAHAM Operation Type: Center Case Number: Visit Date: 8/19/2025 Number Present: 70 Completed Date: 8/19/2025 Age: From 0 To 5 Total Minutes: 207 Time In: 11:35 AM Time Out: 03:02 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor compliance with North Carolina child care requirements. The last Annual Compliance visit was conducted on October 1, 2024. The facility operates under a GS-110 Notice of Compliance, which was issued on May 18, 2015. Upon arrival at the facility, I was greeted by Mila Fields, the school administrator, who assisted me with a walkthrough of the indoor and outdoor learning environments. Attendance was taken, and observations were made throughout the visit. Children were observed participating in routine care, eating lunch, and resting. The last fire inspection was conducted on January 9, 2025, and was approved for daytime care only. The most recent sanitation inspection was conducted on February 18, 2025, and received a “Superior” classification. A full assessment of child care requirements was conducted, including a review of the posted Notice of Compliance and any restrictions, supervision practices, staff-to-child ratios, and general safety. Program records, children’s records, and staff files were reviewed for compliance. The vehicles used to transport children were unavailable for monitoring during the visit. A return visit may be conducted to monitor these at a later date. Today the following violation was observed, cited and technical assistance was provided. Violation Number Comment Rule 525 Menus for all meals and snacks were not planned at least 1 week ahead and dated. **Menus were not posted a week one week in advance of service.** 10A NCAC 09 .0901(b) The above violation must be corrected immediately. A compliance letter is required and must be sent to my office by or before 5:00 PM on September 2, 2025. The letter should explain how and when the violation was corrected, and it must be submitted on the facility’s official letterhead. Please include the name and ID number of your facility, and your electronic signature in the letter. If the letter is not received by the September 2, 2025, a return visit may be necessary to verify compliance. You were provided an opportunity to ask questions, and we discussed each area of noncompliance during the visit. Technical Assistance was provided to address this area of noncompliance, including a reminder that menus must be planned in advance and posted in a visible location for parent review.
