You, Lauren Matthews, Site Coordinator assisted me with the visit. The last annual compliance visit was conducted on April 7, 2022. The last sanitation inspection was completed on September 22, 2023, with a “Superior” classification. The last fire inspection was completed on October 3, 2023, and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-seven percent prior to today’s visit. I observed your Three-Star License, Safe Procedures for Arrival and Departure, current menu, sanitation placard, Emergency Numbers, Emergency Medical Care Plan, First Aid information sheet, tobacco free policy signage at the entrance of the facility, Summary of NC Child Care Law, evacuation plans, current daily schedules, current activity plans, staff-child ratio sheets, and allergies posted as required. I observed children adequately supervised, staff-child ratios maintained, adequate approved space use, and permit restrictions maintained during today’s visit. I observed children transitioning from school to after school care, toileting, handwashing, snack, and outdoor play. I observed toys, equipment, and materials throughout the facility to be of sufficient quantity, developmentally appropriate, and in good repair. I observed a current activity plan posted with developmentally appropriate activities listed. I monitored emergency prescription medication and one medical action plan in the facility today. I observed your Incident Log to be current and up to date as required. I observed your monthly Fire Drill Log documenting drills for the past twelve months. I observed the last fire drill documented on this log dated February 20, 2024, at 7:03am. I observed your Shelter-in-Place/Lockdown Drill Log documenting drills for the past twelve months. I observed the last emergency drill documented on this log was dated December 12, 2023, at 7:19am. I monitored a selection of staff files and children’s files and one staff file during today’s visit. Transportation is not provided. The following violation was documented today: Violation Number Comment Rule 1835 The medical action plan was not updated on an annual basis or when changes to the plan were made by the child's parent or health care professional. One medical action plan was incomplete and missing the second page of the plan. .0801(b) Technical Assistance was provided on the following: 1. We discussed that when medical action plans are receiving that staff should review the documents with the parent to ensure that all areas of the plan are complete. If a question does not apply to the child, the question or statement should be marked with “N/A”. Consultation was provided on the following: 1. We discussed that your facility is listed in Cohort 1 for the Star Rated License Reassessment. July 1, 2023, began your facility’s prep year. July 1, 2024, through June 30, 2025, will be the facility’s reassessment year. 2. We discussed ensuring that your information is updated in your WORKS account. 3. We discussed working with your consultant to establish a timeline for technical assistance visits that the facility may need. 4. We discussed reaching out to your local partners such as the Partnership for Young Children for technical assistance. The Partnership for Young Children also conducts mock assessments and will look at your facility’s materials. 5. We discussed visiting the NCRLAP website (www.ncrlap.org) for resources and training opportunities to prepare for the star rated license. 6. We discussed that the gym is currently not listed as licensed space. If you would like for the gym to be licensed for children’s use you will need to obtain the proper inspections and send to me prior to a scheduled visit to measure and approve this space. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. You, Laura Matthews, Site Coordinator, assisted me with today’s visit. The last Annual Compliance Visit was completed on March 14, 2023. The facility operates with a three star license issued on February 18, 2019, earning two points in program standards, five points in education standards, and one quality point. The 18-month compliance history is at 100% as of September 7, 2023. The sanitation inspection was completed on January 30, 2023, with a “Superior” classification. The last fire inspection was conducted on April 25, 2023, and your facility was approved for daytime care only. I received a copy of the fire inspection during today’s visit. I observed your Three Star License, Safe Procedures for Arrival and Departure, sanitation placard, Emergency Numbers, Emergency Medical Care Plan, First Aid information sheet, tobacco free policy signage, Summary of NC Child Care Law dated January 2021, evacuation plans, current daily schedules, current activity plans, staff-child ratio sheets, and allergies posted as required. I observed adequate supervision, staff/child ratios maintained, meeting enhanced space, and licensed capacity maintained during today’s visit. I observed children transitioning from their school day to after school care, toileting and handwashing routines, outdoor play, and snack. Snack consisted of fruit loop cereal and milk. You stated that no children currently enrolled receive medications while in care or have medical action plans. The outside play area does not meet child care playground safety standards, as indicated on your child care license. No safety concerns were observed today. I observed monthly fire drills documented for the past twelve months. I observed the most recent fire drill was conducted on August 30, 2023, at 5:45pm with two minutes to evacuate. I observed the most recent emergency drill in the form of a lockdown was conducted on June 5, 2023, at 3:50pm with two minutes to lockdown. I observed one new staff member has been hired since the last ACV on March 14, 2023. I monitored this new staff file during today’s visit. I monitored one existing staff file for valid Criminal Background Check (CBC) qualification letters, valid CPR and First Aid training and valid BSAC training. I observed no violations related to staff records. The following violations were cited during today’s visit: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The fire inspection conducted on April 25, 2023 was not received within 7 days. A copy was received during today's visit. 10A NCAC 09 .0304(a) 811 Potentially hazardous items including but not limited to power tools, nails, chemicals, propane stoves, lawn mowers, gasoline, or kerosene were not stored in locked areas, removed from the premises, or made inaccessible to children. The emergency ready to go backpack contained ice packs that were labeled "keep out of reach of children" and other warning labels. The ice packs were removed from the backpack during today's visit. .0604(a) During today’s visit, I provided technical assistance (TA) regarding the following topics: 1. We discussed that fire inspections should be completed annually, on or before the date of the current inspection. Fire inspections should be sent to me within seven days of receiving a visit. 2. We discussed that if children are engaged in any screen that a screen time log should be kept to document children’s screen usage. Children should have no more than two and a half hours of screen time weekly. 3. We discussed that emergency ready to go bags should not contain items such as ice packs, alcohol prep pads, or other items that are labeled keep out of children, and items that have additional warning labels. 4. We discussed that emergency drills are required to be conducted every three months. Please conduct an emergency drill in the form of a lockdown or shelter-in-place during the month of September. 5. We discussed that your facility is listed in Cohort 2. The planning year for your facility is set to begin July 1, 2024, through June 30, 2025. The reassessment year is set to begin July 1, 2025, through June 30, 2026. 6. We discussed that there is a challenging behaviors helpline that can be utilized by staff members. The number for the helpline is 1-888-600-1685. You can also visit childcareresourcesinc.org for more information.
