You, Stephanie Marshall, Site Manager, and Laura Mitchell, Lead Teacher, assisted me Meria Wilder, Child Care Consultant with today’s visit. I observed the following items posted in the foyer: five star rated license, tobacco free signage, emergency phone numbers, emergency medical care plan, sanitation inspection, daily schedule, current activity plan, first aid information sheet, allergies posting, arrival and departure procedure, emergency medical care plan, NC child care law, and menu. The following items were monitored: Supervision, staff/child ratios, licensed capacity, permit restrictions, general safety, adequate/approved space, storage of hazardous substances, storage of medication, program records, staff records, CPR and First Aid training, criminal background checks, and special trainings. I monitored current fire drills and emergency drills documented on the Emergency Drill Log. The facility is documenting completing fire drills monthly as required. The facility is documenting completing emergency drills every three months as required. Playground safety inspections were completed monthly as required. I reviewed all staff files for criminal background qualification letters, first aid, CPR and Recognizing and Responding to Suspicions of Child Maltreatment during today’s visit. Children’s Records were not reviewed during today’s visit. During today’s visit I observed one teacher, Ms. Mitchell, leave the classroom obtaining a staff & training worksheet from the site Manager whose office is outside of the classroom, when Ms. Lackey notified Ms. Robinson that one child who is considered a “runner” was moving towards the door. The child ran out of the classroom and out of the school building to the outside with Ms. Robinson immediately running after the child. At this point Ms. Lackey was left in the classroom with the remaining 16 children that were in the classroom. Ms. Robinson and the child returned to the classroom and the child was safe and unharmed. This facility meets enhanced ratios minus 1 and has a staff child ratio of 1 teacher to 9 children. There are 3 teachers in this room due to the classroom being a stand-alone classroom. The following violations were observed today. Violation Number Comment Rule 1301 Center did not maintain a record of daily attendance. One child who was not present today was not marked absent on the daily attendance record. GS 110-91(9) Technical Assistance: • Daily attendance records must be maintained at all times. One child who was not present today was not marked absent on the daily attendance record. Ms. Mitchell immediately updated the attendance record and as discussed, the attendance record will be printed in paper form and updated as children are signed in and update the electronic copy when time permits. Any observed violations cited during visits will negatively affect the center’s Compliance History Score. Your program must maintain a compliance score of at least 75% as required by G.S. 110-90 (4)(c) in order to be eligible for a star rated license. Consultation: • As of September 2023, the NC Summary of Child Care Law has been updated, be sure that parents receive, sign and acknowledgement of receipt as well as post the updated information with other required postings in your facility. • Although the classroom teacher being out of ratios due to the other teacher following a child that ran out of the classroom was an emergency situation, we discussed, moving forward compliance will be maintained by insuring there is a staff person positioned near the door to prevent the child from being able to exiting, ensuring the doorbell that is already in place is engaged at all times, using a walkie talkie or some type of communication system is in place to reach staff outside of the classroom in the event one of the classroom teachers need to leave the classroom for any reason and ensuring that there are 3 teachers in the classroom at all times, and at any time there are 2 teachers left alone in the classroom 1 teachers will remain with that child until the 3rd teacher returns to the classroom. Ms. Marshall, Site Manager immediately contacted the local Partnership to request support for her staff and the child that ran out of the classroom. Due to the above named incident being considered an emergency situation there was no violation cited based on Child Care Rule 10A NCAC 09 .1801 SUPERVISION IN CHILD CARE CENTERS which states the following: (a) Children shall be adequately supervised at all times in child care centers. Adequate supervision shall mean that: (1) staff must be positioned in the indoor and outdoor environment to maximize their ability to hear or see the children at all times and render assistance; (2) staff must interact with the children while moving about the indoor or outdoor area; (3) staff must know where each child is located and be aware of the children's activities at all times; (4) staff must provide supervision appropriate to the individual age, needs, and capabilities of each child; and (5) staff must be able to see and hear children aged birth to five years old while the children are eating. All of the conditions in this Paragraph shall apply except when emergencies necessitate that adequate supervision is impossible. Documentation of emergencies shall be maintained and available for review by Division representatives upon request. Due to the violations being corrected during the visit, no compliance letter is due to me. Any observed violations cited during visits will negatively affect the center’s Compliance History Score. Your program must maintain a compliance score of at least 75% as required by G.S. 110-90 (4)(c) in order to be eligible for a star rated license. Name of Operation: TAYLORSVILLE ELEMENTARY SCHOOL - NCPK Facility ID: 02000117 Consultant: MERIA WILDER Operation Type: Center Case Number: 1023-056L Visit Date: 10/9/2023 Number Present: 15 Completed Date: 10/9/2023 Age: From 4 To 5 Total Minutes: 135 Time In: 11:45 AM Time Out: 02:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Complaint Visit Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to obtain information regarding alleged violations of child care requirements. The allegation is as follows: There are concerns that children are not being adequately supervised. You, Stephanie Marshall, NC Pre-k Site Manager, assisted us with the visit. Rebbecca Hayes, Child Care Consultant, accompanied me on today’s visit. We observed children transitioning to and from outdoor gross motor play, toileting and handwashing routines, and rest time. Staff stated that a report was made to administration on October 3, 2023, regarding an incident of inadequate supervision. Staff stated that the incident was investigated by administrative staff. Staff stated that on October 3, 2023, children were transitioning from the outdoor play area to the classroom to begin their toileting routines. A staff member took 7 children to another bathroom located outside of the licensed child care space. As the children were finished using the bathroom, the staff member watched them walk back to the classroom. One child, was seen turning as if they were going back into the classroom and the staff member assumed the child had went into the room and resumed monitoring the last child that remained in the bathroom. The child took a detour and managed to go out of the front entrance door of the Before and After School Care room that is located beside the classroom. Once the child exited the door, the door automatically locked. The door to the child’s assigned classroom was also locked. Staff continued with their transition into rest time. Staff stated that they were placing children on their cots and covering them up. Once everyone was laying down, staff proceeded with their normal rest time routine. Staff stated that at approximately 1:20pm they heard someone at the main door with keys trying to get in. A staff member walked out of the classroom to the door to let the principal inside the building. At this time, the principal noticed a child sleeping in a chair and asked was the child supposed to be in that area. Staff stated “no” and they immediately picked the child up and brought them back into the classroom. The child was left unsupervised for approximately thirty minutes. Staff stated that they contacted administration to let them know of the incident that had occurred and contacted the child’s parent to inform them of what happened. Based on the information from staff interviews during today’s visit; the concerns regarding children not being adequately supervised was substantiated. During today’s visit, staff stated that they are actively counting children during transitions, monitoring and scanning the classroom during rest time, conducting head counts verbally to other staff members, utilizing the cot chart that has been put in place, and they are no longer to use any bathrooms located outside of the licensed child care space. Staff stated that no cots are being placed in blind spots that cannot be visibly seen. Based on the information given, there were fifteen children present, ages four to five years of age. The following violation was cited during today’s visit. Violation Number Comment Rule 303 Children were not adequately supervised at all times. On 10/3/23, a child was not adequately supervised during bathroom routines and rest time. .1801(a)(1-5) The following Technical Assistance was provided during today’s visit: 1. We discussed that children should be in eyes view of staff members at all times. 2. We discussed that during rest time, staff members should be positioned so that all children are easily seen. It is recommended that staff move about the classroom to ensure the safety of all children and that all children are accounted for. 3. We discussed that children should be monitored when going to the bathroom during rest time. 4. We discussed that children should be escorted by an adult to and from the classroom. 5. We discussed using bathrooms that are located inside the licensed child care room. 6. We discussed staff members looking at their room to ensure that children do not have access to exits that may lead to other places outside of the licensed child care space. 7. We discussed what active supervision looks like inside the classroom, outdoors, and during transitions while children are in care to ensure that children are being cared for in a safe environment. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation documented must be corrected immediately. On or before October 23, 2023, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be conducted. Due to the nature of this violation an Administrative Action may be issued. You, Laura Mitchell, Lead Teacher, and Stephanie Marshall, Site Manager assisted me with today’s visit. Your program currently operates with a four-star license, issued February 8, 2019, earning 6 points in the education component, 7 points in the program standards and 1 quality point. Your program was also monitored for compliance with implementing an approved curriculum, Creative Curriculum, as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted on October 19, 2022. The sanitation inspection was completed April 17, 23 with a “Superior” classification and (0) zero demerits. I observed the last fire drill was recorded as being completed on September18, 2023. I observed a Lockdown drill was recorded as being completed on September 7, 2023. Outdoor inspections were monitored during today’s visit. I observed the indoor and outdoor space and found sufficient materials. I observed children engaged in outside play including walking around the bike track with the teacher participating in songs and conversation, playing in the sandbox, and riding tricycles. I observed children transition back inside, participate in toileting routines, handwashing, and meal time. I observed a menu dated for today posted consisting of chicken sandwich, peach fruit cups, baked beans, and milk for lunch. I observed the classroom to be organized with easily identified activity centers. I observed teachers interacting with the children in a nurturing way. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. The NC Pre-K Program Site Monitoring Tool was reviewed today. This program uses the DIAL 4 screening tool. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress. The checkpoint assessments are conducted three times per year: at the beginning, middle and end of the school year. The classroom operates from 8:30 am to 2:45 pm. GS 110-91(9) 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. A signed and dated letter of compliance must be received by me no later than 10/16/2023, stating how each violation was corrected and how compliance will be maintained in the future. You, Laura Mitchell, Lead Teacher, and Stephanie Marshall, Site Manager assisted me with today’s visit. Your program currently operates with a four-star license, issued February 8, 2019, earning 6 points in the education component, 7 points in the program standards and 1 quality point. Your program was also monitored for compliance with implementing an approved curriculum, Creative Curriculum, as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted on October 19, 2022. The sanitation inspection was completed April 17, 23 with a “Superior” classification and (0) zero demerits. I observed the last fire drill was recorded as being completed on September18, 2023. I observed a Lockdown drill was recorded as being completed on September 7, 2023. Outdoor inspections were monitored during today’s visit. I observed the indoor and outdoor space and found sufficient materials. I observed children engaged in outside play including walking around the bike track with the teacher participating in songs and conversation, playing in the sandbox, and riding tricycles. I observed children transition back inside, participate in toileting routines, handwashing, and meal time. I observed a menu dated for today posted consisting of chicken sandwich, peach fruit cups, baked beans, and milk for lunch. I observed the classroom to be organized with easily identified activity centers. I observed teachers interacting with the children in a nurturing way. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. The NC Pre-K Program Site Monitoring Tool was reviewed today. This program uses the DIAL 4 screening tool. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress. The checkpoint assessments are conducted three times per year: at the beginning, middle and end of the school year. The classroom operates from 8:30 am to 2:45 pm.
