You, Angela Ferretiz, Program Coordinator, assisted me with today’s visit. The last annual compliance visit was conducted on April 15, 2024. The last sanitation inspection was completed on September 6, 2024, with a “Superior” classification. The last fire inspection was completed on April 15, 2024, and your facility was approved for daytime care only. The program’s compliance history was ninety-seven percent prior to today’s visit. The center's three-star rated license, NC Child Care law summary, safe arrival and departure procedures, emergency phone numbers, tobacco restriction, menu, and Emergency Medical Care Plan were prominently posted. The following was monitored during today’s visit: supervision, staff/child ratios, permit restrictions, group size, capacity, adequate and approved space, and hazardous product storage. A walkthrough of the indoor and outdoor environment was completed. Children were observed transitioning to after school care, teacher directed activities, toileting and handwashing routines, snack, and outdoor play. Fire drills were completed monthly as required. The last fire drill was completed on February 6, 2025. Emergency drills are being completed every three months as required. The last emergency drill was completed on February 10, 2025. Your playground does not meet child care safety standards. No concerns were observed. Materials, toys, and equipment throughout the facility was of sufficient quantity, developmentally appropriate, and in good repair. All medications and medical action plans were monitored to be in compliance today. A selection of staff and children’s files were monitored during today’s visit. The facility does not provide transportation. The following violations were documented during today’s visit: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The activity plan posted was dated March 3rd through March 7th. GS 110-91(12); .0508(a) 811 Potentially hazardous items including but not limited to power tools, nails, chemicals, propane stoves, lawn mowers, gasoline, or kerosene were not stored in locked areas, removed from the premises, or made inaccessible to children. A bottle of rubbing alcohol and a tube of Aveeno Skin Relief lotion labeled "keep out of reach of children" was below 5 feet on the teachers desk. .0604(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Two bottles of Betco Push cleaner with additional warning labels was stored above 5 feet but not locked. .2820(b) Technical Assistance was provided on the following: 1. We discussed completing a walkthrough of the classroom prior to caring for children to ensure that items labeled “keep out of children” are inaccessible to children. Items that are labeled “keep out of reach of children” and have additional warning labels are required to be locked. During today’s visit you locked all hazardous items in the cabinet. 2. We discussed on Friday afternoons posting the activity plan for the following week to ensure that the activity plan is current. During today’s visit you printed out the current activity plan and posted it in the classroom. Consultation was provided on the following: NEW QRIS MODERNIZATION PLAN: The new QRIS Modernization Plan known as “Pathways to Success” is being added to law which included moving to the use of the updated Environmental Rating Scale (ERS-3 versions) for FCCERS, effective February 1, 2025. DCDEE and the NC Child Care Commission will continue the rulemaking process to add the rule language to child care requirements. This process with take several months. Hold Harmless provisions have been extended. Star rated license reassessments are postponed until the QRIS rulemaking process has been completed. Star rated license assessments are still required for new child care programs. A pathway for child care programs currently accredited through the approved accrediting bodies may now move to a 3- or 5-star license if they are not already at this star level. Providers need to submit a request to their licensing consultant and provide a copy of their accreditation award/certificate for processing: Accreditation Licensure Request Form. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. You, Angela Ferretiz, Site Coordinator assisted me with the visit. The last annual compliance visit was conducted on October 19, 2022. The sanitation inspection was completed on January 27, 2023, with a “Superior” classification. The last fire inspection was conducted on March 24, 2023. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-eight percent as of September 25, 2023. I observed your Three Star License, Safe Procedures for Arrival and Departure, current menu, sanitation placard, Emergency Numbers, Emergency Medical Care Plan, First Aid information sheet, tobacco free policy signage, Summary of NC Child Care Law dated January 2021, evacuation plans, current daily schedules, current activity plans, staff-child ratio sheets, and allergies posted as required. I observed children adequately supervised, staff-child ratios maintained, adequate approved space use and permit restrictions maintained during today’s visit. I observed children transitioning from their school day to After School Care, toileting, handwashing, snack, teacher directed activities, and indoor gross motor play. Snack was observed and consisted of an apple, cheez- its, and milk. I observed toys, equipment, and materials throughout the facility to be of sufficient quantity, developmentally appropriate, and in good repair. I observed a current activity plan posted with developmentally appropriate activities listed. I observed your Incident Log to be current and up to date as required. I observed your monthly Fire Drill Log documenting drills for the past twelve months. We observed the last fire drill documented on this log was dated August 29, 2023, at 3:45pm with two minutes to evacuate. I observed your Shelter-in-Place/Lockdown Drill Log documenting drills for the past twelve months. I observed the last emergency drill documented on this log was a lockdown dated August 28, 2023, at 7:40am with three minutes to lockdown. Emergency drills are being conducted every three months as required. Your playground is exempt from meeting the child care safety requirements as indicated on your child care license. No safety concerns were observed. I monitored one staff file and a selection of five children’s files today. I observed no violations related to staff files or children’s files. I monitored one Medical Action Plan to be current and in compliance. You stated that you do not provide transportation. We discussed that children’s water bottles should be labeled with their name and date. 3. We discussed that your facility is listed in Cohort 1 for the Star Rated License Reassessment. We discussed working with your consultant to establish a timeline for technical assistance visits that the facility may need. 6. We discussed reaching out to your local partners such as the Partnership for Young Children for technical assistance. We discussed visiting the NCRLAP website for resources and training opportunities to prepare for the star rated license. 8. We discussed that the Summary of Child Care Law has been updated. A revised document can be found on the DCDEE website under provider documents. Please print the updated version and post it in your licensed space. Also please update this form in your parent handbook and have parents sign that they have received the updated version. 9. We discussed that a training on Child Maltreatment should be taken every five years. You may reach out to the local Partnership for Young Children or Department of Social Services to inquire on training opportunities related to this topic. If a training cannot be found, you may retake the Recognizing and Responding to Suspicions of Child Maltreatment training that can be located at www.preventchildabusenc.org. Please retain this certificate in your staff personnel file for review. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before October 9, 2023, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected.
