The entire home is licensed for child care and must meet the applicable child care requirements. Although you designated space to be used for child care in a specified section of your home, the license is issued for the entire location. Any monitoring visit conducted may include spaces other than those you designated for child care. You, P. Spruill, Operator, were present and available to assist with today’s visit. Your program currently operates with a five-star license issued August 8, 2017, earning 7 points in the education component, 6 points in the program standards component and 1 quality point for infant capacity restricted to no more than two (2) children under one year of age. The last annual compliance visit was conducted on January 24, 2025. The center’s compliance history was reviewed with the operator. The program’s compliance history was 99% percent prior to today’s visit. There were four (4) children, from ages three-years-old to five-years-old, who were present during the visit. The children were observed in the designated caregiving area playing with developmentally appropriate toys and materials. The caregiver was interacting in a caring and nurturing manner. The following were served for lunch: chicken nuggets, applesauce, green beans, bread, and milk. HOUSEHOLD MEMBER(S): The following household member resides in the home with you; Randolph Spruill-spouse with a qualifying criminal background check (CBC) letter that will expire on April 11, 2027. Please remember CBC letters can be renewed up to six (6) months prior to the expiration. TRANSPORTATION: You are currently meeting all the requirements to provide transportation; you have a valid license through February 13, 2033, your car insurance is valid through August 2, 2026, the vehicle was last inspected August 2025. The tires on your vehicle meet the 1/23 tire tread requirement, you had a fire extinguisher and first aid kit in your vehicle. You had working seat belts and child safety car seats. The following violations were documented during today’s visit: Violation Number Comment Rule 1101 Operator or other transportation provider did not have written permission from each child's parents or guardian to transport the child. One child who had a permission to transport form on file dated November 4, 2024; however, the form needed to be updated on or prior to November 4, 2025. .1723(5) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before January 1, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware that any written information submitted by you regarding correction of violations documented during today's visit is legal documentation. Prior to changing the facility name and/or ownership (i.e., forming a corporation), please contact me to discuss procedures. Failure to comply may affect your Family Child Care Home License. You verified the mailing address, phone number, and email address listed on the facility profile are correct. TECHNICAL ASSISTANCE/CONSULTATION: STAFF AND CHILDREN RECORD REQUIREMENTS: I reviewed your files and completed the Verification of Required Information for Operator and Additional Caregivers form and FCCH and Center Located in a Residence Operator’s Statements of Responsibility. All children’s files were monitored during today’s visit. The files were well organized and current. PERMISSION TO TRANSPORT: You had one child who had permission to transport form on file dated November 4, 2024; however, the form needed to be updated on or prior to November 4, 2025. Please remember the permission to transport children form must be updated annually. Once the form is updated and signed, please place the form in the child’s file. QRIS CONVERSATION TEMPLATE: We discussed section 3200 of child care rule. We also reviewed all three (3) pathways for the star rated license. We reviewed staff education, competency evaluation, the DCDEE modernization page, approved curriculum, and formative assessment, self-study, NCRLAP.org, and mentoring and training. You have decided to complete pathway two-classroom and instructional quality and would like to maintain your five-star license. I suggest you contact Down East Partnership for Children to help with mentoring, training, and technical assistance with curriculum and assessment. You would like to transition your license in July 2026. CRMINAL BACKGROUND CHECK-AMBCMS PORTAL: You have completed the ABCMS Moodle training; however, you have not connected your criminal back ground check (CBC) letter or your husband’s CBC letter to your ABCMS Roster. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119 . No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. Patricia Spruill, Owner/Operator, assisted with the visit. Five (5) children were observed in care today ranging from ages two to five years old. Children were observed completing diapering and hand washing routines, and playing with material/toys in the space designated for child care and interacting with the caregiver. You provided children materials to complete a drawing activity. Children were allowed to assist with distributing the materials. Children were cared for in a safe and nurturing environment. Currently this family child care home operates with a Five-Star License earning seven points in education standards, six points in program standards and one quality point for caring for no more than two infants under age one. The last annual compliance visit was conducted on January 25, 2025. The following violation of the child care requirements was documented today. Violation Number Comment Rule 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. Documentation was not available to verify the operator completed the ABCMS training as required. G.S. 110-90.2 & .2703(r) The violation documented must be corrected immediately. P. Spruill, Owner/Operator assisted with the visit. This center currently operates with a five-star license, earning seven points in the education component, six points in the program standard component and 1 quality point for serving no more than two infants under age one. The last annual compliance visit was completed on March 7, 2023. Five children were observed in care ranging from ages two to four years old. Children were engaged in free choice play with developmentally age-appropriate materials/toys in the space designated for child care, completing toileting and hand washing routines, eating snack, and interacting with the caregiver. Snack consisted on Cheezit crackers and milk. The following violations were documented today. Violation Number Comment Rule 925 Signed and dated statement which attests that a copy of the discipline policy was given to and discussed with a parent/guardian of each child prior to enrollment was not on file in the home. The last EPR review date documented was August 16, 2022. .1714(e ) The violations documented must be corrected immediately. EMERGENCY PREPAREDNESS AND RESPONSE (EPR): The last date documented for review of the emergency preparedness and response plan was August 16, 2022. You stated you must have forgot to complete the review this year. I advised you to complete the EPR review today in the Risk Management Portal and print page twenty-eight from the plan to add to the plan for verification of the review. You stated you would complete the review today. It was suggested scheduling an annual date for completing the EPR plan review to ensure it is completed as required. DISCIPLINE POLICY: A copy of the discipline policy was observed in the child’s file; however, it was not signed by a parent/guardian. You stated you were not aware the policy didn’t have a signature. You stated you would have the parent sign the discipline policy during drop off this afternoon. It was recommended reviewing children files upon receipt of documentation to ensure information is accurate and all required documentation has been signed. RATED LICENSE: We discussed steps that will be taken when the rated license assessment process resume. I reminded you that your program is in Cohort 1; therefore, this is your preparation year. You stated there have been no changes in your education status, so points would remain the same. Based on information provided and review of enrollment, you are still eligible to meet the quality point for serving no more than two infants under age one. You stated you wanted to wait until the required time to complete the Environment Rating Scale assessment. I reminded you the three-month self-assessment must be completed, and documentation provided when the rated license assessment is completed. Also, you should contact your local partnership for technical assistance as part of your preparation. Your previous environment rating scale assessment report can be used to guide when you begin preparing for the scales. I will contact you about a technical assistance visit in the future. RULE CHANGES EFFECTIVE JANUARY 2024 Training Modules Available in DCDEE Moodle The Child Care Commission adopted child care rule changes in January 2024. Changes relate to definitions; lead and asbestos for centers and family child care homes; building requirements for family child care homes; multi-unit child care centers; and criminal background checks. Consultants will assist as you begin to review and implement the changes, but please note, some of the rule changes may or may not impact your facility. An example is the rules in section .2600 for multi-unit child care center. These rules are specific to child care centers with multiple licensed centers within one building. Please ensure you are using the updated January 2024 rule book, and view information in the DCDEE Moodle (enroll if necessary).
