T. Avery, Administrator, was present and assisted with the visit. This center currently operates with a five-star license, earning seven points in education standards, six points in program standards and 1 quality point for offering a staff benefits package and infrastructure of parent involvement. The last annual compliance visit was completed on May 28, 2024. The last sanitation inspection was completed on December 12, 2024, earning a “Superior” classification. The last fire inspection was completed on May 7, 2025, and the center was approved for daytime care only. Fifteen (15) children ranging from ages one (1) to three (3) years old were observed in care today. Children were engaged in outdoor play, free-choice play, eating lunch, completing hand washing, and toileting/diapering routines, and interacting with teachers. Lunch consisted of chicken salad, mixed vegetables, peaches, bread, and milk. The following violations were documented. You stated the playground for the younger children was wet this morning due to excessive rain from yesterday; therefore, the group used the same playground. We discussed ensuring the outdoor play schedule is adjusted to ensure younger children utilize the playground at a different time from older children when both groups share the play area. I also recommend having staff providing care for children ages one year old supervise children closely to ensure they do not utilize the play structure because it is designated for children aged two to five years old. Grouping children according to age range such as placing one-year-olds with younger two-year old and older two-year old with three-year-old children. It was recommended you implement a tracking system to help with monitoring children’s ages that will support decisions regarding classroom groupings throughout the year. This approach will support both regulatory compliance and developmentally age-appropriate practices. Maintaining staff/child ratios including grouping of children helps ensure children are cared for in a healthy and safe environment. It also gives caregiver time and opportunity to provide individualized care to the children they serve. Caring for more children than allowed increases the chance for unwarranted behaviors and injuries. FIRE INSPECTION: The fire inspection for your program expired in April 2025. T. Avery, Administrator assisted with the visit. Eleven (11) children were observed in care today ranging from ages one to three years old. Children were engaged in morning activities, free-choice play, completing toileting, diapering, and hand washing routines, interacting with the caregiver and eating breakfast. Breakfast consisted of bagels, apples and milk. Currently this program operates with a five-star License earning seven points in education, six points in program standards and one quality point for offering a staff benefits package and infrastructure of parent involvement. Four tubes of toothpaste labeled “keep out of reach of children was observed in an unlocked drawer less than 5ft. from the finished floor and accessible to children. 10A NCAC 09 .0601(a) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. Plastic bags were in the refrigerator and unlocked cabinet accessible to children in Space #1. .0604(q) The violations documented were corrected during the visit. The toothpaste was removed from the drawer and placed on a shelf higher than 5ft from the finished floor. The plastic bags were removed from the refrigerator and made inaccessible to children. STAFF/CHILD INTERACTIONS: Children were observed serving and eating breakfast. Food was placed on the tables and children in Space #2 were allowed to serve themselves. Children were allowed to clean up their spaces and place their cups in the refrigerator independently. Staff interacted with children while eating. Younger children in Space #1 were constantly reminded and reassured they were in a safe space and visitors were their friends. Staff in Space #1 sat on the floor near children and assisted while eating breakfast. Children played in various interest areas using developmentally age-appropriate materials. Diaper changing was observed. The step for wiping the child and staff member’s hand with a clean wipe prior to putting on the clean diaper was skipped. We discussed ensuring staff complete the diaper changing process as required step by step. Staff can refer to diaper changing chart for reference to ensure all steps are followed. Children were cared for in a safe and nurturing environment. SAFE ENVIRONMENT: I informed you plastic bags should be stored in spaces inaccessible to children under the age of three. Also, items labeled “keep out of reach of children must be stored at least 5ft. from the finished floor. It was recommended reading labels of items brought into the classroom to ensure they are stored correctly. QUALITY RATING IMPROVEMENT SYSTEM: The Governor signed Senate Bill 425 (now Session Law 2024-34), Health and Human Services Omnibus Bill, which includes the Quality Rating Improvement System (QRIS) Modifications recommended to the Legislature by the NC Child Care Commission to update QRIS and create additional pathways for licensed child care facilities to earn star rated licenses. Hold Harmless provisions have been extended and star rated license assessments for licensed child care providers will be conducted by provider request only, until the QRIS rulemaking process has been completed. Star rated license assessments are still required for new child care programs. This legislation also provides a pathway for child care programs currently accredited through the approved accrediting bodies to move to a 3 or 5 star license if they are not already at this star level. The Clean Water for Carolina Kids program is expanding to the Clean Classrooms for Carolina Kids program. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. Rules 10A NCAC 41C .1001 through .1007 require all programs to be free of lead and asbestos hazards. RTI is coordinating all the lead in water, paint and asbestos testing and remediation.
