Your program currently operates with a Five Star Center License issued August 23, 2023. Restrictions on the license include a first shift capacity of seventy-three (73) children ages 0 through 12, meets enhanced ratios, and meets enhanced space. Restrictions were in compliance today. The last annual compliance visit was completed on November 13, 2025. A sanitation inspection was completed on August 28, 2025 with a “Superior” grade. A fire inspection was completed on August 6, 2025 and approved for daytime care. The program's compliance history was reviewed with the operator. Prior to the visit, the facility’s eighteen-month compliance history was ninety-one percent (91%). You, C. Faulk, owner/operator were present for the visit and available for consultation. There were sixty-one (61) children enrolled and fifty-two (52) children present. All licensed spaces were observed today. Age-appropriate materials were accessible to all children and spaces were arranged into activity areas. Infants in group 1 were playing on the floor, either sitting up or practicing tummy time, with a variety of grasping toys and soft toys. Another infant was preparing to be fed. Individual feeding schedules and safe sleep checks are being maintained. Young preschoolers in group 2 transitioned from outdoor play to a coloring activity and group time with songs and a weather discussion and then to lunch. Toddlers in group 3 were observed outdoors using a steering wheel toy, small slides, and dramatic play props. They then transitioned indoors for story time before lunch. Group 4 is the NC Pre-K classroom; they were observed transitioning from songs and a story to lunch. Preschoolers in group 5 were observed outdoors playing on wheeled toys and a small climber as well as ball play. They then transitioned indoors to lunch. Supervision and staff-child ratios were observed and found to be in compliance. Lunch consisted of chicken and pasta, peas and carrots, peaches, and milk. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your program uses Creative Curriculum. Two (2) new staff have been hired since the last visit, and their files were reviewed today. In addition, Health and Safety training requirements as outlined in Child Care Rule 10A NCAC 09 .1102 were monitored and in compliance today. During today's visit child care requirements monitored for compliance included supervision, staff/child ratio, CPR and First Aid, Criminal Record Checks, storage of hazardous substances and medications, approved space, posting of the license, and other applicable requirements. In the NC Pre-K classroom, ongoing assessments are stored electronically and were observed today. On February 27, 2026, a Rated License Request was completed for the NC Pre-K classroom. An ECERS-3 assessment was completed on March 19, 2026, with a score of 5.74. The facility summary report was first reviewed by phone on April 2, 2026, and again at today’s visit. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Reminders: The Emergency Preparedness and Response Plan must be reviewed annually, or when information in the plan changes, to ensure all information is current. The annual review for the EPR plan is due by the end of April 2026. A playground inspection and fire drill are due for the month of April. Your program currently operates with a Five Star Center License issued August 23, 2023. Restrictions include first shift care with a capacity of 73 children ages 0 to 12, meets enhanced ratios and meets enhanced space. The last annual compliance visit was completed on November 19, 2024. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety percent (90%) as of November 10, 2025. The sanitation inspection was completed on August 28, 2025 with a “Superior” classification. The fire inspection was completed on August 6, 2025 and your facility was approved for daytime care only. C. Faulk, administrator, was present for the visit. There were sixty-two (62) children enrolled and forty-eight (48) children present. There are four (4) indoor spaces and two (2) outdoor spaces approved for use. Space 2 has been divided into two separate classrooms. Age-appropriate materials were accessible to all children and spaces were arranged into activity areas. Children in spaces 2a and 2b were outside using the climber, looking for ladybugs playing with balls, and using ride-on toys. Children in space 3 were having story time with a Jack and the Beanstalk theme using picture props. Children in space 4 were having free play throughout the room, engaged in such activities as doll play, dinosaur play, reading books, and building a magnet tower. Infants in space 1 were on the floor playing and held by staff and were being fed and changed according to individual needs. Individual feeding schedules and safe sleep checks are being maintained. Supervision and staff-child ratios were observed and found to be in compliance. Lunch consisted of peanut butter on whole grain bread, corn, oranges, and milk. There were thirteen staff members employed at this facility. One (1) new staff and a percentage of veteran staff’s files were reviewed. A percentage of children’s files were reviewed. NC Pre-K monitoring: Space 3 is used for children enrolled in NC Pre-K. The NC Pre-K requirements in Section .3000 of the Child Care Rules were monitored for compliance today. Staff-child ratios and maximum group sizes were verified in compliance. Children attend the program from 7:30am-2:30pm meeting the instructional day requirement of6.5 hours. This NC Pre-K program uses Creative Curriculum and Brigance Screening, and Teaching Strategies Gold for their ongoing assessments. Developmental Screenings were completed and Health Assessments were collected. NC Pre-K staff information was verified in the NC Pre-K Plan and all staff members meet requirements for qualifications. A Site Monitoring visit has not yet been conducted by the contracting agency, however the tool submitted October 27, 2025 was reviewed today and items needing corrective action were not noted. Prior to the visit, the facility had an eighteen-month compliance history score of 96%. Owner/Administrator, C. Faulk, was present and available for consultation today. Four indoor spaces are approved for use by children; Space #2 is divided into two separate and distinct spaces using furniture and room dividers. Upon arrival, infants and a child one year of age in Space #1 were observed in individual sleeping and feeding routines as well as tummy time activities. Safe sleep practices for infants including documentation were monitored and in compliance today. Children one year of age in Space #2b were observed using connecting blocks, looking at books with their caregivers and playing with stuffed animals. Later, this group was observed on the smaller fenced in playground, using push toys and other portable gross motor equipment. Children two years of age in Space #2a were observed in free choice activities including building with blocks and playing in the dramatic play kitchen. This group was also observed engaged in a music and movement activity. In Space #4, children two and three years of age participated in a large group activity during which they discussed healthy habits, practiced sign language and talked about the letter “G.” Preschool children in Space #3 were observed drawing with markers or working with manipulatives. Lunch served consisted of ham or turkey sandwiches, green beans, fruit salad, and milk. Health and Safety training requirements as outlined in Child Care Rule 10A NCAC 09 .1102 were monitored and in compliance today. You may wish to delegate this task to a staff member if it helps. I also urged you to review ratio charts weekly for accuracy, especially when children have had birthdays. Technical assistance was also provided today regarding a combination truck slide/climber on the preschool playground which you stated was purchased by the NC Pre-K contracting agency. You stated your husband assembled it and you think he did not keep the warning labels or installation instructions. As discussed today, without that documentation it cannot be confirmed that the equipment is approved for commercial use or that it was installed correctly. For example, if installation instructions require it to be anchored, it would need to be anchored and placed over a sufficient amount of resilient surfacing based on the critical height. Today you stated you will probably just ask your husband to remove it from the playground and return it to the contracting agency; please let me know in your compliance letter and send a picture confirming it was removed. Technical Assistance Technical assistance was also provided today regarding a rescue inhaler recently brought in by the parent of a one-year-old child. As discussed, written permission from the parent to use the inhaler at your facility is required prior to administration; I suggested you keep a copy of the permission form with the inhaler and the action plan for easy access. I also advised that you may keep the inhaler unlocked as long as it is five feet off of the floor. Rated License Information Today I shared with you that the “hold harmless” state of rated license reassessments has been extended until the new Quality Rating Improvement System (QRIS) standards are put in place. This means that you will not be required to have a reassessment according to timelines previously discussed, and when your reassessment is due you will have options to choose from regarding how your star level is earned. When more information becomes available about the new QRIS standards, I may assist you in determining which option is best for your program. Please note any Infant Toddler Environment Rating Scales (ITERS) or Early Childhood Environment Rating Scales (ECERS) assessments conducted after February 1, 2025 will be the third editions, or the “3s.” The School Age Care Environment Rating Scales will continue to be used in school age classrooms. You stated your husband assembled it and you think he did not keep the warning labels or installation instructions. As discussed today, without that documentation it cannot be confirmed that the equipment is approved for commercial use or that it was installed correctly. For example, if installation instructions require it to be anchored, it would need to be anchored and placed over a sufficient amount of resilient surfacing based on the critical height. Today you stated you will probably just ask your husband to remove it from the playground and return it to the contracting agency; please let me know in your compliance letter and send a picture confirming it was removed. Technical Assistance Technical assistance was also provided today regarding a rescue inhaler recently brought in by the parent of a one-year-old child. As discussed, written permission from the parent to use the inhaler at your facility is required prior to administration; I suggested you keep a copy of the permission form with the inhaler and the action plan for easy access.
