This facility currently operates with a Five Star license issued April 5, 2023, earning 6 points in Program Standards, 5 points in Education and 1 Quality point. Restrictions on the permit include a first shift capacity of 29 children 3-12 years of age, meets enhanced space and meets enhanced ratios minus one in each age group. Restrictions were in compliance today. The program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled; this program uses The Creative Curriculum, Sixth Edition. The last annual compliance visit was conducted March 13, 2023. A Superior sanitation rating was obtained February 24, 2025. A satisfactory fire inspection was completed June 13, 2024; the facility was approved for day time care only. Prior to today’s visit, the facility had an eighteen-month compliance history score of 95%. Lead teacher, B. Walker, was present and available for consultation today. One indoor and one outdoor space are approved for child care use. Two groups of children are enrolled: a group of preschool children, and a group of school age children who attend before and/or after school as well as on teacher workdays and some holidays. Upon arrival, the preschool children were observed in group circle time. Shortly after my arrival they were dismissed to free choice activities including blocks, fine motor, books, and dramatic play. Ms. Walker and the cook, T. Cox were present when I arrived. Ms. Elliott arrived shortly afterwards and Ms. Cox resumed her responsibilities in the kitchen. Lunch served after outdoor time consisted of corn dogs, tater tots, peaches, potatoes, and milk. All staff members employed at least one year have completed the required Health and Safety trainings as outlined in Child Care Rule 10A NCAC 09 .1102. Staff members employed more than five years have repeated all topics as required. The program does not have any NC Pre K children enrolled this year. Child Care programs are expected to always achieve and maintain compliance with child care rules, and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The following violation was observed today and must be corrected immediately: Violation Number Comment Rule 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. A staff member hired May 2024 who was observed supervising children this morning did not have at least 16 hours of orientation completed. .1101(a) A compliance letter including detailed information about how all violations have been corrected must be received by March 12, 2025. Include any supporting documentation with your response (if applicable). Your letter will be used as verification that violations have been corrected and compliance is maintained; your written response is considered a legal document so must be accurate and truthful. If you email the compliance letter, it must be sent using the email address registered with DCDEE and include your facility name and ID number. If sufficient information is not received by the due date, a follow-up visit will be conducted. Technical Assistance: Each employee who is expected to have contact with children shall receive six clock hours of training in required topic areas within the first two weeks of employment and 16 hours of on-site orientation within the first six weeks of employment. Ms. Cox has an orientation worksheet that was signed; however, information of when each training took place and how long the training lasted was not included. Rated License: The “hold harmless” state of rated license reassessments has been extended until the new Quality Rating Improvement System standards are put in place. This means that you will not be required to have a reassessment according to the timelines discussed previously; however, you may request one voluntarily. Today I encouraged you to review information about the new standards as it becomes available and to plan on training your staff on the new assessment tools if you are interested in Environment Rating Scales assessments. This facility currently operates with a Five Star license issued July 22, 2024, earning 6 points in Program Standards, 7 points in Education and 1 Quality point. Restrictions on the permit include a first shift capacity of 29 children 3-12 years of age, meets enhanced space and meets enhanced ratios. Restrictions were in compliance today. The program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled; this program uses The Creative Curriculum, Sixth Edition. The last annual compliance visit was conducted March 13, 2024. A Superior sanitation rating was obtained January 10, 2024, and a satisfactory fire inspection was conducted June 13, 2024; the facility was approved for day time care only. Prior to today’s visit, the facility had an eighteen-month compliance history score of 97%. Lead teacher, B. Walker, and Owner/Administrator, M. Spivey, were present and available for consultation today. One indoor and one outdoor space are approved for child care use. Since the last visit, Ms. Spivey has changed the enrollment, and this facility no longer serves NC Pre-K funded children. There is currently one group of preschool children enrolled full time; between four and six school age children attend the facility in the early mornings only prior to catching the school bus. Upon arrival, Ms. Walker was the only staff member present and children were observed in rest time routines. They rested on appropriately spaced cots with individual linens. Lights were dimmed and soft music played. Ms. Spivey arrived at approximately 1:40 P.M, and a second teacher arrived back at the facility from a break at approximately 1:50 P.M. After rest time, children engaged in personal care routines prior to transitioning to the fenced in playground, where they played with various portable gross motor materials. Teachers supervised and interacted with children outdoors while Ms. Spivey prepared a supper of breaded chicken tenders, peas, peaches and milk. New staff members have not been hired since the Annual Compliance visit. Child Care programs are expected to always achieve and maintain compliance with child care rules, and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The following violations were observed today and must be corrected immediately: Violation Number Comment Rule 317 The staff/child ratios for children, two years and older, during naptime were not maintained by having at least one person in each room, visually supervising all children and the total number of required staff on the premises within calling distance of each room occupied by children. Upon arrival today, only one staff member was on the premises while nineteen children three and four years of age rested. .1801(b) 1756 Enhanced staff/child ratios and group sizes were not met. Based on a review of children's arrival and departure times and reported staff schedules, one staff member was present with more than ten children three to twelve years of age between 6:00 A.M and 8:00 A.M. each day this week. 10A NCAC 09 .2818 1842 Staff members and child care administrators who were counted in meeting the staff/child ratios concurrently performed food preparation or other duties that are not direct child care responsibilities. Per staff member report, she occasionally goes into the kitchen to prepare breakfast for service when she is counted in meeting staff/child ratios. .0713(a)(8) A compliance letter including detailed information about how all violations have been corrected must be received by September 26, 2024. Include any supporting documentation with your response (if applicable). Your letter will be used as verification that violations have been corrected and compliance is maintained; your written response is considered a legal document so must be accurate and truthful. If you email the compliance letter, it must be sent using the email address registered with DCDEE and include your facility name and ID number. If sufficient information is not received by the due date, a follow-up visit will be conducted. Technical Assistance Regarding Violations Cited As you selected to operate under voluntary enhanced staff-child ratios, these ratios must be followed at all times. When absent or late staff put you at risk of not maintaining ratios, you must have a plan to provide substitute staff so that ratios may be followed. On days when you are short staffed, you may need to tell parents they cannot drop off their children. Today you stated this is a new group of children and you didn’t realize how many of them are coming in before 8:00 A.M., when the second staff member typically arrives. Today I assisted you in reviewing your staffing plan alongside records of children’s arrival and departure times to determine if you need to make changes to your staff schedule. Since 15-17 children are now typically present before 8:00 A.M., I urged you to immediately start scheduling a second teacher earlier to maintain compliance. I also strongly urged you to keep a dry erase board by the front door where you can document the total number of children present, and change it as children arrive so you will always have an accurate count. Since breakfast must be prepared and served around 8:00 A.M. as well, it may benefit you to schedule a floater to come in from 7:00 A.M. to 9:00 A.M. to assist with ratios and breakfast. Regarding lunch routines, we also discussed that you must have enough staff members present to maintain ratios in the classroom and prepare and serve lunch, which will typically be three people based on attendance records reviewed today. If only one or two staff members are present at the facility, having one of them go into the kitchen to prepare lunch would result in noncompliance. Regarding supervision during rest time, today I reviewed Child Care Rule 10A NCAC 09 .1801(b) with you, which states “for groups of children aged two years or older, the staff/child ratio during nap time shall comply with the requirements of this Chapter if at least one person remains in the room, all children are visible to that person, and the total number of required staff are on the premises and within calling distance of the rooms occupied by children.” This means that two staff members must on the premises even when children are resting if the number present requires two staff members. Today you called a staff member at one of your other facilities and asked if she could start working at this facility from 7:00 A.M. to 9:00 A.M. to assist with ratios and breakfast preparation. You also stated you would schedule a third person for lunch time tomorrow since one of the regular staff members is still on vacation. Moving forward, I strongly recommend that you review staff schedules and arrival and departure times on a regular basis, as well as meal service and rest time routines with staff break schedules, so you can ensure full compliance with staff/child ratio requirements. You must continue to keep accurate records of children’s arrival and departure times as well as written records of staff arrival and departure times so I may verify correction of these violations. Rated License Information Today I shared with you that the “hold harmless” state of rated license reassessments has been extended until the new Quality Rating Improvement System (QRIS) standards are put in place. This means that you will not be required to have a reassessment according to timelines previously discussed, and when your reassessment is due you will have options to choose from regarding how points are earned. When more information becomes available about the new QRIS standards, I may assist you in determining which option is best for your program. Please note any Environment Rating Scales (ERS) assessments conducted after February 1, 2025 will be the third editions, so for your program the Early Childhood Environment Rating Scale- 3rd edition (ECERS-3) will be used if you select the pathway that includes this assessment. This facility currently operates with a Five Star license issued July 22, 2024, earning 6 points in Program Standards, 7 points in Education and 1 Quality point. Restrictions on the permit include a first shift capacity of 29 children 3-12 years of age, meets enhanced space and meets enhanced ratios. Restrictions were in compliance today. The program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled; this program uses The Creative Curriculum, Sixth Edition. The last annual compliance visit was conducted March 13, 2024. A Superior sanitation rating was obtained January 10, 2024, and a satisfactory fire inspection was conducted June 13, 2024; the facility was approved for day time care only. Prior to today’s visit, the facility had an eighteen-month compliance history score of 97%. Lead teacher, B. Walker, and Owner/Administrator, M. Spivey, were present and available for consultation today. One indoor and one outdoor space are approved for child care use. Since the last visit, Ms. Spivey has changed the enrollment, and this facility no longer serves NC Pre-K funded children. There is currently one group of preschool children enrolled full time; between four and six school age children attend the facility in the early mornings only prior to catching the school bus. Upon arrival, Ms. Walker was the only staff member present and children were observed in rest time routines. They rested on appropriately spaced cots with individual linens. Lights were dimmed and soft music played. Ms. Spivey arrived at approximately 1:40 P.M, and a second teacher arrived back at the facility from a break at approximately 1:50 P.M. After rest time, children engaged in personal care routines prior to transitioning to the fenced in playground, where they played with various portable gross motor materials. Teachers supervised and interacted with children outdoors while Ms. Spivey prepared a supper of breaded chicken tenders, peas, peaches and milk. New staff members have not been hired since the Annual Compliance visit. Child Care programs are expected to always achieve and maintain compliance with child care rules, and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The following violations were observed today and must be corrected immediately: Violation Number Comment Rule 317 The staff/child ratios for children, two years and older, during naptime were not maintained by having at least one person in each room, visually supervising all children and the total number of required staff on the premises within calling distance of each room occupied by children. Technical Assistance Regarding Violations Cited As you selected to operate under voluntary enhanced staff-child ratios, these ratios must be followed at all times. When absent or late staff put you at risk of not maintaining ratios, you must have a plan to provide substitute staff so that ratios may be followed. On days when you are short staffed, you may need to tell parents they cannot drop off their children. Today you stated this is a new group of children and you didn’t realize how many of them are coming in before 8:00 A.M., when the second staff member typically arrives. Today I assisted you in reviewing your staffing plan alongside records of children’s arrival and departure times to determine if you need to make changes to your staff schedule. Since 15-17 children are now typically present before 8:00 A.M., I urged you to immediately start scheduling a second teacher earlier to maintain compliance. I also strongly urged you to keep a dry erase board by the front door where you can document the total number of children present, and change it as children arrive so you will always have an accurate count. Since breakfast must be prepared and served around 8:00 A.M. as well, it may benefit you to schedule a floater to come in from 7:00 A.M. to 9:00 A.M. to assist with ratios and breakfast. Regarding lunch routines, we also discussed that you must have enough staff members present to maintain ratios in the classroom and prepare and serve lunch, which will typically be three people based on attendance records reviewed today. If only one or two staff members are present at the facility, having one of them go into the kitchen to prepare lunch would result in noncompliance. Regarding supervision during rest time, today I reviewed Child Care Rule 10A NCAC 09 .1801(b) with you, which states “for groups of children aged two years or older, the staff/child ratio during nap time shall comply with the requirements of this Chapter if at least one person remains in the room, all children are visible to that person, and the total number of required staff are on the premises and within calling distance of the rooms occupied by children.” This means that two staff members must on the premises even when children are resting if the number present requires two staff members. Today you called a staff member at one of your other facilities and asked if she could start working at this facility from 7:00 A.M. to 9:00 A.M. to assist with ratios and breakfast preparation. You also stated you would schedule a third person for lunch time tomorrow since one of the regular staff members is still on vacation. Moving forward, I strongly recommend that you review staff schedules and arrival and departure times on a regular basis, as well as meal service and rest time routines with staff break schedules, so you can ensure full compliance with staff/child ratio requirements. You must continue to keep accurate records of children’s arrival and departure times as well as written records of staff arrival and departure times so I may verify correction of these violations. Rated License Information Today I shared with you that the “hold harmless” state of rated license reassessments has been extended until the new Quality Rating Improvement System (QRIS) standards are put in place. This means that you will not be required to have a reassessment according to timelines previously discussed, and when your reassessment is due you will have options to choose from regarding how points are earned. When more information becomes available about the new QRIS standards, I may assist you in determining which option is best for your program. Please note any Environment Rating Scales (ERS) assessments conducted after February 1, 2025 will be the third editions, so for your program the Early Childhood Environment Rating Scale- 3rd edition (ECERS-3) will be used if you select the pathway that includes this assessment. A third staff member, the NC Pre-K assistant, arrived at 9:15 A.M. At 9:45 A.M., staff members reminded children it was time to clean up, and asked one of the children to help with this task as it was her assigned job. Next, Ms. Walker applied sunscreen and bug spray as needed to some of the children, and the assistant engaged the rest of the children in a color letter recognition activity as they waited for restroom routines to be completed. At 10:00 A.M., the entire group went to the fenced in playground, where they engaged in gross motor activities including using the merry-go-round and playing lawn darts. Children were also observed using magnifying glasses and bug catchers as well as drawing on a chalkboard. Lunch served after outdoor time consisted of pulled pork, peas, mixed fruit, whole grain roll and milk. All staff members employed at least one year have completed the required Health and Safety trainings as outlined in Child Care Rule 10A NCAC 09 .1102. Staff members employed more than five years have repeated all topics as required. NC Pre-K Site Monitoring This facility is approved for eleven NC Pre-K slots. Children in this program attend from 7:30 A.M. to 2:30 P.M., meeting the instructional day requirement of 6.5 hours. This NC Pre-K program uses Creative Curriculum, Brigance Developmental screening, and Teaching Strategies GOLD for their ongoing assessments. Health assessments including vision, hearing, and dental screenings were observed in selected NC Pre-K children’s files. Developmental screenings and evidence of on-going assessment were observed in the site monitoring tool notebook. The site monitoring tool itself was signed by Terri Ard, NC Pre-K Specialist with the contracting agency Columbus County Schools, on October 23, 2023. Items needing corrective action were not noted. When no longer needed by the child, or when the child withdraws from the program, all medications should be returned to the child’s parent/guardian or disposed of after an attempt to reach parent/guardian within 72 hours of completion of treatment. Today I also reminded you to review enrollment forms when parents turn them in to ensure all blank lines are filled out and all required information is included. Having accurate and complete information for all children will assist you in providing the best quality care. Remember to send a copy of the completed application form for C. You may also wish to move the larger vehicles outside or to another area so they do no interfere with block play. Rated License Information As discussed during the Routine Unannounced visit October 25, 2023, your program was assigned to “Cohort 3” in the “cohort model” of resuming rated license reassessments after June 30, 2024. As discussed today, 7/1/25-6/30/26 will be your “planning/preparation” year, and your next reassessment will be required between 7/1/26-6/30/27. As you typically have the Environment Rating Scales (ERS) conducted, we discussed that you may have the option to request ERS assessments during your planning year if you wish, or you may wait until your reassessment year. Today we also discussed that the way points are earned towards your star rated license may be different by the time your reassessment is due. I will share more information as it becomes available and be in touch as your planning year approaches. Regarding Education standards, it was verified today that you are maintaining compliance with five points, and discussed that you may eligible for seven points once you have earned your Level III administration credential. As discussed today, 7/1/25-6/30/26 will be your “planning/preparation” year, and your next reassessment will be required between 7/1/26-6/30/27. As you typically have the Environment Rating Scales (ERS) conducted, we discussed that you may have the option to request ERS assessments during your planning year if you wish, or you may wait until your reassessment year. Today we also discussed that the way points are earned towards your star rated license may be different by the time your reassessment is due. I will share more information as it becomes available and be in touch as your planning year approaches. Regarding Education standards, it was verified today that you are maintaining compliance with five points, and discussed that you may eligible for seven points once you have earned your Level III administration credential. It’s time to enroll your facility in the Clean Classrooms for Carolina Kids program to identify and eliminate exposure to lead and asbestos hazards in building infrastructure. This program is an expansion of Clean Water for Carolina Kids, which previously tested all child care centers in the state for lead in water at drinking and food preparation taps. As this is a legislatively mandated effort, facilities are required to meet rule requirements to have tap water tested every three years and to have inspections for lead paint and asbestos if applicable. This facility currently operates with a Five Star license issued April 5, 2023, earning 7 points in Program Standards, 5 points in Education and 1 Quality point. Restrictions on the permit include a first shift capacity of 29 children 3-12 years of age, meets enhanced space and meets enhanced ratios minus one in each age group. Restrictions were in compliance today. The program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled; this program uses The Creative Curriculum, Sixth Edition. The last annual compliance visit was conducted April 5, 2023. A Superior sanitation rating was obtained January 10, 2023, and a satisfactory fire inspection was conducted June 28, 2023; the facility was approved for day time care only. Prior to today’s visit, the facility had an eighteen-month compliance history score of 100%. Lead teacher, B. Walker, and Owner/Administrator, M. Spivey, were present and available for consultation today. Child Care Consultant April Lester was also present and assisted with the visit today. One indoor and one outdoor space are approved for child care use. Two groups of children are enrolled: a group of preschool children including some who are NC Pre-K funded, and a group of school age children who attend before and/or after school as well as on teacher workdays and some holidays. Upon arrival, the preschool children were observed in rest time routines. They rested on appropriately spaced cots with individual linens. Lights were dimmed and soft music played. Snack planned for after rest time consisted of milk and yogurt. Four new staff members have been hired since the Annual Compliance visit. Requirements were monitored and in compliance today. Child Care programs are expected to always achieve and maintain compliance with child care rules, and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The following violation was observed today and must be corrected immediately: Violation Number Comment Rule 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. A playground inspection has not been completed since April 2023. .0605(q) A compliance letter including detailed information about how all violations have been corrected must be received by November 8, 2023. Include any supporting documentation with your response (if applicable). Your letter will be used as verification that violations have been corrected and compliance is maintained; your written response is considered a legal document so must be accurate and truthful. If you email the compliance letter, it must be sent using the email address registered with DCDEE and include your facility name and ID number. If sufficient information is not received by the due date, a follow-up visit will be conducted. Technical Assistance Regarding Violations Cited As discussed, playground inspections are required monthly for the safety of the children. Today you stated you must have overlooked them at this facility since you are currently working between all three of your facilities. As you are the only one trained in Playground Safety, you must conduct one by the end of October and send it to me to verify compliance. Moving forward, I suggest you get an additional staff member trained so someone else can conduct the inspections if you are not able to get to them. You may seek out the training from Region 4 CCR&R or Smart Start of Brunswick County. Technical Assistance Technical assistance was provided today regarding NC Pre-K requirements. As discussed, you must ensure the NC Pre-K Contracting Agency approves L. Lewis as a long term substitute for your teacher assistant position. You must also ensure she completes 15 hours of on-going training. As discussed, B. Walker now only has to complete 8 hours of on-going training since she has taken the substitute lead teacher position. You must also ensure Ms. Walker receives the appropriate training to administer developmental screenings and conduct on-going assessments as required by the NC Pre-K program. NC Pre-K requirements will be monitored fully at your next Annual Compliance visit. Technical assistance was also provided today regarding enhanced space requirements. As discussed, Child Care Rules were revised July 1, 2023, and one of the changes was to the rule regarding enhanced space. Rule .2809 now states there must be “100 square feet outside space for each child using the outdoor learning environment at any one time.” Your new outdoor space capacity is 30 children at one time. Rated License Information Your facility’s three-year rated license reassessment was conducted in April 2023, which means that your program has been assigned to “Cohort 3” in the recently announced “cohort model” of resuming reassessments. As discussed today, 7/1/25-6/30/26 will be your “planning/preparation” year, and your next reassessment will be required between 7/1/26-6/30/27.
