C. Tate, Lead Teacher, assisted me with today’s visit. Your program currently operates with a five-star license, issued 10/30/24, earning 7 points in the education component, 6 points in the program standards component (meeting enhanced requirements and reduced ratios) and 1 quality point for 75% of teachers have an AAS or higher in ECE/CD. Your program was also monitored for compliance with implementing an approved curriculum, Creative Curriculum, as required for all four and five star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted 11/21/24. The sanitation inspection was completed 10/15/25 with a “Superior” classification. The last fire inspection was conducted 5/21/25 and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-two percent as of 11/10/25. We visited indoor and outdoor spaces. Children were engaged in indoor free play including the following areas: art, blocks, science, dramatic play, and sand play. The class transitioned to lunch. Proper handwashing was observed. After lunch the children washed hands and transitioned to rest time. Children rested on linen covered cots. The outdoor area includes stationary climbing structures, ride on toys, swings, balls, and sand area. Lunch was observed during today’s visit and included vegetable beef soup, grilled cheese sandwich, carrots with dip, peaches, and milk. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC Pre-K staff are lead teacher, C. Tate and teacher, D. Smith. A selection of files were monitored for completed health assessments and developmental screenings. The facility meets DPI requirements for transportation. The program uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress. Checkpoint assessments are conducted three times per year: at the beginning, middle and end of the school year. The first day of school was 8-12-25. The classroom operates from 7:45 am to 3:00 pm. Parent conferences are held three times per year. You stated that you communicate with families by utilizing newsletters, text message, along with daily notes that goes back and forth with reminders, and information for families and from families. The environment rating scale score received on 3/3/20 was 5.32. The following violation(s) were documented: Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). The qualification letter for Cheri Tate, Teacher expired on 6/22/25. CPR/First Aid: Knowing what to do in an emergency medical situation can mean the difference between life or death. Being trained in CPR and first aid can be invaluable when someone is in serious medical distress. Therefore, it is so important that all staff working directly with children have current CPR and First Aid Certification. Staff have 90 days from the day of hire to complete the required training. One staff fired on hired on 8/5/24 First Aid and CPR training expired 2/2025 and a new training was not on file. You stated you thought it didn’t expire until next year. The knowledge and ability to perform emergency measures on another person whose breathing or heart activity has stopped is critical to potentially saving a life. Documentation of the successful completion of pediatric first aid and demonstration of pediatric CPR skills in the facility must be available for review and is required within 90 days of employment and before the expiration date listed on each card. Additional comments: NCID: Did you know that if you do not login on any DCDEE platforms (e.g., Moodle, WORKS, CBC) for a period of 12 months, your account will be archived? T. Jordan, administrator, assisted me with today’s visit. Your program currently operates with a five-star license, issued 10/30/24, earning 7 points in the education component, 6 points in the program standards component (meeting enhanced requirements and reduced ratios) and 1 quality point for 75% of teachers have an AAS or higher in ECE/CD. Your program was also monitored for compliance with implementing an approved curriculum, Creative Curriculum, as required for all four and five star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted 1/4/24. The sanitation inspection was completed 10/25/23 with a “Superior” classification. The last fire inspection was conducted 8/15/24 and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was one-hundred percent as of 11/20/24. We visited indoor and outdoor spaces. Children enrolled in space 1 were engaged in group time reviewing letter sounds. The class transitioned to outdoor free play. Outside the children were running, climbing, riding tricycles, interacting with their peers. The staff positioned themselves to be able to supervise the children at all times. Staff moved around the playground interacting with the children. The playground included stationary equipment, basketball goal, ride-on cars, stepping stones, and playhouses. The storage shed includes more items for fine and gross motor play. Children rested on linen covered cots. Proper hand washing techniques were observed. Lunch was observed during today’s visit and included popcorn chicken, broccoli, black-eyed peas, applesauce, roll, and milk. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC Pre-K staff are lead teacher, C. Tate and teacher, D. Smith. A selection of files were monitored for completed health assessments and developmental screenings. The facility meets DPI requirements for transportation. The program uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress. Checkpoint assessments are conducted three times per year: at the beginning, middle and end of the school year. The first day of school was 8-17-24. The classroom operates from 7:45 am to 3:00 pm. Parent conferences are held three times per year. You stated that you communicate with families by utilizing newsletters, text message, along with daily notes that goes back and forth with reminders, and information for families and from families. The environment rating scale score received on 3/3/20 was 5.32.
