The facility has a compliance history of 93%. Upon arrival, I was greeted by the owner/operator, Teresa Perryman. There were six children present ages 1 – 4 years old. Children were observed eating lunch and napping. The last documented fire drill was December 2023. The last outdoor inspection was documented October 2023. Required items were posted. There were three (3) items of non-compliance observed during today’s visit. One item was corrected during the visit. Violation Number Comment Rule 102 Number of preschool children exceeded five children, including the provider's own preschool children. There were six preschool children in attendance. One child left during the visit. GS 110-91(7)(b) 923 Operator did not keep records of monthly fire drills, quarterly lockdown or shelter-in-place drills, incident reports, incident logs, accurate attendance, emergency numbers, emergency preparedness and response plan, field trip/transportation permission, pet vaccinations records and/or playground inspections monthly outdoor safety checks for a minimum of one year. The last playground inspection was documented October 2023. The last fire drill was documented December 2023 .1721 (e)(7) Violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 3/6/24, describing how and when the violation(s) were corrected and how compliance will be maintained in the future. This facility currently has a compliance history of 94%. I was greeted by the owner/operator, Teresa Pegram. A volunteer, Kaitlyn F., was present today. There were six children present ages one- nine years old. Children were observed reviewing site words, looking at books and playing with other small toys. They were also observed playing outside with a variety of toys and balls. I monitored indoor areas used by the children during this visit including the bathroom, the kitchen and napping room. The outdoor area was free of any visible hazards. Playground inspections and emergency drills for 2023 were not documented. A fire inspection was completed on 9/15/22. Lead water testing was completed on 5/3/23. I reviewed children’s files, there were two children’s files unavailable for review. I also reviewed the operator’s file and monitored current certificates for CPR, First Aid and ITS-SIDS training, and qualification letters for Mrs. Perryman and household member D. Perryman. I also monitored the file for the volunteer Kaitlyn F. She had a current qualification letter and other annual paperwork on file. You have completed Health and Safety trainings except the course on Maltreatment, it was not due this year so you are saving it for next year's on-going training. There were four items of non-compliance observed during today’s visit. Violation Number Comment Rule 905 Record of monthly fire drills, giving the date, time of day and length of time to evacuate, and operator's signature, were not available. There were no records for 2023. .1721 (e )(2) 920 Records were not made available for review. Children records for Riley B and Holden F. were not available for review today. The file for Holden was not available for review at the last annual visit. G.S. 110-91(9) 921 Operator did not maintain accurate daily attendance records including documentation of arrival and departure for all children in care, including the operator's own preschool children. The last attendance and arrival and departure was documented on 6/9/23. .1721(e)(6) 923 Operator did not keep records of monthly fire drills, quarterly lockdown or shelter-in-place drills, incident reports, incident logs, accurate attendance, emergency numbers, emergency preparedness and response plan, field trip/transportation permission, pet vaccinations records and/or playground inspections monthly outdoor safety checks for a minimum of one year. There were no records of lockdown or shelter-in-place drills for 2023. .1721 (e)(7) Violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 8/8/23, describing how and when the violation(s) were corrected and how compliance will be maintained in the future. Please remember your letter should include facility name, facility ID#, and original signature. I provided technical assistance regarding the Shaken Baby and Head Trauma policy. It must be signed by all parent with children 0-5 not just infant parents. Due to technical issues with my printer, I was unable to print a typed visit summary today or leave any copies of paperwork.
