    Name of Operation: CHILDREN'S CHOICE, INC. Facility ID: 4155180 Consultant: ELIZABETH CHILDRESS Operation Type: Center Case Number: Visit Date: 11/14/2024 Number Present: 30 Completed Date: 11/14/2024 Age: From 1 To 5 Total Minutes: 210 Time In: 10:00 AM Time Out: 01:30 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor compliance with child care rules. The last annual compliance visit was 12/19/2023. An Annual Compliance Monitoring Checklist was completed during this visit and a copy was emailed to the provider. I was greeted by Marsha Gossage & Mitzi McDaniel, Owner/Operators of the facility. I completed a walk-through of the facility and reviewed children and staff files and all other required documentation. There were thirty eight children present today. The staff were observed engaged in activities with the children and were positive in their interactions with the children. Toddlers were observed washing hands and preparing for lunch. The last fire inspection was completed on October 30, 2023. A sanitation inspection was completed on 4/9/2024 and the facility received a Superior rating. Emergency drills and playground inspections were current and documented monthly. Required documents were posted and current. There was one item of non-compliance observed during today’s visit. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last inspection was completed October 30,2023. The provider has contacted the inspector to schedule an appointment. 10A NCAC 09 .0304(a) The violation documented above must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 11/27/2024, describing how and when the violation was corrected and how compliance will be maintained in the future. Please remember your letter should include facility name, facility ID#, and original signature. -I reminded you of the five-year requirement to take the Health and Safety Trainings. You and the staff are in the process of completing on-going training for this year. -Criminal background checks- A CBC must be completed every five years. Submissions can be made six months before the expiration date. To complete online criminal background check applications, go to www.ncchildcare.nc.gov/general/dhhscrc.asp. If you have any questions regarding the criminal background check process, dial 1-800-859-0829 and ask for the criminal background unit. All staff had updated qualification letters. I reminded you that two of the staff would be expiring soon and would need to update information. You showed me information documenting the process is being completed.
