The program currently operates with a Three-Star Child-Care License that was issued on December 3rd, 2024. You, Ms. T. Reed, assisted me with the visit. The home's compliance history was reviewed with the operator. The program’s compliance history was ninety seven percent (97%) as of 6/3/25. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. A walk-through of the facility was completed today; all indoor and outdoor areas were monitored. I observed six (6) children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. The children at the family child care home were engaging in free play, and outside time. The caregiver was interacting and meeting the developmental needs of the children. The following items were observed in compliance during today's visit: license posted, and restrictions followed, summary of law posted, attendance taken daily, emergency medical care plan, first aid poster posted, EPR plan, daily schedule, activity plan, storage of hazardous materials and substances, CPR/First Aid and SIDS training completed as required, and Criminal Record Checks completed as required. The provider’s Daughter C. Gill is currently on a provisional until June 12th, 2026. The last fire drill was conducted on 5/18/26. The last shelter-in-place and lock down drills were conducted on: 3/24/26. The last Outdoor inspection was completed on 5/17/26. The provider’s file and one newly enrolled child’s file was monitored during today’s visit. Transportation requirements were also monitored during today’s visit. Two violations were cited during today’s visit. The provider asked questions to ensure compliance. (i) The family child care home operator and staff members shall complete a professional development plan within one year of employment and at least thereafter. The plan shall: (1) document the individual's professional development goals; (2) be appropriate for the ages of children in their care; (3) include the continuing education, coursework or training needed to meet the individual's planned goals; (4) be completed by the operator and staff member in a collaborative manner; and (5) be maintained in their personnel file. Sample professional development plan templates may be found on the Division's website at http://ncchildcare.nc.gov/providers/pv_provideforms.asp. Another form may be used other than the sample templates provided by the Division as long as the form includes the information set forth in this Rule. (j) Each family child care home operator shall have a record of training activities in which each staff member participates, including copies of training certificates or official documentation provided by the trainer. That record shall include the subject matter, topic area in G.S. 110-91(11), training provider, date provided, hours, and name of staff who completed the training. This documentation shall be on file and current. 10A NCAC 09 .1714 EMERGENCY PREPAREDNESS AND RESPONSE (e) The operator shall review the Emergency Preparedness and Response Plan annually, or when information in the plan changes, to ensure all information is current. 10A NCAC 09 .1721 REQUIREMENTS FOR RECORDS (a) The family child care home operator shall maintain the following health records for each enrolled child, including his or her own child(ren) who are not school-age: (1) a copy of the child's health assessment as required by G.S. 110-91(1); (2) a copy of the child's immunization record; 10A NCAC 09 .1718 REQUIREMENTS FOR DAILY OPERATIONS 8) A written activity plan that shall: (A) Include activities intended to stimulate the following developmental domains, in accordance with North Carolina Foundations for Early Learning and Development, available on the Division's website at https://www.ncchildcare.ncdhhs.gov//pdf_forms/nc_foundations.pdf: (i) emotional and social development; (ii) health and physical development; (iii) approaches to play and learning; (iv) language development and communication; and (iv) cognitive development. (B) Identify activities that allow children to choose to participate with the whole group, part of the group, or independent of the group; (C) Reflect that children have at least four different activities daily, at least one of which is outdoors, if weather conditions permit, as specified in G.S. 110-91(12) as follows: (i) art and other creative play; (ii) children's books; (iii) blocks and block building; (iv) manipulatives; and (v) family living and dramatic play. (D) Provide materials and opportunities at least weekly, indoors or outdoors, for the following: (i) music and rhythm; (ii) science and nature; and (iii) sand and water play. (9) A clean and open area that allows freedom of movement shall be available, both indoors and outdoors; and (10) Operators who provide care to school-age children shall provide a balance of activities appropriate to the age, needs and interests of the school-age children. 10A NCAC 09 .2703 CRIMINAL HISTORY RECORD CHECK REQUIREMENTS FOR CHILD CARE PROVIDERS (f) Provisional child care providers are eligible for employment at a child care facility and are eligible to reside in a family child care home, nonlicensed home, or child care center in a residence and shall be counted in staff/child ratio. Provisional child care providers shall be supervised at all times by an individual who received a qualifying result on a criminal background check within the past five years and may not be left alone with children. Child care facilities found to be in violation of this Paragraph may be issued an administrative action up to and including revocation of their child care license or notice of compliance in accordance with Section .2200 of this Chapter. (g) When applying for provisional status, the applicant shall complete and submit any documents specific to their former state(s) of residence that are necessary to complete the out-of-state portion of their background check. When requested by the Division, the applicant shall submit a copy and proof of submission of the documents necessary to complete the out-of-state portion of their background check. The Division shall provide to the applicant the necessary information to make these requests, if applicable. (h) Within 45 days of issuing a provisional status to an applicant, the Division shall issue a qualification letter to a provisional child care provider if no criminal background check results that would disqualify the applicant are returned or if there is no response to the applicant's or the Division's request for the state sex offender registry check, the state abuse and neglect registry, or the state criminal history check from the state or states in which the provisional provider currently resides or has resided at any time during the preceding five years. § 110 91. Mandatory standards for a license (1) Each child shall have a health assessment before being admitted or within 30 days following admission to a child care facility. The assessment shall be done by: (i) a licensed physician, (ii) the physician's authorized agent who is currently approved by the North Carolina Medical Board, or comparable certifying board in any state contiguous to North Carolina, (iii) a certified nurse practitioner, or (iv) a public health nurse meeting the Departments Standards for Early Periodic Screening, Diagnosis, and Treatment Program. However, no health assessment shall be required of any staff or child who is and has been in normal health when the staff, or the child's parent, guardian, or full time custodian objects in writing to a health assessment on religious grounds which conform to the teachings and practice of any recognized church or religious denomination. The program currently operates with a Three-Star Child-Care License that was issued on December 3rd, 2024. You, Ms. T. Reed, assisted me with the visit. The home's compliance history was reviewed with the operator. The program’s compliance history was ninety seven percent (97%) as of 6/3/25. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. A walk-through of the facility was completed today; all indoor and outdoor areas were monitored. I observed six (6) children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. The children at the family child care home were engaging in free play, and outside time. The caregiver was interacting and meeting the developmental needs of the children. The following items were observed in compliance during today's visit: license posted, and restrictions followed, summary of law posted, attendance taken daily, emergency medical care plan, first aid poster posted, EPR plan, daily schedule, activity plan, storage of hazardous materials and substances, CPR/First Aid and SIDS training completed as required, and Criminal Record Checks completed as required. The provider’s Daughter C. Gill is currently on a provisional until June 12th, 2026. The last fire drill was conducted on 5/18/26. The last shelter-in-place and lock down drills were conducted on: 3/24/26. The last Outdoor inspection was completed on 5/17/26. The provider’s file and one newly enrolled child’s file was monitored during today’s visit. Transportation requirements were also monitored during today’s visit. The program currently operates with a Three-Star Child-Care License that was issued on December 3rd, 2024. You, Ms. T. Reed, assisted me with the visit. The home's compliance history was reviewed with the operator. The program’s compliance history was ninety seven percent (97%) as of 6/3/25. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. A walk-through of the facility was completed today; all indoor and outdoor areas were monitored. I observed six (6) children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. The children at the family child care home were engaging in free play, and outside time. The caregiver was interacting and meeting the developmental needs of the children. The following items were observed in compliance during today's visit: license posted, and restrictions followed, summary of law posted, attendance taken daily, emergency medical care plan, first aid poster posted, EPR plan, daily schedule, activity plan, storage of hazardous materials and substances, CPR/First Aid and SIDS training completed as required, and Criminal Record Checks completed as required. The provider’s Daughter C. Gill is currently on a provisional until June 12th, 2026. The last fire drill was conducted on 5/18/26. The last shelter-in-place and lock down drills were conducted on: 3/24/26. The last Outdoor inspection was completed on 5/17/26. The provider’s file and one newly enrolled child’s file was monitored during today’s visit. Transportation requirements were also monitored during today’s visit. Provisional child care providers shall be supervised at all times by an individual who received a qualifying result on a criminal background check within the past five years and may not be left alone with children. Child care facilities found to be in violation of this Paragraph may be issued an administrative action up to and including revocation of their child care license or notice of compliance in accordance with Section .2200 of this Chapter. (g) When applying for provisional status, the applicant shall complete and submit any documents specific to their former state(s) of residence that are necessary to complete the out-of-state portion of their background check. When requested by the Division, the applicant shall submit a copy and proof of submission of the documents necessary to complete the out-of-state portion of their background check. The Division shall provide to the applicant the necessary information to make these requests, if applicable. (h) Within 45 days of issuing a provisional status to an applicant, the Division shall issue a qualification letter to a provisional child care provider if no criminal background check results that would disqualify the applicant are returned or if there is no response to the applicant's or the Division's request for the state sex offender registry check, the state abuse and neglect registry, or the state criminal history check from the state or states in which the provisional provider currently resides or has resided at any time during the preceding five years. § 110 91. Mandatory standards for a license (1) Each child shall have a health assessment before being admitted or within 30 days following admission to a child care facility. (g) When applying for provisional status, the applicant shall complete and submit any documents specific to their former state(s) of residence that are necessary to complete the out-of-state portion of their background check. When requested by the Division, the applicant shall submit a copy and proof of submission of the documents necessary to complete the out-of-state portion of their background check. The Division shall provide to the applicant the necessary information to make these requests, if applicable. (h) Within 45 days of issuing a provisional status to an applicant, the Division shall issue a qualification letter to a provisional child care provider if no criminal background check results that would disqualify the applicant are returned or if there is no response to the applicant's or the Division's request for the state sex offender registry check, the state abuse and neglect registry, or the state criminal history check from the state or states in which the provisional provider currently resides or has resided at any time during the preceding five years. § 110 91. The home's compliance history was reviewed with the operator. The program’s compliance history was ninety seven percent (97%) as of 6/3/25. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. A walk-through of the facility was completed today; all indoor and outdoor areas were monitored. I observed six (6) children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. The children at the family child care home were engaging in free play, and outside time. The caregiver was interacting and meeting the developmental needs of the children. The following items were observed in compliance during today's visit: license posted, and restrictions followed, summary of law posted, attendance taken daily, emergency medical care plan, first aid poster posted, EPR plan, daily schedule, activity plan, storage of hazardous materials and substances, CPR/First Aid and SIDS training completed as required, and Criminal Record Checks completed as required. The provider’s Daughter C. Gill is currently on a provisional until June 12th, 2026. The last fire drill was conducted on 5/18/26. The last shelter-in-place and lock down drills were conducted on: 3/24/26. The last Outdoor inspection was completed on 5/17/26. Gilette Parker, Child Care Licensing Consultant accompanied me during today's visit. We observed four children present upon our arrival engaged with the substitute caregiver Ms. Robin Love. Mrs. Reed arrived back to the facility after our arrival, she was heading to a scheduled dentist appointment. The temporary license was issued to this family child care home on 12/21/23 and expires on 6/21/24. The Verification of Required Information for Operator and Additional Caregiver form was completed during today's visit. The last fire drill and outdoor play inspection was completed on 5/1/24. In addition, the provider will complete the lockdown or shelter in place once she completes her EPR training prior to December 21, 2024. Once completed, we informed Mrs. Reed that the EPR plan needs to be completed in the risk management portal within four months after the completion of the training. The provider does not provide transportation at this time but informed us that she will be interested in providing transportation in the future. We referred her to review the NC Child Care Rules regarding transportation. The license was posted along with the NC Summary of the Law, Emergency Number and Emergency Procedures. A first aid reference sheet and daily schedule was available for review. Attendance records have been maintained since opening. Keep this information on file for at least one year. COMPLIANCE HISTORY: Prior to today’s visit, the 18-month compliance history for this facility was 100%. In regards to the Quality Point, Mrs. Reed has decided to choose the option of serving NMT two infants under the age of one.
