Amy Matthews child care health consultant was present during today's visit. Currently this facility operates with a Five (5) Star license that was issued February 7thth, 2020. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety eight percent (98%) as of 9/2/25. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. During today’s visit, a partial assessment of applicable Child Care Requirements was conducted. I monitored your program for compliance with applicable child care requirements including compliance with requirements located in Child Care Rule Section .3000 in classroom 142, where children participating in the NC Pre-K program are cared for. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. Ms. Terry and Ms. Gresham assisted me with today’s visit. Children throughout the facility were participating in group time, free play in activity areas, transitions, and personal care routines. The caregivers were interacting and meeting the developmental needs for each of the children. The last sanitation inspection was 10/25/24. The last fire drill was conducted on 8/19/25, no children were present. The last fire inspection was 9/2/25. Shelter and lock down drill will be conducted 9/3/25. Playground inspection was conducted on 8/12/25. Two violations were cited during today’s visit. One was corrected during today’s visit Technical assistance was given to the provider to ensure compliance. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection was conducted on 8/26/24. During today's visit the fire Marshall came out and did a inspection. 10A NCAC 09 .0304(a) 716 All stationary equipment, more than 18 inches high, was not installed over protective surfacing. One piece of equipment that is more than 18 inches high was installed over protective surfacing however it was not the required depth. This has been documented on the playground check list. The mulch should be delivered within the next few weeks. .0605(j) The violation(s) documented must be corrected immediately. On or before 9-16-25, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Also, please provide information regarding how the facility will maintain compliance in the future. Please be aware that any information submitted by you is considered legal documentation. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The operator shall submit the original of the approved annual fire inspection report to the Division within one week of the inspection visit on the form provided by the Division. Loose-fill surfacing materials used to provide impact absorption beneath play equipment should be checked frequently to ensure surfacing is of sufficient depth and has not shifted or displaced significantly, especially in areas under swings and slide exits. Missing or displaced loose-fill surfacing should be raked back into proper place or replaced so that a constant depth is maintained throughout the playground. Ensure that all new staff members’ education information is current in the W.O.R.K.S. database. Each staff member must register with the WORKS system as soon as they are hired at any location. Education evaluations can take some time to process so beginning the process early will help to ensure we have the accurate information needed when it is time to complete the rated license. The new QRIS options regarding the three Pathways are online. Please go to our website DCDEE to become knowledgeable about the changes that came into effect July 1, 2025. The center's compliance history was reviewed with the operator. The program’s compliance history was one hundred percent (100%) as of 2/10/25. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. There were seventeen children present during today’s visit. Staff files and (ten percent of enrollment) children files were reviewed today. Fire inspection (8/26/24) remains current. The sanitation inspection remains current (10/25/24). The last playground inspection was completed and documented on 2/4/25. The last fire drill was conducted and documented on 1/31/25. The shelter and lock down drills were conducted and documented on 1/24/25. One violation was cited during today’s visit; however, the provider did ask questions about the topics below to ensure the facility stays in compliance. Protective surfacing shall be either: (1) loose surfacing material, including wood mulch, double shredded bark mulch, uniform wood chips, fine sand, coarse sand, and pea gravel, except that pea gravel shall not be used if the area will be used by children under three years of age. Footings which anchor equipment shall not be exposed. Protective surfacing shall be either: (1) loose surfacing material, including wood mulch, double shredded bark mulch, uniform wood chips, fine sand, coarse sand, and pea gravel, except that pea gravel shall not be used if the area will be used by children under three years of age. Loose surfacing material shall not be installed over concrete; The depth of the loose surfacing material shall be based on the critical height of the equipment, which is defined as the maximum height that a child may sit, or stand, as follows: (1) equipment with a critical height of 5 feet or less shall have 6 inches of loose surface materials; (2) equipment with a critical height of more than 5 feet, but less than 7 feet, shall have 6 inches of loose surfacing material, except for sand; (3) equipment with a critical height of 7 feet to 10 feet shall have 9 inches of any of the loose surfacing material, except for sand Agency Information The North Carolina Child Care Rules and Laws are on the Division’s website.
