A checklist was used to note the requirements I monitored today. This facility currently operates with a three-star license issued 9/20/24. This facility has a 94% compliance score as of 5/4/26. The last annual compliance visit was on 5/23/25. Upon my arrival, I was greeted by Kristi Spruill. I informed Ms. Spruill, Administrator, of the purpose of today’s visit. Ms. Spruill gave me the necessary paperwork I needed to complete today’s visit. The last fire drill was conducted 4/15/26. The last emergency drill(Shelter-in-place) was conducted on 4/22/26. The last playground inspection was 4/21/26. The last fire inspection was 10/7/25 and is attached to this report. Thirty-six (36) children were present during today’s visit. I observed the indoor and outdoor spaces. I observed both staff/child ratio and supervision and found both to be in compliance. During today’s visit, I observed children playing in centers, reading a book at group time, infants being fed in high chairs and playing outside. All medication and hazardous items were out of the children's reach. The paperwork for medication was complete. Staff and children’s files were monitored during today’s visit. Five children files were reviewed. The staff and training worksheets were reviewed during today’s visit. Your signature on these forms will serve as verification of the information provided is accurate and complete. There were three violations cited during today’s visit. Violation Number Comment Rule 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. One child did not have a health assessment in their file. GS110-91(1) 1323 Each child was not immunized as per Article 6 of Chapter 130A and an immunization record was not on file before or within 30 days after admission. One child did not have an immunization record in their file. 10A NCAC 09 .0302(d)(2) 1867 The depth of the loose surfacing was not based on critical height of the equipment. More mulch is needed under the slides and around the swing fall zones. .0605(k)(1-4) To maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. Please send me a correction letter by email stating how the violations were corrected and how you plan to maintain compliance in the future. It must be submitted to me by 5/19/26. Training hours will be recorded annually and health and safety trainings will be conducted every five years. Most staff had recently participated in CPR/First Aid and It’s SIDS. We discussed the Southwestern Child Development site and that CEUs can be obtained through this organization to meet desired star levels. Staff files: Record keeping is one of the most time-consuming tasks associated with administering a child care program. It is an important part of licensure. Review staff files often to ensure the program is maintaining accurate licensing records. Organization and completeness of records is important to ensure easy access to information in emergency situations. We discussed making sure that you develop a record keeping system for staff and children’s records that promotes organization and efficiency. Administrative staff must allow time to review all necessary documentation prior to employment of staff to ensure accuracy. As an administrator, it is your responsibility to review all employment documentation and verify that required documents are received within the specified time frame. Using a checklist to check and date receipt of documents will help you track employment documents. A staff file checklist is available on the DCDEE website under the Provider tab and provider documents links. Please refer to the Staff and Training Worksheets to continue to keep all staff members current with their upcoming due dates for CPR/First Aid training, child maltreatment training, and health and safety training requirements. EQUiPD The EQUiPD website has many webinar trainings that can be completed online for no cost and have been approved by NCDCDEE. Linking Criminal Background Checks – Staff need to log into the Criminal Background Check Portal on the DCDEE website and link to the facility license number. It is important to do this so staff at DCDEE can contact the center if a random criminal history check uncovers a staff member with a criminal charge or conviction working in a child care facility. It is also a way to for staff at the CBC Unit to keep you updated of the status of out-of-state checks if a new employee has a Provisional Qualification. Mary McMillan, Lead Consultant, accompanied me on the visit. We were greeted by Christie Hill, Director. This center currently operates with a Three Star License since November 20, 2019. A sanitation inspection was conducted on October 9, 2025. This facility received a superior classification and six (6) demerits. Monthly fire drills have been documented as well as quarterly disaster drills. Playground inspections have not been completed since May 2025. A satisfactory fire inspection was completed on October 7, 2025. This facility’s corporation status was verified current and active on the NC Department of the Secretary of State on October 27, 2025. I verified the lead in water was up to date as of August 28, 2024 and the facility is exempt from lead-based paint and asbestos testing. Ms. Hill accompanied me on a walk-through of the center and the playgrounds. I monitored staff/ child ratios and supervision as I walked throughout the facility and found both in compliance. Children were participating in center activities in all classrooms. All the classrooms were neat and organized. The playground had lots of different activities for the children. More ground covering was needed around the large climber. There was three violations cited during today's visit. Violation Number Comment Rule 847 Parent's medication authorization did not include required information. In the two year old classroom the parent's medical authorization to apply diaper creams had expired. 10A NCAC 09 .0803(4)(6-9) 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. Monthly playground inspections had not been conducted since May 2025. The Director completed an inspection for October 2025 during the visit. .0605(q) 1867 The depth of the loose surfacing was not based on critical height of the equipment. The ground covering (mulch) under the large climber did not meet the depth requirements. .0605(k)(1-4) To maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. This is the third temporary visit conducted since the Change of Ownership occurred on April 2, 2024. Christie Hill, Director accompanied me during today's visit. I conducted a walk-through and observed children preparing for lunch, eating lunch and napping. All of the children were being cared for in an appropriate manner. During today's visit I observed the following items: Supervision, Staff/Child Ratio, age appropriate activities, program records, items posted and there were no new staff files since the last visit. The last fire drill and playground inspections were recorded on 7/23/24. There were no drills or inspections recorded for August 2024. Ms. Hill stated that they would be done today and a second time in September for this months drill and inspection. The playground balance beam is rusting in multiple places and creates a hazard. The menu for today was not current, the main menu was corrected to reflect today's meal. We discussed that all menus need to be changed if the menu is updated. It is required to post one menu, you do not have to have one in every classroom. In the room with one year old children, the staff member had a bottle with soda brand on it sitting on the counter. We discussed modeling appropriate eating behaviors. The following violations were cited during today's visit: Violation Number Comment Rule 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. The menu for today was not what was posted. This was corrected during the vsiit. 10A NCAC 09 .0901(b) 705 Equipment and furnishings were not sturdy, stable and free of hazards. The outdoor balance beam is rusted in multiple places. .0601(c) 805 Fire drills were not practiced monthly and/or the drill record was incomplete. The last fire drill was recorded on 7/23/24. .0604(t); .0302(d)(5) 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. The last playground inspection was completed in July 2024. .0605(q) 1792 Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. One staff member had a soda bottle on the counter of the classroom. This was corrected during the visit. .0901(i) We discussed the following items of consultation: -Rated license points and how they are calculated. -What it takes to maintain a three star license without having the Environmental Rating Scale (ERS) completed. 2 points in Program requirements, 4 points in education, 1 quality point, for 7 points. A three star license is 7-9 points. -Right now you are under the hold harmless status, therefore you are under the previous owners stars, until the hold harmless status is no longer valid. However you must meet ALL minimum requirements, including education for staff. All lead teachers must enroll in EDU 119 by the end of September 2024 and have it completed by September 2025. -We reviewed the qualifications for 4 points for a director and a lead teacher. -We discussed what it means to use enhanced space and that you are meeting minimum ratios. Reminders: -Cots/mats/Cribs must either have a solid barrier between them or be at least 18 inches apart and children's hooks must be either 12 inches apart or further if the coats and items touch each other. We discussed the following items of technical assistance: -Rust is a hazard and cannot be on equipment of any items where children are. -Fire drills and playground inspections must be conducted monthly. You must use the alarm system for the fire drills and record the time for when everyone has exited the building. -Menus can be repeating as long as they are marked for the appropriate weeks, and no consecutive two weeks have the same food served. If a menu change has taken place the menu should be updated as soon as the change is known. You must correct your violations immediately and you have to send me a compliance letter stating how the violations are corrected before 9/17/24.
