The previous Annual Compliance Visit was completed March 04, 2026, and prior to today’s visit, the eighteen-month compliance history score was at 98%. This program currently operates with a Four -Star License issued March 06, 2020. The License was based on the program earning four points in education standards, seven points in program standards, and one quality point by choosing to meet a programmatic option for: Has reduced group size by one in each group from the seven-point level as required in Rule .2818 (c). The information on record with DCDEE was verified as correct by Ms. Katishia Graham, Lead Teacher, and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of Superior completed September 3, 2025. Fire Classification of Approved completed September 02, 2025. Ms. Graham was present and accompanied me while observing the indoor areas. Upon arrival, the children were resting on their cots. After the children finished with rest time, the children cleaned up their things, and completed toileting and handwashing routines. There were eight (8) children present. All children were supervised. Snack served was applesauce, fruit cups, and juice. There were violations observed for supervision and staff to child ratio. There is a concern that children are not being adequately supervised and staff to child ratios are not being followed. Regarding the concern that children are not adequately supervised, observations and interviews were conducted. The concerns were discussed with Ms. Graham, teacher. Interviews were conducted with the principal, one staff member, and one public school staff member. During observations, the staff members conducted a headcount before leaving the classroom. On the day of the incident, there was one (1) teacher with a group of fifteen (15) children. The ages of the children were 4-5 years of age. The incident occurred at 09:56 am. During transitioning out of the classroom, the staff member (A) stated that she conducted a head count and left out of the classroom to give the children a tour of the kindergarten classroom. It was stated that one (1) Teacher (B) took one child to the room next to their classroom, to allow the child to complete the required testing for the NCPK requirements. It was stated that all children will need to be tested prior to the end of the school year which ends on May 22, 2026. The NCPK testing involves the children speaking into a microphone and requires a quiet area. Both staff members stated that they always maintain proper staff to teacher ratio. On the day of the incident, Teacher (B) stepped out of the classroom to help the child with a testing requirement for the NCPK classroom. The staff member (A) was out of proper staff to child ratio for twenty (20) minutes. The child was left unsupervised for twenty (20) minutes. It was stated that the child left the group and went to the bathroom. It was also stated that the child may have gone to look for his brother. His brother was with the Teacher (B) in the classroom next door. The video footage shows the child leaving out of the classroom, and the child proceeded to the hallway, through the media center, and then the child walked out of the doors to exit the building, to the playground area. The child is seen on the video looking around on the playground and then proceeded to the door to “tap” on the door. The child was outside of the building from 9:59 am – 10:16 am. At 10:16 am, the media teacher employed by Cumberland County Schools, stated that she heard tapping sounds and she received the child. She stated that the child was not upset. The media teacher proceeded down the hallway to locate the teacher. The teacher and the child were reunited at 10:21 am. Regarding the concern that children are not maintaining staff to child ratios, observations and interviews were conducted. The concerns were discussed with Ms. Graham, teacher. Interviews were conducted with the principal, one staff member, and one public school staff member. During observations, the staff members conducted a headcount before leaving the classroom. The child was retrieved by the "media teacher", employed by Cumberland County Schools. The staff member heard the child tapping on the outside door. The "media teacher" proceeded to locate the staff member, and at 10:21 am, the staff member received the child from the "media teacher". This was corrected during the visit on May 6, 2026, all children were adequately supervised. .1801(a)(1-5) 1775 NC Pre-K program staff/child ratios and group sizes for the number and ages of children in care were not met (1/10 with maximum group size 20). A classroom of 10 or less did not have at least one teacher.NC Pre-K program staff/child ratios and group sizes were not met. It was confirmed during interviews, that on April 30, 2026, there were fifteen children with one teacher. The ages of the children were fours and fives. This was corrected the staff/child ratios were met. .3009 Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The violation observed and documented today may impact the compliance history score. The violation observed and documented today was corrected during today’s visit. A compliance letter is not required. Facility Specific TA/Consultation/Reminders: Administrative Action: Due to a substantiated complaint, an administrative action may be warranted. Corrective Action: It was stated that the administrator provided a conference with the staff members and reviewed the policy. The staff members are required to conduct head counts during each transition. Supervision: A violation was cited today for supervision. It was confirmed during staff interviews that a child was left in the classroom, and on the playground unsupervised. Children need to be supervised at all times. It is best practice for teachers to make sure to use the face to name sheet to help use as a tool to ensure that all children are accounted for. The staff members will need to make sure to conduct head counts during each transition. Refer to Child Care Rule: 10A NCAC 09 .1801 Ratios for NCPK The ratios will need to be maintained for the NCPK group. The ratio is 1 teacher to 9 children. Today we talked about strategies for maintaining compliance. If one of the teachers need to leave the classroom, the teacher can take some children with him/her to ensure that ratio of 1:9 will always be maintained. The previous Annual Compliance Visit was completed March 26, 2024, and prior to today’s visit, the eighteen-month compliance history score was 98% which is at or above 75%, as required by North Carolina General Statute 110-90(4) (c-d). This program currently operates with a Five (5)-Star License issued October 10, 2024. The License was based on the program earning Cognia Accreditation. The information on record with DCDEE was verified as correct. Current inspections are as follows: Sanitation Classification of Superior was completed on February 11, 2025. Fire Classification of Satisfactory was completed on March 03, 2025. Katishia Graham, Lead Teacher, was present while I monitored the indoor and outdoor areas. Upon my arrival, fourteen (14) children and two (2) teachers were present. The children were observed participating in free-choice activities and art activities. Children had access to blocks, fine motor, dramatic play, creative art, and literacy materials. In Space #8, children were observed playing with toy dinosaurs and insects at a table, drawing with pencils and crayons at a table, rolling and cutting shapes with playdough at a table, dressing in costumes in the dramatic play area, building structures with wooden blocks in the block area, building structures with magnetic bolts and Legos at a table, and drawing on dry-erase boards with dry-erase markers. The teacher instructed the children to clean up and put away their materials, and to line up to use the bathroom and wash their hands before transitioning to lunch. Routine care and handwashing practices were observed. Lunch consisted of turkey hotdog on bun, green beans, mandarin oranges, and milk. NC Pre-K requirements were reviewed. The Creative Curriculum was being implemented in the classroom. Teaching Strategies GOLD assessments were placed in the children’s files. The Ages and Stages Questionnaires (ASQ-3) were used for developmental screenings and were placed in the children’s files. Hearing and Vision screenings were documented on the children’s health assessment form. Dental screening forms were placed in the children’s files. Two staff files were reviewed, and all documentation was observed in compliance with applicable child care rules. Two children’s records were reviewed, and all documentation was observed in compliance with applicable child care rules. Program files were reviewed. The most recent fire drill was completed on February 25, 2025. The most recent lockdown drill was completed on September 11, 2024. The most recent playground inspection was completed on March 03, 2025. I monitored for compliance by reviewing the physical files as well as the off-site verification forms for the staff and children. The most recent sanitation inspection report was received during today’s visit. At the completion of the visit, the Visit Summary was reviewed with K. Graham, Lead Teacher. In Space #8, children were observed playing with toy dinosaurs and insects at a table, drawing with pencils and crayons at a table, rolling and cutting shapes with playdough at a table, dressing in costumes in the dramatic play area, building structures with wooden blocks in the block area, building structures with magnetic bolts and Legos at a table, and drawing on dry-erase boards with dry-erase markers. The teacher instructed the children to clean up and put away their materials, and to line up to use the bathroom and wash their hands before transitioning to lunch. Routine care and handwashing practices were observed. Lunch consisted of turkey hotdog on bun, green beans, mandarin oranges, and milk. NC Pre-K requirements were reviewed. The Creative Curriculum was being implemented in the classroom. Teaching Strategies GOLD assessments were placed in the children’s files. The Ages and Stages Questionnaires (ASQ-3) were used for developmental screenings and were placed in the children’s files. Hearing and Vision screenings were documented on the children’s health assessment form. Dental screening forms were placed in the children’s files. The previous Annual Compliance Visit was completed March 26, 2024, and prior to today’s visit, the eighteen-month compliance history score was at 96%. This program currently operates with a Four -Star License issued March 06, 2020. The License was based on the program earning four points in education standards, seven points in program standards, and one quality point by choosing to meet a programmatic option for: Has reduced group size by one in each group from the seven-point level as required in Rule .2818 (c). After playing in centers, the children cleaned up their toys, and the children sat on the carpet. The children picked a book of their choice, and the teacher allowed each child to talk about what their book was about. Lunch served was chicken nuggets, mixed vegetables, pears, bread, and milk. At the completion of the visit, the Visit Summary was reviewed with Ms. Graham. The following violation was observed: Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. There was one package of Clorox wipes stored in an unlocked cabinet, less than 5 feet from the floor. Store items that are labeled “keep out of reach of children” and have no other warning can be stored on a shelf or in an unlocked cabinet. The shelf or cabinet must be mounted at least five feet above the floor. In the classroom, there was a package of Clorox wipes stored in an unlocked cabinet less than five feet above the floor. It is important to make sure cleaning supplies are stored at least 5 feet from the floor. This will ensure that potentially harmful products will be out of reach for the children. Classroom Observation In the NCPK classroom, the children were observed playing in a variety of activities throughout their classroom space. The teachers talked to the children and encouraged play with all the children. The children played well with their peers and talked about the activities that they were completing together. The teachers worked with the children one on one on various skills. When expanding on activities, it is important to incorporate sensory activities. This can include finger painting on paper or textured paper. In addition, the children can use paintbrushes and select the paint of their choice. In the classroom, this can be one of the interest areas that are offered to the children. Teachers can provide smocks that cover the upper part of the child to ensure the paint will not get on the children’s clothing. Before utilizing paint, the teachers can talk to children and demonstrate proper ways to use paint in the classroom. Teachers can provide safe based paint that is washable.
