You, Carrie Hogan, Director, assisted me with today’s visit. The last annual compliance visit was conducted on March 10, 2025. The last sanitation inspection was completed on November 6, 2025, with a “Superior” classification. The last fire inspection was completed on September 17, 2025, and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-seven percent prior to today’s visit. The center's five-star rated license, NC Child Care law summary, safe arrival and departure procedures, emergency phone numbers, tobacco restriction, monthly menu, and Emergency Medical Care Plan were prominently posted. The following was monitored during today’s visit: supervision, staff/child ratios, permit restrictions, group size, capacity, adequate and approved space, and hazardous product storage. A walkthrough of the environment was completed. Children were observed engaging in free choice activities, toileting and handwashing routines, outdoor gross motor, lunch, and rest time. Infants were observed receiving care according to their individual needs. Fire drills were completed monthly as required. The last fire drill was completed on January 13, 2026. Emergency drills are being completed every three months as required. The last emergency drill in the form of a lockdown was completed on January 21, 2026. Your playground meets child care safety standards. The last playground inspection was completed on January 23, 2026. No concerns were observed. Materials, toys, and equipment throughout the facility was of sufficient quantity, developmentally appropriate, and in good repair. All medications and Medical Action Plans were monitored during today’s visit. A selection of staff and children’s files were monitored during today’s visit. The program does not provide transportation. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. The NC Pre-K Program Site Monitoring Tool was reviewed today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC PreK staff are lead teachers, Candice Adams and Hannah Smith. The teachers are Carolyn Withers and Mary Potts. A selection of files were monitored for complete health assessments and developmental screenings. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress. The checkpoint assessments are conducted three times per year: at the beginning, middle and end of the school year. The classroom operates from 8:00am- 2:30pm and 8:30am- 3:00pm. Parent conferences are held three times per year. Staff use the Brightwheel app, Ready Rosie, Teaching Strategies family app, and face to face to communicate with parents. In addition, classrooms use communication folders, and complete family engagement activities monthly, and monthly newsletters. The following violations were documented today: Violation Number Comment Rule 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. In space 4, one emergency medication did not contain the pharmacy label. .0803(2)(a) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. In space 3, one emergency medication expired 1/2026. .0803(12) Technical Assistance was provided on the following: • It is recommended that medications be monitored quarterly to ensure that the medication and permission to administer forms are in compliance and have the proper prescription label on the medication from the pharmacy. Two children's application did not have the health care needs section complete. .0801 (a)(5) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. A staff member hired on 8/28/23 and 8/19/24 did not complete the shaken baby head trauma policy prior to caring for children. .0608(d)(1-4) Technical Assistance was provided on the following: 1. We discussed that new staff should review and sign the shaken baby head trauma policy prior to caring for children. This can be reviewed with staff during initial orientation before they are released to work in the classroom. 2. We discussed administrative staff reviewing children’s applications during the enrollment process with families to ensure that all items on the application are completed. If a question does not apply to the child, it should be marked with “N/A”. 3. We discussed staff monitoring the outdoor play area daily for hazards and broken materials. It is recommended that signs of chipping paint be documented on the outdoor inspection checklist and administrative staff should be notified for the repairs to be completed in a timely manner. Consultation was provided on the following: NEW QRIS MODERNIZATION PLAN: The new QRIS Modernization Plan known as “Pathways to Success” is being added to law which included moving to the use of the updated Environmental Rating Scale (ERS-3 versions) for FCCERS, effective February 1, 2025. DCDEE and the NC Child Care Commission will continue the rulemaking process to add the rule language to child care requirements. This process with take several months. Hold Harmless provisions have been extended. Star rated license reassessments are postponed until the QRIS rulemaking process has been completed. Star rated license assessments are still required for new child care programs. A pathway for child care programs currently accredited through the approved accrediting bodies may now move to a 3- or 5-star license if they are not already at this star level. Providers need to submit a request to their licensing consultant and provide a copy of their accreditation award/certificate for processing: Accreditation Licensure Request Form. Please be reminded any ERS assessment completed on February 1, 2025, and after will be assessed using the ERS-3 versions. The center's five-star rated license, NC Child Care law summary, safe arrival and departure procedures, emergency phone numbers, tobacco restriction, first aid and CPR poster, menu, and Emergency Medical Care Plan were prominently posted. The following was monitored during today’s visit: supervision, staff/child ratios, permit restrictions, group size, capacity, adequate and approved space, and hazardous product storage. A walkthrough of the indoor and outdoor environment was completed. Children were observed during free choice activities, outdoor play, lunch, and rest time. You, Mary Parr, Director, assisted me with today’s visit. The last annual compliance visit was conducted on June 6, 2023. The last sanitation inspection was completed on February 28, 2024, with a “Superior” classification. The last fire inspection was conducted September 22, 2023, and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was 94 percent prior to today’s visit. The center's five-star rated license, NC Child Care law summary, safe arrival and departure procedures, emergency phone numbers, tobacco restriction, first aid and CPR poster, menu, and Emergency Medical Care Plan were prominently posted. The following was monitored during today’s visit: supervision, staff/child ratios, permit restrictions, group size, capacity, adequate and approved space, and hazardous product storage. A walkthrough of the indoor and outdoor environment was completed. Children were observed during free choice activities, teacher led small groups, outdoor play, and lunch. Fire drills were completed monthly as required. The last fire drill was conducted on March 17, 2024. Emergency drills are being conducted every three months as required. The last emergency drill in the form of a lockdown was completed on January 30, 2024. The last outdoor playground inspection was completed on March 26, 2026. No concerns were observed on your outdoor play areas. Materials, toys, and equipment throughout the facility was of sufficient quantity, developmentally appropriate, and in good repair. One Medical Action Plan was monitored during today’s visit. All medications, topical ointments, and sunscreens were monitored during today’s visit. A selection of staff files and children’s files were monitored during today’s visit. The program does not provide transportation. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. The NC Pre-K Program Site Monitoring Tool was reviewed today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC PreK staff are lead teachers, Candice Adams and Carrie Hogan, and teachers, Carolyn Withers and Kaylie Holmes. A selection of files were monitored for completed health assessments and developmental screenings. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress. The checkpoint assessments are conducted three times per year: at the beginning, middle and end of the school year. The classroom operates from 8:00am- 2:30pm and 8:30am- 3:00pm to 2:00pm. Parent conferences are held three times per year. Staff use the Brightwheel app, ClassDojo app, Ready Rosie, and face to face to communicate with parents. In addition, you use communication folders, and complete family engagement activities monthly, and weekly newsletters. The following violations were documented today: Violation Number Comment Rule 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. We discussed reviewing prescription medications when they are brought into the facility to document the specified timeframe that the medication should be given to children. A checklist of medications, topical ointments, and/or sunscreens can be made for staff to easily reference expiration dates of the medication, ointment, and permission to administer forms. Once a medication, ointment, and/or sunscreen expires or its permission to administer form expires; it should be returned to the parent or discarded within 72 hours. 2. We discussed that items that can be easily torn apart, small in size, and items stored in plastic such as Ziploc bags should be stored 5 feet or locked when children under three years of age are in care.
