You, Katherine Pimiento, Owner/Director, assisted me with today’s visit. The last annual compliance visit was conducted on September 10, 2024. The last sanitation inspection was completed on September 2, 2024, with a “Superior” classification. The last fire inspection was conducted on January 28, 2025, and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was 98 percent prior to today’s visit. The center's three-star rated license, NC Child Care law summary, safe arrival and departure procedures, emergency phone numbers, tobacco restriction, monthly menu, and Emergency Medical Care Plan were prominently posted. The following was monitored during today’s visit: supervision, staff/child ratios, permit restrictions, group size, capacity, adequate and approved space, and hazardous product storage. A walkthrough of the indoor and outdoor environment was completed. Children were observed engaging in free choice activities, teacher directed activities, toileting and handwashing routines, and outdoor play. Fire drills were completed monthly as required. The last fire drill was completed on January 21, 2025. Emergency drills are being completed every three months as required. The last emergency drill was completed on January 29, 2025. Your playground meets child care safety standards. The last playground inspection was completed on January 28, 2025. No concerns were observed. Materials, toys, and equipment throughout the facility was of sufficient quantity, developmentally appropriate, and in good repair. Emergency medications and Medical Action plans were monitored during today’s visit. Staff files were monitored for a current Criminal Background Check, current First Aid and CPR, ITS- SIDS training, and Recognizing and Responding to Suspicions of Child Maltreatment training during today’s visit. The facility does not provide transportation. The following violation was documented during today’s visit. Violation Number Comment Rule 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. In space 3, an emergency medication expired November 2024. .0803(12) Technical Assistance was provided on the following: 1. We discussed staff checking medications quarterly to ensure that they have not expired. If medications or permission to administer medication forms expired the facility should discard or return the medication to parents within seventy-two hours. The owner discarded the medication during today’s visit. Consultation was provided on the following: 1. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. The last fire inspection was completed on February 27, 2024, and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was 90 percent prior to today’s visit. The center's three-star rated license, NC Child Care law summary, safe arrival and departure procedures, emergency phone numbers, tobacco restriction, menu, and Emergency Medical Care Plan were prominently posted. The following was monitored during today’s visit: supervision, staff/child ratios, permit restrictions, group size, capacity, adequate and approved space, and hazardous product storage. All Health and Safety requirements were monitored during today’s visit. A walkthrough of the indoor and outdoor environment was completed. Children were observed engaging in free choice activities, teacher directed activities, and lunch. Fire drills were completed monthly as required. The last fire drill was completed on August 28, 2024. Emergency drills are being conducted every three months as required. The last emergency drill was completed on July 22, 2024. Your playground meets child care safety standards. The last outdoor playground inspection was completed on August 26, 2024. No concerns were observed. Materials, toys, and equipment throughout the facility was of sufficient quantity, developmentally appropriate, and in good repair. I monitored one emergency medication and one Medical Action Plan in the facility to be in compliance. A selection of staff files and children’s files were monitored during today’s visit. The following violation was documented today: Violation Number Comment Rule 620 All walls and ceilings including doors and windows were not kept clean, free of visible fungal growth, and in good repair. In space 3, there were spots of chipping paint on the baseboards of the wall. 15A NCAC 18A .2825(a) Technical Assistance was provided on the following: 1. We discussed monitoring the facility quarterly for chipping paint, exposed sheet rock, or items that may be considered not kept in good repair. Consultation was provided on the following: 1. You, Katherine Pimiento, Administrator, assisted with today’s visit. Currently this center operates with a temporary license issued on September 18, 2023, with the following restrictions: Daytime care only, children in care on ground level only, fireplace /woodstove not used during operating hours. The Secretary of State website was checked today, and your business Crayolitas Childcare Centers LLC is still active and in good standing. If any changes to the corporation need to be made or you decide to sell your business, then you must notify your consultant at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. A walkthrough of the indoor and outdoor environment was completed. I observed children transitioning from outdoors to indoors, participating in free choice play inside, and eating lunch. The center's temporary license, NC child care law summary, safe arrival and departure procedures, and Emergency Medical Care Plan were prominently posted. The following was monitored during today’s visit: supervision, staff/child ratios, permit restrictions, group size, capacity, adequate and approved space, hazardous product storage. The last monthly fire drill was completed on November 21, 2023 at 11:05am. The last monthly playground inspection was completed on November 22, 2023. The administrator completed the Emergency Preparedness and Response in Child Care training (EPR) on October 16, 2023. You will have four months from the date of the EPR training to complete the online EPR plan. When you complete the EPR plan, print a copy for your records, and review it with your staff members. Have staff sign a statement that you have reviewed the plan with them. The latest sanitation inspection was completed on October 30, 2023, and you received a superior rating and zero demerits. The last fire inspection was completed on March 7, 2023. Three staff files were monitored. Six new children’s files were monitored. The menu for October 2023 did have document all the food components to comply with Meal Patterns for Children in Child Care Programs. The children were served all the required food components during today’s visit by serving Spaghetti with meatballs, Broccoli, pear slices, and milk. We discussed the rated license process and how you will earn points towards a star rated license at the end of your temporary license that expires on March 18, 2024. Program Standards- if you choose to have the Environmental Rating Scale (ERS) completed, your program points will be determined by your overall score on the ERS that will be requested in January 2024. If you choose not to have the ERS completed during your temporary license, you will earn 2 points in program standards. Education Standards-the points earned will be determined by the education of your Administrator and classroom staff. All staff must have their current education in WORKS. Quality Point-you may earn one additional point by meeting an enhanced requirement on the Quality Point options list. I gave you a copy of the Application for Assessment for a Two Component Star Rated License today. You will fill this form out if you would like to request to be issued a two through five star rated license at the end of your temporary license. I gave you a copy of the Rated License Request Review form today. If you want to request the Environmental Rating Scale to be completed, fill this form out as well. I reminded you that programs with less than a three-star license will not be eligible to receive subsidy funding. The following violation was observed and cited during today's visit. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The current lesson plans were not posted for the week. GS 110-91(12); .0508(a) Technical Assistance: 1. Lesson plans must be posted for each week. It will help if teacher post the lesson plan for the following week the Friday before that week. The lesson plan will then be done and posted on Monday morning. The Administrator can check on Monday morning at opening to make sure the plan is posted. All violations cited must be corrected immediately. Please submit a written, signed, and dated statement to me at the email/address below detailing the steps taken to correct each violation. Your letter must include the following: 1. The name of your center 2. The center’s ID number 3. The date you write the letter 4. Address each violation stating how you corrected the violation and are now in compliance. 5. Address your plan to ensure that you will not have that violation again. 6. Health and Safety Training must be completed by all staff within one year of employment unless the staff member has completed all health and safety training within one year of employment at your facility. Recognizing and Responding to Child Maltreatment training must be completed by all staff within 90 days of employment. CPR and First Aid training must be completed by all staff within 90 days of employment. Ensure that CPR and First Aid training is taken in-person, by an approved trainer and training agency, as well as the type of training is Pediatric or Infant/Child CPR and First Aid training. New staff must have a current criminal background check, medical exam, and TB test or TB screening completed prior to employment and every five years after. You, Katherine Pimiento, Administrator, assisted with today’s visit. Toni Washington, Child Care Consultant, accompanied me on today’s visit. Currently this center operates with a temporary license issued on September 18, 2023, with the following restrictions: Daytime care only, children in care on ground level only, fireplace /woodstove not used during operating hours. The Secretary of State website was checked today, and your business Crayolitas Childcare Centers LLC is still active and in good standing. If any changes to the corporation need to be made or you decide to sell your business, then you must notify your consultant at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. A walkthrough of the indoor and outdoor environment was completed. I observed children transitioning from outdoors to indoors, participating in free choice play inside, and eating lunch. The center's temporary license, NC child care law summary, safe arrival and departure procedures, and Emergency Medical Care Plan were prominently posted. The following was monitored during today’s visit: supervision, staff/child ratios, permit restrictions, group size, capacity, adequate and approved space, hazardous product storage. The last monthly fire drill was completed on October 24, 2023 at 3:18pm. The last monthly playground inspection was completed on October 23, 2023. The administrator completed the Emergency Preparedness and Response in Child Care training (EPR) on October 16, 2023. You will have four months from the date of the EPR training to complete the online EPR plan. When you complete the EPR plan, print a copy for your records, and review it with your staff members. Have staff sign a statement that you have reviewed the plan with them. The latest sanitation inspection was completed on October 30, 2023, and you received a superior rating and zero demerits. The inspection form was received and keyed during today’s visit. The last fire inspection was completed on March 7, 2023. Three staff files were monitored. Four children’s files were monitored. Program Standards- if you choose to have the Environmental Rating Scale (ERS) completed, your program points will be determined by your overall score on the ERS that will be requested in January 2024. If you choose not to have the ERS completed during your temporary license, you will earn 2 points in program standards.
