You, Sylvia Holmes, Center Director assisted me with today’s visit. Your last annual compliance visit was conducted on September 07,2023. Your program currently operates with a five-star license, issued on February 18, 2019, earning 6 points in staff education, 7 points in program standards, and 1 quality point. Your program’s compliance history was 98% as of August 27, 2024, and was reviewed with you today. I observed your sanitation inspection was dated February 28, 2024, with a superior classification and 4 demerits. I observed your last fire inspection was dated January 12, 2024. I visited each license indoor space and each outdoor space with you today. I observed children adequately supervised during today’s visit. I observed enhanced staff-child ratio maintained during today’s visit. I observed adequate approved enhanced space use maintained during today’s visit. I observed children engaged in free play activities indoor and outdoor. I observed toys, materials, and equipment to be child-sized, developmentally appropriate, and in good repair. I observed and monitored no medication today. You stated you have no child on medication at the facility at this time. I observed the outdoor play environment. I observed the equipment and materials to be in good repair. I observed the fence to be of adequate height and in good repair in all areas. I observed that in some areas on the fence it needs to be repaired at the top where the fencing meets the pole area. I observed monthly outdoor inspections documented for the past twelve months. I observed the last outdoor inspection was dated August 5, 2024. I observed your monthly fire drill log documented for the past twelve (12) months. I observed your last fire drill was August 15,2024. I observed your quarterly shelter-in-place and lock-down drill log. I observed you last dill was a Shelter-in-place drill and was dated August 19, 2024. I observed your Emergency Preparedness Response (EPR) Plan was last updated on October 17, 2023. I observed menus posted for the week of August 26- 30, 2024. I observed today’s lunch consisted of slices Ham, tossed salad and cheese , ranch dressing, apple sauce, wheat rolls, and milk. I observed your Incident Log to be current for the school year. I reviewed children’s records today and observed no violations regarding children’s records. I reviewed staff records today and observed no violations regarding staff records. I reviewed your Staff and Training worksheet. The visit summary packet was discussed at the end of you visit. The following violation was observed and cited during today's visit. Violation Number Comment Rule 824 Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. Fence in some areas needs to be repaired at the top where the fencing meets the pole. GS 110-91(6); .0605((i) Technical Assistance: 1. When staff conduct monthly playground checks make sure they are checking the condition of the equipment for hazards and reporting if repairs need to be done. All violations cited must be corrected immediately. Please submit a written, signed, and dated statement to me at the email/address below detailing the steps taken to correct each violation. Your letter must include the following: 1. The name of your center 2. The center’s ID number 3. The date you write the letter 4. Address each violation stating how you corrected the violation and are now in compliance. 5. Address your plan to ensure that you will not have that violation again. 6. Your signature I advise you to be prepared to provide supporting documentation in the form of photos, documents, certificates, etc. should I request them in order to clarify or verify compliance. You, Sylvia Holmes, Center Director assisted me with the visit. Your program currently operates with a 5-star license. Your last ACV was on September 7, 2023. I observed your Five-Star License, NC Child Care Summary of the Law, current menu, Sanitation placard, Emergency Medical Care Plan, daily schedule, activity plans posted as required. I observed children adequately supervised during today’s visit. I observed enhanced staff-child ratio maintained during today’s visit. I observed adequate approved space used during today’s visit. I observed permit restrictions maintained during today’s visit. I observed a menu posted for the week of January 29- February 2, 2024. Today’s lunch consisted of ham sandwiches, apple sauce, salad mix, and milk. This was not the food that was posted on the menu for today. The cook changed the menu to make it current. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. I observed children throughout the child care facility were participating in group time, free play in activity areas, transitions, and personal care routines. I observed no hazardous materials or equipment to be accessible to the children during today’s visit. I observed three medications in space 2 today. All medications were current and all medication forms were in compliance today. Your sanitation inspection was conducted on October 31, 2023, with a Superior classification and 4 demerits. Your last Fire Inspection was conducted on January 12, 2024. I observed your monthly Fire Drill Log, a drill was conducted on January 08, 2024. I observed your Emergency Drill Log, and a Lockdown drill was conducted on December 05, 2023. Emergency drills are to be conducted every three months I observed all staff Criminal Background Qualifying Letters, CPR & First Aid trainings, ITS-SIDS, and Recognizing & Responding to Suspicious of Child Maltreatment training , Emergency Questionnaires, Health Questionnaires, and all other certification were in compliance The following violations observed, cited, and corrected during my visit today: Violation Number Comment Rule 528 Food substitution was not of comparable food value or recorded on the menu prior to the meal or snack being served. The menu for the day was not posted. The menu was substituted and the cook had not changed the menu. The menu was changed by the kitchen staff. 10A NCAC 09 .0901(b) 1301 Center did not maintain a record of daily attendance. Attendance was not done in classroom 1, space 1. This was corrected by teacher. GS 110-91(9) Technical Assistance: 1. Make sure the attendance is completed in each classroom everyday. When talking to the center director it was suggested that the attendance must be completed by each teacher by the cut off time for the children in the morning. It was also suggested that the teachers mark the children present as they enter the classroom. 2. Make sure that if the menu changes it is corrected on the menu.
