You, Myajah Milam, Owner/Operator, assisted me with today’s visit. The last annual compliance visit was conducted on May 30, 2025. The program’s compliance history was ninety-five percent prior to today’s visit. The NC Secretary of State website was reviewed during today’s visit. Beautiful Minds Learning Academy LLC was listed as current-active. The program's three-star rated license, NC Child Care law summary, emergency phone numbers, tobacco restriction, monthly menu, and Emergency Medical Care Plan were prominently posted. The following was monitored during today’s visit: supervision, permit restrictions, group size, capacity, adequate and approved space, and hazardous product storage. A walkthrough of the environment was completed. Children were observed engaged in free choice activities, toileting and handwashing routines, outdoor play, lunch, and rest time. Fire drills were completed monthly as required. The last fire drill was completed on April 9, 2026. Emergency drills are being conducted. The last emergency drill documented was a lockdown completed on March 4, 2026. Your outdoor play area meets child care safety standards. The last outdoor inspection was completed on April 1, 2026. No concerns were observed. Materials, toys, and equipment throughout the facility were of sufficient quantity, developmentally appropriate, and in good repair. You stated that no children currently enrolled have medications or Medical Action Plans. Your personnel file and all children’s files were monitored during today’s visit. The program has completed lead water, lead paint, and asbestos testing with Clean Water for Carolina Kids. The program has not completed the provider portal within ABCMS. On March 5, 2026, I received an email from you requesting to increase the capacity of your family child care home along with a zoning verification letter from the City of Statesville. In your email you stated that your facility meets the requirements of Session Law 2025-36 by all rooms for child care being located on ground level of home with a direct exit to the outside. You have at least one 2A:10B:C fire extinguisher per 40 feet travel distance. You have an EPR Plan completed and updated annually. You stated that you have smoke detectors and carbon monoxide alarms within forty feet of all space used by children. You have smoke detectors and carbon monoxide with battery backup within forty feet of all spaces used by children. These items were verified during today’s visit. A permit change will be completed and submitted for review. Please continue to follow your current license and restrictions until you have received a new license in the mail. Once you have received your new license, please mail me your original license at P.O. Box 954 Taylorsville, NC 28681. The capacity increase that will be noted on your new license can be up to 10 children, however, the number and ages of children present at your facility must be in compliance with one of the capacity options found in General Statute 110-91(7)(b) at all times. The capacity options in statue are as follows: 1. A maximum of eight children, with no more than five children who are from birth to 5 years of age, plus three school-age children. (five children 0-5yrs, three school-age children, total of 8 children) 2. A maximum of three children from birth to 24 months of age, plus three children from 2 to 5 years of age and three school-age children up to 13 years of age, for a total of nine children. (three children 0-1yr old, three children 2-5yrs old, three school-age children, total of 9 children) 3. A maximum of 10 children if all children are older than 24 months of age. (all children must be 2 yrs old and older, total of 10 children) The program does not provide transportation. I verified today that this pathway is the option you would like to move forward with. The program rated license is due to be reassessed by January 11, 2027. Please review the child care rules section 10A NCAC 09 .3206 CLASSROOM AND INSTRUCTIONAL QUALITY PATHWAY FOR FAMILY CHILD CARE HOMES. OUTSTANDING REQUIRED DOCUMENTATION NEEDED FOR VERIFICATION no later than October 9, 2026. In addition to the “Application for Assessment for a Rated License for FCCH”, please be prepared to submit the following to me for verification: 1. Education Standards- FCCH Operator: Please submit the QRIS Staff Information and Education Worksheet along with your application. 2. Family and Community Engagement Standards Child Care Centers: Please submit the Family and Community Engagement Standards Child Care Centers document along with a written description of your plan to implement each foundational practice and additional practice as applicable. 3. Operator Continuous Quality Improvement (CQI) and Professional Development (PD) Plan Individual Please submit the Operator CQI/PD Plan document. 4. Curriculum or Approved Curriculum: If applicable, please submit documentation that can be used to verify that a curriculum is in place. The operator shall implement a curriculum that has been approved by the Commission as defined in Rule .0102(13) of this Chapter for all ages served. Approved Formative Assessment If applicable, please submit documentation that can be used to verify that an approved formative assessment is in place. The operator shall use a formative assessment(s) that has been approved by the Commission, available at http://ncchildcare.ncdhhs.gov/Services/Licensing/Star-Rated-License/Curriculum. Evidence of formative assessments shall include documentation required or contemplated by the approved formative assessment and may also include: (A) anecdotal notes; (B) portfolio of child's work samples; (C) developmental screenings; or (D) photographs, audio, or video recordings with permission from the child's family. 6. Formative Assessment shared with families: If applicable, please submit documentation that can be used to verify a plan to share the formative assessment with families annually or twice annually is in place. The operator shall share the results of the formative assessment with families at least once annually at the four-star level or twice annually at the five star level. The manner in which the results are shared shall be convenient for the family such as by telephone, email, virtually, or in-person. 7. The operator has not completed the ABCMS provider portal roster for the facility. G.S. 110-90.2 & .2703(r) Technical Assistance was provided on the following: • ABCMS SYSTEM: The process of notifying the Division of any new child care providers working who are hired or moved into the child care facility within five business days has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement as stated in G.S. 110-90.2 & .2703(r) to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. This will prevent the current training from expiring and experiencing delays in training opportunities. • Ten (10) hours of on-going training will need to be completed by May 20, 2026, for the 2024-2025 monitoring year. • Eight (8) hours of on-going training will need to be completed by May 30, 2026, for the 2025-2026 monitoring year. The program’s compliance history was ninety-five percent prior to today’s visit. The NC Secretary of State website was reviewed during today’s visit. Beautiful Minds Learning Academy LLC was listed as current-active. The program's three-star rated license, NC Child Care law summary, emergency phone numbers, tobacco restriction, monthly menu, and Emergency Medical Care Plan were prominently posted. The following was monitored during today’s visit: supervision, permit restrictions, group size, capacity, adequate and approved space, and hazardous product storage. A walkthrough of the environment was completed. Children were observed engaged in free choice activities, toileting and handwashing routines, outdoor play, lunch, and rest time. Fire drills were completed monthly as required. The last fire drill was completed on April 9, 2026. Emergency drills are being conducted. The last emergency drill documented was a lockdown completed on March 4, 2026. Your outdoor play area meets child care safety standards. The last outdoor inspection was completed on April 1, 2026. No concerns were observed. Materials, toys, and equipment throughout the facility were of sufficient quantity, developmentally appropriate, and in good repair. You stated that no children currently enrolled have medications or Medical Action Plans. Your personnel file and all children’s files were monitored during today’s visit. The program has completed lead water, lead paint, and asbestos testing with Clean Water for Carolina Kids. The program has not completed the provider portal within ABCMS. On March 5, 2026, I received an email from you requesting to increase the capacity of your family child care home along with a zoning verification letter from the City of Statesville. In your email you stated that your facility meets the requirements of Session Law 2025-36 by all rooms for child care being located on ground level of home with a direct exit to the outside. You have at least one 2A:10B:C fire extinguisher per 40 feet travel distance. You have an EPR Plan completed and updated annually. You stated that you have smoke detectors and carbon monoxide alarms within forty feet of all space used by children. You have smoke detectors and carbon monoxide with battery backup within forty feet of all spaces used by children. These items were verified during today’s visit. You, Myajah Milam, Owner/Operator, assisted me with today’s visit. The last annual compliance visit was conducted on May 30, 2025. The program’s compliance history was ninety-five percent prior to today’s visit. The NC Secretary of State website was reviewed during today’s visit. Beautiful Minds Learning Academy LLC was listed as current-active. The program's three-star rated license, NC Child Care law summary, emergency phone numbers, tobacco restriction, monthly menu, and Emergency Medical Care Plan were prominently posted. The following was monitored during today’s visit: supervision, permit restrictions, group size, capacity, adequate and approved space, and hazardous product storage. A walkthrough of the environment was completed. Children were observed engaged in free choice activities, toileting and handwashing routines, outdoor play, lunch, and rest time. Fire drills were completed monthly as required. The last fire drill was completed on April 9, 2026. Emergency drills are being conducted. The last emergency drill documented was a lockdown completed on March 4, 2026. Your outdoor play area meets child care safety standards. The last outdoor inspection was completed on April 1, 2026. No concerns were observed. Materials, toys, and equipment throughout the facility were of sufficient quantity, developmentally appropriate, and in good repair. You stated that no children currently enrolled have medications or Medical Action Plans. Your personnel file and all children’s files were monitored during today’s visit. The program has completed lead water, lead paint, and asbestos testing with Clean Water for Carolina Kids. The program has not completed the provider portal within ABCMS. On March 5, 2026, I received an email from you requesting to increase the capacity of your family child care home along with a zoning verification letter from the City of Statesville. In your email you stated that your facility meets the requirements of Session Law 2025-36 by all rooms for child care being located on ground level of home with a direct exit to the outside. You have at least one 2A:10B:C fire extinguisher per 40 feet travel distance. You have an EPR Plan completed and updated annually. You stated that you have smoke detectors and carbon monoxide alarms within forty feet of all space used by children. You have smoke detectors and carbon monoxide with battery backup within forty feet of all spaces used by children. These items were verified during today’s visit. A permit change will be completed and submitted for review. Please continue to follow your current license and restrictions until you have received a new license in the mail. Once you have received your new license, please mail me your original license at P.O. Box 954 Taylorsville, NC 28681. The capacity increase that will be noted on your new license can be up to 10 children, however, the number and ages of children present at your facility must be in compliance with one of the capacity options found in General Statute 110-91(7)(b) at all times. The capacity options in statue are as follows: 1. A maximum of eight children, with no more than five children who are from birth to 5 years of age, plus three school-age children. (five children 0-5yrs, three school-age children, total of 8 children) 2. A maximum of three children from birth to 24 months of age, plus three children from 2 to 5 years of age and three school-age children up to 13 years of age, for a total of nine children. (three children 0-1yr old, three children 2-5yrs old, three school-age children, total of 9 children) 3. A maximum of 10 children if all children are older than 24 months of age. (all children must be 2 yrs old and older, total of 10 children) The program does not provide transportation. Per child care rule 10A NCAC 09 .3224 (a & b) Recognition Of Quality Initiatives, a child care operator may choose to request one or more areas of recognition to accompany the issuance of a two through five-star rated license earned through compliance with any licensure pathway requirement of this Section. Quality initiatives will not be used to earn a star rating. Verification of requirements for each recognition shall be reassessed at least every three years, at the time of reassessment for a two through five-star rated license. I verified today that this pathway is the option you would like to move forward with. The program rated license is due to be reassessed by January 11, 2027. Please review the child care rules section 10A NCAC 09 .3206 CLASSROOM AND INSTRUCTIONAL QUALITY PATHWAY FOR FAMILY CHILD CARE HOMES. OUTSTANDING REQUIRED DOCUMENTATION NEEDED FOR VERIFICATION no later than October 9, 2026. In addition to the “Application for Assessment for a Rated License for FCCH”, please be prepared to submit the following to me for verification: 1. Education Standards- FCCH Operator: Please submit the QRIS Staff Information and Education Worksheet along with your application. 2. Family and Community Engagement Standards Child Care Centers: Please submit the Family and Community Engagement Standards Child Care Centers document along with a written description of your plan to implement each foundational practice and additional practice as applicable. 3. Operator Continuous Quality Improvement (CQI) and Professional Development (PD) Plan Individual Please submit the Operator CQI/PD Plan document. 4. Curriculum or Approved Curriculum: If applicable, please submit documentation that can be used to verify that a curriculum is in place. The operator shall implement a curriculum that has been approved by the Commission as defined in Rule .0102(13) of this Chapter for all ages served. The operator shall use a formative assessment(s) that has been approved by the Commission, available at http://ncchildcare.ncdhhs.gov/Services/Licensing/Star-Rated-License/Curriculum. Evidence of formative assessments shall include documentation required or contemplated by the approved formative assessment and may also include: (A) anecdotal notes; (B) portfolio of child's work samples; (C) developmental screenings; or (D) photographs, audio, or video recordings with permission from the child's family. 6. Formative Assessment shared with families: If applicable, please submit documentation that can be used to verify a plan to share the formative assessment with families annually or twice annually is in place. The operator shall share the results of the formative assessment with families at least once annually at the four-star level or twice annually at the five star level. The manner in which the results are shared shall be convenient for the family such as by telephone, email, virtually, or in-person. 7. Coaching/training options for the operator: If applicable, the operator participates in one of the following activities regarding classroom and instructional quality practices: (A) annual completion of five hours of coaching or mentoring by a center administrator of a licensed child care center with a star-rating of four or five stars, an operator of another licensed family child care home with a star-rating or four or five stars, or a community resource partner; (B) annual completion of five on-going training hours, in addition to applicable requirements in Rule .1703(f) of this Chapter and Rule .3217 of this Section; or (C) annual completion of 0.5 continuing education unit, in addition to applicable requirements in Rule .1703(f) of this Chapter and Rule .3217 of this Section. The following violations were cited during today’s visit: Violation Number Comment Rule 1301 Operator did not complete the required number of on-going training hours as specified in rule. The operator did not complete ten (10) hours of on-going training for the 2024-2025 monitoring year. GS 110-91(11); 10A NCAC 09 .1705(b)(5) 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The operator has not completed the ABCMS provider portal roster for the facility. G.S. 110-90.2 & .2703(r) Technical Assistance was provided on the following: • ABCMS SYSTEM: The process of notifying the Division of any new child care providers working who are hired or moved into the child care facility within five business days has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement as stated in G.S. 110-90.2 & .2703(r) to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. Training opportunities can also be found with Southwestern Child Development Commission Inc. Once training has been completed, obtain the training certificates and log the information on the on-going documentation log and maintain the information in your personnel file. • We discussed inquiring and registering for training courses such as First Aid and CPR and ITS-SIDS at least 3-6 months in advance. This will prevent the current training from expiring and experiencing delays in training opportunities. • Ten (10) hours of on-going training will need to be completed by May 20, 2026, for the 2024-2025 monitoring year. • Eight (8) hours of on-going training will need to be completed by May 30, 2026, for the 2025-2026 monitoring year. Consultation was provided on the following: • We discussed that when using a curriculum, it is important that activity plans algin with the curriculum, NCFELD, and are updated weekly. • We discussed that when school age children enrolled, they will also need an approved curriculum. School age children are not required to have a formative assessment completed. The program’s compliance history was ninety-two percent prior to today’s visit. The Secretary of State’s (SOS) website was monitored, and Beautiful Minds Learning Academy LLC has been suspended. You stated that you had been on the website but were unsure about a form that was needing to be submitted. You stated that you plan to call the Secretary of State to resolve this matter. An administrative action may be issued if the corporation status does not get resolved in a timely manner. I requested that you submit a letter to me outlining your plan to resolve this suspension on the with your signature, facility’s name, and ID # by June 6, 2025. The center's three-star rated license, NC Child Care law summary, emergency phone numbers, tobacco restriction, menu, and Emergency Medical Care Plan were prominently posted. The following was monitored during today’s visit: supervision, permit restrictions, group size, capacity, adequate and approved space, and hazardous product storage. A walkthrough of the indoor and outdoor environment was completed. Children were observed engaging in free choice activities, caregiver directed activities, painting, lunch, indoor gross motor, and toileting and handwashing routines. Fire drills were completed monthly as required. The last fire drill was completed on May 20, 2025. Emergency drills are being completed every three months as required. The last emergency drill was completed on March 3, 2025. Your outdoor play area meets child care safety standards. The last outdoor inspection was completed on May 20, 2025. Materials, toys, and equipment throughout the facility was of sufficient quantity, developmentally appropriate, and in good repair. Your personnel file and all children’s files were monitored during today’s visit. I monitored your five (5) pound fire extinguisher to be charged and easily accessible to you. I observed an electric smoke detector mounted on the ceiling. The following violations were documented today: Violation Number Comment Rule 508 Operator did not successfully complete a first aid course as referenced in Rule.1702(b)(2) First aid training was not renewed on or before the expiration of the certification. The first aid certification expired on January 27, 2025. .1703(a)(2) 511 Operator did not successfully complete a CPR course as referenced in Rule.1702(b)(2) CPR training was not renewed on or before the expiration of the certification. The CPR certification expired on January 27, 2025. .1703(a)(3) 701 All indoor and outdoor areas used by the children were not kept clean, orderly, and free of items which are potentially hazardous to children including removal of items a child can swallow; the removal of loose nails or screws and splinters on inside; and use of outdoor equipment that is too hot to touch. Walls, cabinets, trashcan, and refrigerator were not kept clean from spills and markings. Training opportunities may be sought outside of the county where you reside. Please ensure that training is completed through an approved training agency. • We discussed creating a daily and weekly cleaning schedule to ensure that all walls, cabinets, trashcans, and appliances remain in good repair, free of visible dirt, and markings from writing utensils. • We discussed completing lawn services biweekly and when there is an overgrowth of grass due to heavy amounts of rain. • We discussed conducting a walkthrough of your outdoor environment prior to caring for children to ensure that there is no standing water on materials or in buckets after heavy rain. Consultation was provided on the following: • Your ITS-SIDS training is due to be renewed by March 21, 2026. • You are required to receive 10 hours of on-going training annually by July 10 of each year. • While you are completing school, please print unofficial transcripts of completed courses and place them in your personnel file to be reviewed by DCDEE during visits. • Please ensure that children are only using approved spaces. NEW QRIS MODERNIZATION PLAN: The new QRIS Modernization Plan known as “Pathways to Success” is being added to law which included moving to the use of the updated Environmental Rating Scale (ERS-3 versions) for FCCERS, effective February 1, 2025. DCDEE and the NC Child Care Commission will continue the rulemaking process to add the rule language to child care requirements. This process with take several months. Hold Harmless provisions have been extended. Star rated license reassessments are postponed until the QRIS rulemaking process has been completed. During today’s visit I observed seven (7) preschool children ages one to five in care. You stated that you verbally told parents during pick up on June 7, 2024, that you misunderstood your allowed capacity and that you are only able to care for 5 preschool children at a time. You stated that parents brought up your operational policies that states you would give them two weeks’ notice. I explained to you that you are required to follow all child care rules and your current restrictions listed on your license. The center's three-star rated license, NC Child Care law summary, emergency phone numbers, tobacco restriction, menu, daily schedule, and Emergency Medical Care Plan were prominently posted. The following was monitored during today’s visit: supervision, permit restrictions, capacity, adequate and approved space, and hazardous product storage. A walkthrough of the indoor and outdoor environment was completed. Children were observed engaged in free choice activities, circle time, lunch, toileting and handwashing routines, and rest time. Fire drills were completed monthly as required. The last fire drill was completed on May 9, 2024. Emergency drills are being conducted every three months as required. The last emergency drill was completed on March 15, 2024. Outdoor Playground Inspections are being conducted monthly. The last outdoor inspection was completed on May 9, 2024. Materials, toys, and equipment throughout the facility was of sufficient quantity, developmentally appropriate, and in good repair. You, Myajah Milam, Owner/Operator, assisted me with today’s visit. Your facility currently operates with a temporary license issued on September 29, 2023, with the following restrictions: first and second shift, and maximum of 5 preschool children at any time. The Secretary of State website was checked prior to today’s visit, and Beautiful Minds Learning Academy LLC is listed as current- active. I observed your Temporary License, NC Child Care Summary of the Law, Safe Procedures for Arrival & Departure, Emergency Numbers, Emergency Medical Care Plan, First Aid poster, tobacco free facility signage and evacuation plans posted as required. I observed children adequately supervised, licensed capacity of eight enrolled children, and approved spaced used maintained during today’s visit. During today’s visit, children were down for rest time. I observed a sign-in and out sheet in located on a clipboard at the entrance of your facility. I observed attendance records to be current as required. I observed toys, equipment, and materials throughout the facility to be of sufficient quantity, developmentally appropriate, and in good repair. I monitored your outdoor play space for general safety today. You currently do not have any stationary climbing structures. The outdoor area is fenced in and contains a variety of natural elements to be used by the children. I observed your Incident Log to be current and up to date as required. Children’s incident reports are being filed in their individual files. I monitored all children’s files during today’s visit. I monitored your staff file for all applicable information. I observed no violations related to staff records. I observed your last fire drill was conducted on November 7, 2023 at 8:47am with twenty-two seconds to evacuate. Please ensure that emergency drills in the form or a lockdown or shelter-in-place are being conducted every three months and documented on your drill log. You stated that you do not provide transportation. You stated that you do not provide medications to any children currently enrolled. We discussed the star rated license process during today’s visit. You stated that you are not interested in completing the Environmental Rating Scale (ERS) at this time. Your education has been verified by the workforce unit. Please send me your three-month self-study by December 1, 2023. I reminded you that programs with less than a three-star license will not be able to receive subsidy funding. Rated License Assessment Summary: Compliance History: The facility has a compliance history of 84% as of November 21, 2023. Education Standards: The family child care home operator qualifies at the four point level by having 6 additional semester hours in Early Childhood Education and by having NC FCC Credentials. The facility will earned 4 points in Education Standards. Program Standards: The provider is in the process of completing the 3-month self-study. The facility will not complete the FCCERS Assessment. The facility will limit infant enrollment to no more than 2 infants under the age of 1 year. The facility will earn 2 points in Program Standards. Quality Point: The facility will earn 1 quality point by limiting infant enrollment to no more than 2 infants under the age of 1 year. 4+2+1=7 The facility has earned a total of 7 points and is eligible to receive a Three Star Rated License at the end of the temporary time period. Violation Number Comment Rule 1895 Application did not include health care needs or concerns, symptoms of and the type of response required for the health care needs or concerns. Two children's applications did not have the health care needs section completed. You contacted the parents during today's visit to get permission to correct the application. .1721 (a ) (3)(D) 2033 Signed discipline statement did not include the required information, as outlined in rule. One child's discipline policy did not contain the enrollment date. You filled out the enrollment date during today's visit. .1727(c )(1-3) The following Technical Assistance (TA) was provided during today’s visit: 1. We discussed looking over children’s application and paperwork during the enrollment process with parents. This will ensure that all paperwork is completely filled out and to check for errors. Please ensure that enrollment dates are documented on the children’s discipline policy. 2. We discussed that the online EPR plan should be completed within four months after completing the EPR training. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Due to the violations being corrected during today’s visit, a compliance letter is not due at this time. Lunch consisted of a dino nuggets, mixed vegetables, pears, french fries, a roll, and milk. I observed a sign-in and out sheet in located on a clipboard at the entrance of your facility. I observed attendance records to be current as required. I observed toys, equipment, and materials throughout the facility to be of sufficient quantity, developmentally appropriate, and in good repair. I monitored your outdoor play space for general safety today. You currently do not have any stationary climbing structures. The outdoor area is fenced in and contains a variety of natural elements to be used by the children. I observed your Incident Log to be current and up to date as required. You completed the application for a two-five star rated license and request the rated license to be completed. You should have your current education verified by the workforce unit. Please print a copy of your workforce status letter and place it in your file for review. I reminded you that programs with less than a three-star license will not be able to receive subsidy funding. Violation Number Comment Rule 705 All medicines, including refrigerated and unrefrigerated, were not kept in locked storage. A bottle of liquid Tylenol was located on a shelf in the bathroom used by children. You locked the medication up during today's visit. .1719(a)(5) 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. An aerosol can of cooking spray was located in an unlocked cabinet in the kitchen. You placed the cooking spray in locked storage during today's visit. .1719 (a)(7) 1853 The operator did not conduct a monthly fire drill. A fire drill was not completed for the month of July. .1719(a)(15) & .1721( e)(2) The following Technical Assistance (TA) was provided during today’s visit: 1. We discussed that the EPR training should be completed within the first year of operation. 2. We discussed ensuring that fire drills are being conducted monthly and all applicable information is documented on the Emergency Drill Log and Report form. 3. We discussed conducting a walkthrough of your child care space prior to caring for children to ensure that hazardous products, aerosol cans, medications, etc. are properly stored. Laundry supplies can be kept out of reach(5 feet) OR locked. If the items are not at 5 feet, then it must be locked. A lock is considered a lock/key, combination lock, magnetic lock. 4. Health and Safety Training must be completed within one year of employment and every five years thereafter. You can access these trainings for free on DCDEE’s website www.ncchildcare.dhhs.gov by going to Provider, Professional Training, and DCDEE Moodle. 5. We discussed that after heavy rain and while completing outdoor inspections, pour off excess water that may have accumulated on equipment. 6. We discussed ensuring that the grass is kept maintained to prevent it from being overgrown. 7. We discussed adding the weeks to your menus to make them clear for what days meals are being served. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before September 12, 2023, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information.
