Upon arrival, I was greeted by you, Shakitia Jones, Provider, as you were cleaning up from lunch time and the children were observed resting/sleeping on their cots. Your Temporary License was issued on July 14, 2025, and will end on January 14, 2026. The outdoor play area was monitored. The last outdoor inspection and a lockdown drill were conducted on November 5, 2025, and the last fire drill was conducted on November 6, 2025. You have completed the EPR training and I monitored the EPR Plan during today’s visit. We discussed that you will want to update the Child Care Consultant contact information once your temporary time period has ended. You will also want to contact your local Partnership to see if they can give you the contact information for the local Child Care Health Consultant. You stated that the items we discussed concerning the van that you would like to use for transportation during my last visit have been corrected. Except you are still waiting on the locking mechanism part. Please submit a letter to me when all has been corrected in addition to the registration and insurance information. Once received an announced visit will be conducted to monitor the vehicle. There was one new child enrolled since my last visit and that file was monitored today. We reviewed the "Pathway #2-Classroom & Instructional Quality Star Level Assessment for a FCCH" and signed and dated the document. {CC RULE .3206(e)} 1) You stated that you will serve no more than four children under the age of 24 months. 2) I received the Family and Community Engagement Foundational Practices on November 10, 2025, and we reviewed it during today’s visit. You are implementing the foundational practices and three additional options, with one form each category of engagement. We discussed keeping documentation on the opportunities you offer. CC RULE .3220 3) I received the FCCH Continuous Quality Improvement (CQI) on November 10, 2025. We discussed that Ms. Chase, Additional Caregiver, will need to complete a separate one. We also discussed that the CQI means a process that individuals and facilities shall use to identify areas of growth and determine a path toward enhanced teaching, learning and practices to achieve better outcomes for children, staff, and programs. The CQI process builds over time, using established goals to support individuals and facilities in achieving and sustaining continual improvements. CC RULE .3221 4) You stated that the approved curriculum that will be used for all ages is Gee Whiz and the approved formative assessment that will be used is Teaching Strategies Gold. Both will be purchased within the next couple of weeks by the grant that you received from the Innovation Grant for Family Child Care Homes. You shared with me the communication between you and Kelly Shaw, NC Statewide Childcare Business Anchor, about the request approval. We discussed that you would need to send me documentation that they have been purchased. You will also be required to complete training related to the curriculum and formative assessment tool that is used. Verification of the training will need to be in your file. CC RULE .3206 5) You stated that child assessments will be shared with families twice annually. I encouraged you to keep documentation on file. 6) We reviewed CC RULE .3206(e)(10) and the various options you, as the FCCH Operator, have to complete annually regarding classrooms and instructional quality practices. You stated that you have a mentor through FCCH Innovation Grant and you were able to show me the documentation that you have virtually. Please keep this as documentation so that your Child Care Consultant can monitor it. 7) I received a copy of your WORKS status letter, and you are eligible for five stars in education. This program is eligible for five stars once I receive the revised individual CQI for you and Ms. Chase. There was one violation observed during today's visit. Violation Number Comment Rule 1927 Openings in equipment, steps, decks, and handrails were not smaller than 3 1/2 inches or greater than 9 inches to prevent entrapment. There is a section of the fence on the left hand side closest to the house that has an opening that is measuring between 4-5 inches between the mesh and fence post. .1719(a)(23) TECHNICAL ASSISTANCE: -It is important to monitor the outdoor play area daily to ensure that there are no hazards, which includes but not limited to the barrier attached to the fence to ensure that there are not entrapments. You stated that you had additional barrier material that you can use to cover the entrapment area. CONSULTATION: -We discussed during the visit the requirements that need to be met to increase your capacity, and the number and ages of children present at your facility must be in compliance with one of the capacity options found in General Statute 110-91(7)(b) at all times. This information was also emailed to you on December 5, 2025. The capacity options in statue are as follows: 1. A maximum of eight children, with no more than five children who are from birth to 5 years of age, plus three school-age children. (five children 0-5yrs, three school-age children, total of 8 children) 2. A maximum of three children from birth to 24 months of age, plus three children from 2 to 5 years of age and three school-age children up to 13 years of age, for a total of nine children. (three children 0-1yr old, three children 2-5yrs old, three school-age children, total of 9 children) 3. A maximum of 10 children if all children are older than 24 months of age. (all children must be 2 yrs old and older, total of 10 children) -Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before December 4, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. While the Division allows you time to explain how you have corrected violations when violations are cited, it is expected you correct all violations immediately. When you are asked to send a compliance letter to me, it is expected and required that your compliance letter be sent to me no later than the date indicated in this documentation. Your letter must include the following: -Name of your program -ID number of your program -Date of Letter -Violation number -Explain how you corrected the violation (send supporting documentation to show compliance) -Describe your plan to make sure you will not have that same violation in the future -Name or signature of the person who wrote the letter. (If emailing the letter, your email address will be considered your digital signature.) Upon arrival, I was greeted by Tabitha Chase, Additional Caregiver. She stated that you, Shakitia Jones, was working outside the home providing therapy. Ms. Chase stated that you are at the home until 9:00 AM when she arrives and cares for the children until you return around 1:30 Monday-Friday. The children were observed during rest/nap time. Some were sleeping and others were sitting quietly on their cots looking at books, getting up from nap, eating snack, hand washing routines and free choice activities indoors. Positive interactions between you, Ms. Chase and the children was observed throughout today’s visit. I monitored Ms. Chase’s file, and she has all the required documents needed. There are four children enrolled, and four children present today. I monitored the children’s files and the Daily Child’s Sign-In/Out sheets. -Please add a space on the parent acknowledgement form for the parents to write their child’s name and their date of enrollment. -Two of the children’s files did not have the date the Discipline Policy was adopted. This family childcare home (FCCH) was issued a temporary license on July 14, 2025, with a capacity of eight children and the following restrictions: daytime care only and maximum of five preschool children at any time. You, Shakita Jones, Operator/Owner, arrived at 1:30 and we reviewed the facility input form to ensure that there were no changes to your facility information, and you stated there were no changes. We reviewed the facility permit to ensure that there were no changes to your facility’s operation. The facility is owned/operated by you, Shakitia Jones. As the owner/operator, you are required by Child Care Rule .0102 (19) to reside in the home where the license is issued and according to General Statute 110-91(8) to be the full-time caregiver to the children enrolled at the family child care home. You stated you live in the home and that you and Ms. Chase are the persons working to care for the children enrolled at your family child care home. We discussed that you cannot work at a different job outside the home during operating hours. You stated that it is not every day, and it is for short periods of time and there are also times that you and Ms. Chase work together at the home. You will submit a work schedule to me, and I will submit for review. We reviewed the restrictions on your family child care home license as well as the capacity, shifts of care, and the age range of the children served at your facility. You stated that you did not have any questions regarding the information reviewed with you regarding your license. The outdoor play area was monitored. The last outdoor inspection was completed on September 2, 2025. The last fire drill was conducted September 10, 2025. You requested that I monitor your vehicle because you may want to provide transportation for school-aged children. The following was observed as I monitored the vehicle: -Two of the seat belts in the far back on the passenger side are not retracting. -The fire extinguisher will need to be mounted in the front area. -The ice pack and antiseptic wipes will need to be removed from the first aid kit. -The registration tag expired in February 2025. You stated that you have not received the current one in the mail yet. -A “No Tobacco Use” sign will need to be posted in the vehicle. -One of the side doors used by the children does not open. -The two side doors are unable to be locked. Please review SECTION .1000 - TRANSPORTATION STANDARDS and become families with all the requirements. Program Standards: We discussed the rated license process. It is very important that you familiarize yourself with the QRIS Pathways to the Stars. There are now three options available to demonstrate quality and earn a 2-5 Star Rated License. It includes updates to education standards for early childhood educators to expand options; and adds standards for both educators and facilities that support continuous quality improvement and family/community engagement practices that promote communication and collaboration. You will find more information and details about the three different pathways so that your program can begin working towards the star rated license. We also discussed the QRIS Information Meeting that is scheduled for September 22, 2025 in the evening. I sent you an email with the registration information. We discussed and reviewed the Self-Study Verification Form. As part of the NC Rated License process for the Program Assessment Pathways, you must participate in a self-study process using the FCCER-3. This form provides documentation that the 4-step process has taken place and identifies key insights gained. Visit www.ncrlap.org for information and resources about the process. During today’s visit we reviewed the following documents: -Continuous Quality Improvement (CQI) for Family Child Care Homes (FCCH)(Rule .3221) -Education Standards for FCCH Operators (Rule .3217) -Family & Community Engagement Standards for FCCH (Rule .3220) -Pathway 1- Program Assessment for Family Child Care Homes (FCCH) -Pathway 2- Program Assessment for Family Child Care Homes (FCCH) Please submit to me the copy of the rated license application by the end of the business day on October 1, 2025, if you have decided to apply for the 2-4 star license and which pathway this program will choose. You were reminded that a compliance history score of seventy five percent or above must be maintained to receive a rated license. The following violations were observed during today’s visit: Violation Number Comment Rule 1927 Openings in equipment, steps, decks, and handrails were not smaller than 3 1/2 inches or greater than 9 inches to prevent entrapment. The protective covering used to cover the entrapment hazards (spaces between posts larger than 3.5 inches) on the railing on the deck and fence surrounding the fence around the outdoor play area was very loose and pulled away from the railing and fence. .1719(a)(23) TECHNICAL ASSISTANCE: -We discussed that the backing around the fence used for the outdoor play area to prevent entrapment hazards will need to be pulled tighter so the children can not place their limbs through the posts. This will need to be done for the railing around the deck and stairs as well. CONSULTATION: -We discussed that the smoke detector located in the living area was chirping during the visit which means the battery will need to be replaced. A good rule of thumb is to change the batteries two times a year when we turn the clocks in the fall and spring. -For continued eligibility in the subsidized childcare program, you must achieve a three star or higher rated license at the end of the temporary license period. -As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. When you are asked to send a compliance letter to me, it is expected and required that your compliance letter be sent to me no later than the date indicated in this documentation. Your letter must include the following: -Name of your program -ID number of your program -Date of Letter -Violation number -Explain how you corrected the violation (send supporting documentation to show compliance) -Describe your plan to make sure you will not have that same violation in the future -Name or signature of the person who wrote the letter. (If emailing the letter, your email address will be considered your digital signature.) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score.
