The last annual compliance visit was completed March 27, 2025, and prior to today’s visit, the eighteen-month compliance history score was 94%, which is above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Three-Star License issued July 24, 2024. The License was based on the program earning five (5) points in education standards, two (2) points in program standards, and one (1) quality point by all teachers having an AAS or higher in ECE/CD. The information on record with DCDEE was verified as correct by Jacklyn Biagas and the owner’s corporation status, Precious Years Learning Center Inc, was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of superior completed December 8, 2025. Fire Classification of approved completed June 17, 2025. Jacklyn Biagas, Administrator, was present and assisted me while I observed the indoor and outdoor areas. Two children arrived during the visit. The children and their mom were greeted by staff. They washed their hands and proceeded to their classrooms. Children were observed playing with dinosaurs, zoo animals, and large transportation vehicles. I observed hand washing, sanitation, and lunch. Lunch consisted of mac/cheese, chicken nuggets, corn, apple sauce, wheat bread, and milk. Both children departed during the visit. At the completion of the visit, the Visit Summary was reviewed with J. Biagas. The following violations were observed: Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. One staff member did not have a medical report on file prior to employment. A medical report was on file during the visit with a later date than the employee's hired date. 10A NCAC 09 .0701(a) 1041 Prior to employment a Criminal Background Check was not completed. One new staff member, A. Word (DOE 1/28/26), did not complete a criminal background check prior to employment. G.S. 110-90.2(b) 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. A medical exam was not on file for one child within 30 days of enrollment. The child's medical exam was on file during the visit, dated more than 30 days of the child's enrollment date. Each of the following pathway options were discussed: Program Assessment Pathway, Classroom and Instructional Quality Pathway, and Accreditation and Head Start Pathway. Education requirements were also reviewed. Based on today’s conversation you will be pursuing the Classroom and Instructional Quality Pathway (2). Your goal is to submit your application on August 15, 2026. Go to DCDEE website, providers, documents and forms, to access the application for assessment for a Rated License for Centers. ABCMS – CBC Provider Portal Reminder: Remember, add new employees on your facility’s ABCMS roster and remove terminated employees. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Use the ABCMS Provider Portal to update information regarding new hires or residents. The information should be updated in ABCMS on an ongoing basis as staff members are hired and when employment is terminated. The facility's last sanitation inspection was conducted on 2/26/24. The last annual compliance visit was completed April 2, 2024, and prior to today’s visit, the eighteen-month compliance history score was 94%, which is above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Three-Star License issued July 24, 2024. The License was based on the program earning five (5) points in education standards, two (2) points in program standards, and one (1) quality point by all teachers having an AAS or higher in ECE/CD. The information on record with DCDEE was verified as correct by Jacklyn Biagas and the owner’s corporation status, Precious Years Learning Center, Inc., was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of superior completed February 28, 2024. Fire Classification of approved completed August 12, 2024. Jacklyn Biagas, Administrator, was present and assisted me while I observed the indoor and outdoor areas. Upon my arrival, there were no children in care. The child left just before I arrived at approximately 10:30 am. At the completion of the visit, the Visit Summary was reviewed with J. Biagas. The following violations were observed. Violation Number Comment Rule 899 The child care operator did not comply with all State laws, federal laws and/or local ordinances that pertain to child health, safety, and welfare, as required by General Statute 110-91. The facility's last sanitation inspection was conducted on 2/26/24. GS 110-91 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. Two staff members did not have medical reports on file prior to employment. This is a Repeat Violation from 9/10/24. 10A NCAC 09 .0701(a) 1851 The operator did not notify the parent of each child enrolled in writing of the smoking and tobacco restriction. One child's file did not have a parent's signed acknowledgment form of the smoking and tobacco restriction. .0604(j) The violation must be corrected immediately. I received fire inspection during today’s visit. Jacklyn Biagas, Administrator, was present and assisted me while I observed the indoor and outdoor areas. Upon my arrival, there were two (2) children in care. One child was sitting at a table and another child was washing her hands. First, the children participated in a playdough activity. The children picked a color of playdough and rolling pin to use. Ms. Biagas, demonstrated how to make various body parts like, the body, eyes, and feet. She also pointed to body parts for the children to name. She gave affirmation and praise by saying, “very good”. Next, the children washed their hands and cut different colors of paper to make a collage. Ms. Biagas gave the children step-by-step instructions to help them cut properly. The children sung songs like, “If you Happy and you know it”. I observed, toileting, hand washing, sanitation, and lunch. Today’s lunch consisted of hot dog, bun, French fries, mix fruit, and milk. At the completion of the visit, the Visit Summary was reviewed with Ms. Biagas. The following violations were observed. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The facility's current fire inspection (dated 7/11/23) was not mailed to DCDEE within one week of the inspection. 10A NCAC 09 .0304(a) 808 The outdoor premises were not clean, drained and free of litter and hazardous materials grass and other vegetation in a manner which does not encourage vermin. The grass on the playground was not cut. 15A NCAC 18A .2832(a) The violation must be corrected immediately. Ask more open-ended questions like, “Tell me something about your picture”. This helps with language and vocabulary. During today’s visit we discussed: Your sanitation inspection expires on September 9, 2023. You stated you contacted Environmental Health and will call again. Mailing Fire Inspection: A violation being cited regarding not mailing the facility’s current fire inspection to DCDEE within one week of the inspection. Refer to child care rule 10A NCAC 09 .0304(a). Consider mailing the fire inspection to DCDEE immediately after the inspection.
