The previous Annual Compliance Visit was completed April 18, 2024, and prior to today’s visit, the eighteen-month compliance history score was 97%, which is at or above 75%, required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Four-Star License issued May 6, 2024. The License was based on the program earning five (5) points in education standards, five (5) points in program standards, and one quality point by choosing to meet a programmatic[DM1.1] option by using staff benefits and infrastructure of parent involvement. [DM2.1]The information on record with DCDEE was verified as correct by Iris Zwicker and the owner’s corporation, Mommy’s Day Care, LLC status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of superior completed December 8, 2025. The inspection form was received during the visit. Therefore, the information was put into the Regulatory system. Fire Classification of approved completed December 8, 2025. Iris Zwicker, Administrator, was present and assisted me while I monitored the indoor and outdoor areas. Upon my arrival, there were thirteen children in care. Children were sitting at tables participating in a group activity. They sang songs like, good morning and wheels on the bus. They chanted the days of the week in Spanish/English. Children’s names were called and they were asked about the first letter of their name. A musical interactive movement song was played. The children would freeze when the music stopped playing. Next, children went outdoors. I observed toileting, hand washing, sanitation, and lunch. Lunch consisted of meatballs, mashed potatoes, gravy, apple sauce, and milk. At the completion of the visit, the Visit Summary was reviewed with I. Zwicker. The following violations were observed. Violation Number Comment Rule 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The facility's ABCMS Roster was not available for review. G.S. 110-90.2 & .2703(r) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. Two staff members did not complete the health and safety trainings within five years of completing the initial trainings. .1103(b) The violations must be corrected immediately. Mail or email a written, dated, and signed statement to me by 5:00 pm on April 15, 2026, describing how and when the violations were corrected and how the facility plans to maintain compliance in the future. If the compliance letter is not received by the due date, an additional visit will be conducted to verify that violations have been corrected. You chose the Classroom and Instructional Quality Pathway. The following documents were reviewed and monitored for compliance today: • Application for Assessment for a Rated License for Centers – The facility meets enhanced staff/child ratio. • Family and Community Engagement Standards (Child Care Centers) – Family and Community Engagement Foundational Practices and three additional options: C-1, EL-2, and EO-1. • Facility Continuous Quality Improvement (CQI) Plan – Goal/Plan was established; reason for goal, timeframe to achieve goal, and steps to achieve goal were documented and available for review. • Individual Continuous Quality Improvement (CQI) Professional Development (PD) Plan (Child Care Center) – Individual PD plans were on file and available for review. Goals were identified for each employee, time frame to achieve the goal, and steps to achieve the goal were documented on each plan. • Staff Information and Education – Education was verified in DCDEE WORKS. The administrator, lead teacher, and teacher qualified at an education staff level of 4. The facility’s highest Education Star Level eligibility is a 4 Star. • The Curriculum and formative assessment - Creative Curriculum for Preschoolers. The required formative assessments for each child and coaching/training opportunities will be monitored during the next annual compliance visit. Based on the information provided, the facility is eligible for a four (4) Star License. The completed packet will be submitted for review and processing. A new license will be mailed directly to you and must be posted upon receipt. You must mail the old license to me once the new one is received, as it remains the property of the State of North Carolina. Facility Specific TA/Consultation/Reminders: ABCMS – CBC Provider Portal Reminder: The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. The information should be updated in ABCMS on an ongoing basis as staff members are hired and when employment is terminated. This satisfies the requirement to notify the Division of new child providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Should you need assistance contact the Criminal Background Check Unit at (919)814-4401 and someone will assist you. Health and Safety Trainings You and another employee’s Health and Safety Trainings have expired. Health and Safety Training courses are available in DCDEE Moodle, free of charge. Log into DCDEE Moodle with your NCID, search for and register for the required trainings. Health and Safety training topics must be completed within the first year. After the first year, staff must complete the health and safety training topics at least once every five years. Documentation of training must be kept on file for review. Complete the health and safety training record as found on the DCDEE website. NCDHHS Responds to Increase in Measles Cases: Measles is on the rise globally and in North Carolina. As a highly contagious, airborne disease, even one case is a major priority for public health. NCDHHS is taking action by launching new outreach resources for child care providers, families, and community stakeholders to help curb the spread. Child care providers can share information about vaccinations with the families served and be informed with the most current information. Flu Season Precautions for Child Care Since so many children spend part of their day in child care in North Carolina, it is vitally important that child care providers follow the child care licensing rules pertaining to health and safety during the flu season. This includes recognizing when children are too ill to be in child care, separating children who become ill in child care, and preventing the spread of illness. Children should be excluded from child care when any of these symptoms are present: • fever of 101° F or higher • sudden onset of diarrhea • episodes of vomiting • strep throat or • if the child is unable to participate in regular activities. Child care providers who have questions about suspected flu symptoms of young children, or who need advice on health issues, may contact the NC Child Care Health and Safety Resource Center at 1-800-367-2229. Upon our arrival, there were no children in care due to the facility being closed for Spring Break. Eleven children are enrolled in the program. I will return sometime within two weeks to verify children in care. At the completion of the visit, the Visit Summary was reviewed with I. Zwicker. The following violations were observed: Violation Number Comment Rule 1314 Emergency information did not name child's health care professional. One child's Application and Emergency information did not include a Health Care Professional or Hospital Preference. .0802(c)(2) 1908 A child's file did not have a statement with parent signature acknowledging receipt and explanation of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy and/or the acknowledgement did not have all the required information. One child's file did not include a parent signed acknowledgement form regarding the Prevention of Shaken Baby Syndrome and Abusive Head Trauma Policy. .0608(b)(1-6) The violation must be corrected immediately. Mail or email a written, dated, and signed statement to me by 5:00 PM on April 29, 2025, describing how and when the violation was corrected and how the facility plans to maintain compliance in the future. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 Technical Assistant/Reminders/Discussions: A violation was cited for one child not having an acknowledgement form signed by a parent or guardian for the Shaken Baby Syndrome and Abusive Head Trauma Policy: SBS/AHT is the name given to a form of physical child abuse that occurs when an infant or small child is violently shaken and/or there is trauma to the head. Shaking may last only a few seconds but can result in severe injury or even death. According to NC Child Care Rule 10A NCAC 09.0608, each child care facility licensed to care for children up to five years of age shall develop and adopt a policy to prevent SBS/AHT. This policy must be provided to parents and staff working at the facility and an acknowledgement signed by both parents and staff verifying receipt of the policy. AHT can occur in children up to 5 years of age. Shaken baby syndrome can lead to serious conditions including brain damage, problems with memory and attention, blindness, or hearing loss; intellectual, speech or learning disabilities; and developmental delays. The signed acknowledgement form must be on file for staff and children on day one. Please refer to the resource on the DCDEE website left with you today: Children’s File Checklist – Center. Health Care Professional/Hospital Preference: A violation was cited for a child not having a hospital preference and a health care professional included on their application or emergency information form. This information is needed in case of an emergency and must be on file day one of enrollment. Refer to child care rule .0802(c)(2).
