You are currently operating with a temporary license that is effective from October 6, 2025, through April 6, 2026. You, your husband, your mother, and two (2) children are the only household members at this location. Upon arrival, you were present providing care to five (5) children ranging in ages from one (1) to three (3) years. Your mother, who works as a substitute, was also present and assisted you today. Your two (2) children were present. One (1) of your children remained in the kitchen area/living area of your home with you during the visit. A total of six (6) children were present, two (2) of which are your own children, both are preschool aged. While I met with you today, your mother cared for the children and one of your children remained with us in the kitchen/living area of your home, separate from the room you have designated for child care. Your mother cared for the children and interacted with them during the visit. You switched with your mother so she could prepare lunch and care for one of your children. You assisted the children with bathroom routines and hand-washing routines. The following violations were documented today: Violation Number Comment Rule 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. Documentation was not on file verifying outdoor inspections were completed since licensed October 6, 2025. 10A NCAC .1721(e)(5)(A-F) 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. A health assessment was not on file for two (2) children. GS 110-91(1); 10A NCAC 09.1721(a)(1) The violations documented must be corrected immediately. Email or mail me a compliance letter addressing the correction of the violations on or before December 24, 2025. Ensure you specify the correction of the violations. If a program’s compliance history falls below 75% over an eighteen-month time frame, an administrative action may be imposed. Administrative Actions can be issued based on different types of violations documented, repeat violations, failure to comply with NC Child Care rules, not correcting violations, and based on compliance history. The violations documented today may affect your current compliance history. CHILD CARE RULES: You can access the NC Child Care rules from the Division of Child Development and Early Education’s website: https://ncchildcare.ncdhhs.gov, click on the “provider” tab, then “child care rules” tab. Download a copy of the NC Child Care rules onto your computer so you have access to the rules and can reference them as needed. The most current NC Child Care rules were effective July 1, 2025. RULR REMINDERS: CHILDREN’S RECORDS: All children’s records were monitored today. A medical exam was not on file for two (2) children. The parents completed the top portion of the Children’s Medical Report form, but the bottom portion for the physical was not completed for two (2) children. You stated both children were enrolled before you became licensed. You further stated you think you may have discarded original medical exam after the parent of this for one of the children when they brought you an updated shot record. You stated you will contact the parents of both children to obtain a copy of each child’s medical report/health assessment. A health assessment can be accepted if completed within the last 12 months prior to enrollment. If the health assessment is older than 12 months, an updated health assessment is required and must be on file within 30 days of enrollment. MONTHLY FIRE AND OUTDOOR INSPECTIONS: Ensure to conduct monthly fire drills using the smoke detector, when children are present, at different times and days each month and to document when you complete the drills on the applicable form. Maintain documentation of the drills on file for review. You can use your phone to set reminders of when to conduct the monthly fire drill and outdoor inspection. You stated you conduct outdoor inspections each day prior to going outdoors; however, no documentation was on file for review. EMERGENCY PREPAREDNESS AND RESPONSE TRAINING: When asked, you have not yet completed the EPR training. This training is required to be completed within the first year of licensure, on or before October 6, 2026. After you complete the training, you will use the Risk Management Portal to access and complete your EPR Plan. The EPR plan is required to be completed within four months after completion of the EPR training. Once the EPR plan is complete, print a copy of your EPR Plan and maintain the plan in a notebook or folder along with copies of other required information related to EPR. Use the EPR checklist to ensure all required forms and information is on file with your EPR Plan. The EPR Plan must be reviewed annually and maintain documentation of the annual review on file. You can review the plan and print out the last page reflecting when the review was completed. CAPACITY: Ensure you are caring for no more than five children at any given time. With your two children, you have six children enrolled. At no time can any more than five children be present in the area used for child care with you or your mom, who works as a substitute. Your mother also cooks meals for you during the day. If you have all five children present, one of your children remains with your mom or your husband in another area of the home, separate from the other children enrolled. MEDICATION: When asked, you are not administering any medication. TRANSPORTATION: When asked, you are not providing any transportation. ON-GOING IN-SERVICE TRAINING: The ongoing training hours (in-service) will be monitored at your first annual compliance visit based on when you were initially licensed. No training hours were monitored today. Complete the On-going Training Log and attach your certificates of completion for each training to the log for verification. HEALTH AND SAFETY TRAINING: We discussed completing the required Health and safety trainings via Moodle for free. You are required to complete these trainings on or before October 6, 2026. When asked, you are aware of how to access the trainings via Moodle. Ensure these trainings are completed every five years. Continue to maintain copies of your training certificates on file for verification. As a reminder, you access the trainings via Moodle by: 1. Click “2025 CCDF” Tab RECOGNIZING AND RESPONDING TO SUSPICIONS OF CHILD MALTREATMENT: The Recognizing and Responding to Suspicions of Child Maltreatment training is required to be completed within the first 90 days of employment/licensure. You are due to complete this training on or before January 6, 2026. You can access the training by going to the Prevent Child Abuse NC website: www.preventchildabusenc.org. RATED LICENSE INFORMATION: During the visit today, we reviewed the requirements for a star rated license have changed and programs are now being evaluated via the QRIS as of July 1, 2025. We reviewed Pathway 1 and 2 today, and you stated you plan on applying using Pathway 2. We discussed the requirements for Pathway 2 and referenced appliable forms and information today. I showed you how to access information regarding the QRIS Modernization by going to the “What’s New” tab from DCEEE’s website. We also discussed curriculum and formative assessments today. We discussed submitting your official transcripts to the Workforce Education Unit at DCDEE for education evaluation. We referenced your WORKS account today. We further reviewed that if you do not earn at least a Three Star Family Child Care Home license at the conclusion of your six-month temporary time, you will not be eligible to enroll or care for children enrolled in the subsidy program. We will finalize your initial Rated License assessment during a future monitoring visit. When asked, none of the children currently enrolled are participating in the subsidy program. You stated you will keep me updated on your progress for Pathway 2. Feel free to contact me with any questions.
