The visit was conducted with you, Alexandra Denton, Administrator. You provided me with applicable program, staff, and children’s records for review. The NC Secretary of State website was viewed before the visit and Choice Care Playschool, Inc. was current/active as of August 1, 2025. The facilities contact information was reviewed and confirmed with you. This program currently operates with a Five star rated license effective June 8, 2022. The permit restrictions were in compliance including 1st shift, meets enhanced ratios, meets enhanced space, and meets enhanced ratios minus one. The program’s compliance history was 99 percent prior to today’s visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. A checklist was used to note the requirements I monitored today. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios in compliance. Children and staff were participating in free play in activity centers, outdoor gross motor play, lunch, and rest time. A sample of eight children’s records were reviewed. A total of six staff files were reviewed including three new staff and three existing staff. NC Pre K requirements were not monitored as the program is on summer break. NC Pre K students will begin on September 2, 2025. The last monthly fire drill was practiced on July 31, 2025. The last shelter-in-place drill was practiced on May 8, 2025. The last monthly playground inspection was completed on July 16, 2025. The Emergency Preparedness Response Plan was reviewed and published on January 15, 2025. The Emergency Medical Care Plan was updated on October 31, 2024. The most recent fire inspection for your facility was approved on April 9, 2025 for day time care only. The most recent sanitation inspection for your facility was conducted on April 4, 2025. A superior sanitation classification was issued with 4 demerits noted on the grade card. The most recent lead water test results were completed on September 19, 2023. Lead water testing must be completed every three years. The following violations were observed: Violation Number Comment Rule 1325 Parent’s statement includes the child’s name and date of enrollment and the date the parent signed the statement. One child four years of age that enrolled on 7/14/25 did not have a signed statement that included the date of enrollment for the child. .1804(b) 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. As of today’s visit, all staff members had not been added to the ABCMS Provider Portal facility roster. G.S. 110-90.2 & .2703(r) The violations documented above must be corrected immediately. Send me a letter of compliance describing in detail specifically how each violation was corrected and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit and license ID number in the letter. I must receive your compliance statement by August 18, 2025. Please send your letter to Beth.Archer@dhhs.nc.gov. Failure to correct the violations and send the written statement by the due date may result in an unannounced follow-up visit being conducted or an administrative action may be recommended. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. If you state in your violation correction letter that corrections have been made when they have not, it will be considered falsification of information. Technical Assistance was provided on the following: -Each child must have a verification of receipt/review of the center’s discipline policy that includes the child’s name, the date of enrollment, the signature of the parent, and the date the parent signed. We discussed that the enrollment packet for this child came from another one of the Choice Care facilities and did not include the correct verification form regarding the discipline policy. I encouraged you to meet with the owner and other administrators of the Choice Care organization to make sure that all enrollment packets have the needed information for compliance. -You have created your ABCMS Provider Portal and you currently have one current qualified staff member and one current employee with an expired qualification listed on your roster. There are ten previous staff members that are listed on the roster as well. You must verify your facility roster to ensure current staff are noted on the roster within the next two weeks. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. Should you need assistance please contact the Criminal Background Check Unit at (919) 814-8401 and someone will assist you. I emailed you the ABCMS provider portal access guide for reference. Consultation: We discussed the following during the visit: -There should be adequate light and appropriate volume for music during rest time. I encouraged you to turn on bathroom lights, partially open doors and/or blinds to make sure there is enough light throughout the classroom setting. We discussed that volume for music should be low enough to be able to hear a child from across the room. -One employee that was hired 7/19/23 needs three additional on-going training hours. Please send me verification of the completed training by no later than August 14, 2025. The Secretary of State website was checked on August 13, 2024, and your business Choice Care Discovery, LLC, is active and in good standing. Upon arrival, the purpose of today’s visit was reviewed with you, Alexandra Denton, Administrator. Your program currently operates with a five star license effective March 22, 2019. The permit restrictions were in compliance including 1st shift, meets enhanced ratios, meets enhanced space and meets enhanced ratios minus one. The NC Secretary of State website was reviewed on August 31, 2023 and Choice Care Playschool, LLC was listed as current/active. You reported no changes to the ownership of the facility. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. You conducted the walkthrough of the facility with me. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas and outdoor play time. The center's compliance history prior to today’s visit was reviewed with the operator. The program’s compliance history was 96 percent as of August 31,2023. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Seven children’s records were reviewed. An additional visit was scheduled for September 1, 2023 to review staff records. Any additional violations cited during this visit will be added to the Annual Compliance visit violations. The most recent sanitation inspection for your facility was conducted on June 26, 2023. A superior sanitation classification was issued with five demerits. The most recent fire inspection for your facility was conducted on April 27, 2023. The facility was approved for day time care.
