F. Lineberry, Assistant Administrator, assisted me with the visit. Your program currently operates with a one star license issued August 14, 2023 due to a capacity increase. The last annual compliance visit was conducted January 15, 2025. The sanitation inspection was completed October 3, 2025 with a “Superior” classification. The last fire inspection was conducted on October 12, 2025 and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety five percent as of January 12, 2026. You visited each indoor and outdoor space with me. Children were observed playing outside, toileting, preparing for and eating lunch, playing in centers, completing table art activities, having story time and resting. Infant/toddlers were observed having free play and napping. Lunch was observed and all meals and snacks are brought from home. The following violation(s) were documented today- Violation Number Comment Rule 1867 The depth of the loose surfacing was not based on critical height of the equipment. The mulch surfacing in front of the slide on the stationary piece of equipment was not at least six inches deep. I suggested pulling mulch out from under the based of the slide to meeti the depth requirements in front of the slide. .0605(k)(1-4) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The program’s compliance history was ninety four percent as of July 30, 2025. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. Supervision and staff interaction were appropriate for the children in care. Children were observed having free play in centers, completing teacher directed activities, having story time, toileting, preparing for and eating lunch. All meals and snacks are provided by parents and an opt out form is on file for each child. There was one violation documented and discussed today. Effective immediately, when the teacher changes diapers, she must call another staff person into the room to assist with supervision. It is recommended to hire another staff to assist in the infant/toddler room, especially in the mornings until nap time. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 Technical Assistance- The Governor signed Senate Bill 425 (now Session Law 2024-34), Health and Human Services Omnibus Bill, which includes the Quality Rating Improvement System (QRIS) Modifications recommended to the Legislature by the NC Child Care Commission to update QRIS and create additional pathways for licensed child care facilities to earn star rated licenses. The Child Care Commission is currently in the process of drafting proposed child care rule language to begin the rulemaking process for implementing the new requirements. The rulemaking process will include publishing of proposed revised rules as well as a public hearing and public comment period. Hold Harmless provisions have been extended and star rated license assessments for licensed child care providers will be conducted by provider request only, until the QRIS rulemaking process has been completed. Star rated license assessments are still required for new child care programs. Mrs. Lineberry is interested in increasing the center’s stars. I instructed her to have all staff register in Workforce on the DCDEE website and submit official transcripts as soon as possible. Staff had questions regarding storage of diaper cream. I explained if the diaper cream is over the counter cream it can be stored five feet up out of reach. If the cream is prescribed by a physician, it must be stored under lock and key. The environmental health inspector required them to move the diaper change table into the bathroom closer to the sink. There is only one staff person and she cannot see the other children when diaper changing. Effective immediately, when the teacher changes diapers, she must call another staff person into the room to assist with supervision. It is recommended to hire another staff to assist in the infant/toddler room, especially in the mornings until nap time. Will also follow up with the health inspector about placing the diaper change table directly outside the bathroom door.
