Your program currently operates with a three-star license, issued October 27, 2022, earning 4 points in the education component, 2 points in the program standards component meeting enhanced space and 1 quality point for the center having a combined turnover rate of 20% or less for the administrator, program coordinator, lead teacher, teacher and group leader over the last 12 months. The last annual compliance visit was conducted April 1, 2024. The sanitation inspection was completed August 21, 2024, with a “Superior” classification. The last fire inspection was conducted on January 6, 2025, and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-three percent as of March 25, 2025. The NC Secretary of State website was reviewed on March 25, 2025, and Little Pines Academy LLC was listed as current- active. You visited each indoor and outdoor space with me. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, transitions, and personal care routines. The caregivers were interacting and meeting the developmental needs for each of the children. Files for staff were reviewed. Rated license requirements were reviewed to ensure compliance was maintained with the current program and education components as well as the quality point. The following was cited during today’s inspection. Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. KO did not complete their Training within 90 days of employment. Teachers were seen walking around the room and playground engaging with children. During the visit, I inspected the indoor learning environment and outdoor learning environment. I found ratios, supervision and interactions to be in compliance. I also reviewed all program records to include, staff records, trainings, children records, emergency drills, as well as all child care requirements. The following was cited during today’s visit. Clean Classrooms for Carolina Kids program is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. We discussed using the staff/substitute file checklist located on the divisions website to help record required staff records and timelines. We also discussed reviewing staff records twice a year to ensure that training and certifications are completed prior to the certification expiring. We also discussed the importance of having prescription medicine with the original label and container. This helps ensure that staff are able to verify and confirm that the child is receiving the right medication. The allegations are as follows: There is a concern that adequate precautions are not taken to prevent the spread of infections. Examples: Handwashing as required, sanitizing or disinfecting surfaces. During today’s visit Staff/child ratio, group size, supervision, use of licensed space, space capacity, license restrictions, and the indoor learning environments were monitored. The license and emergency care plan were posted. Children were participating in group activities. Teachers were seen interacting with the children. Children were washing their hands after using the bathroom. I spoke will administrator, Aslynn Millen, and informed her of the allegations that were reported. She took me to the two-year-old classroom where I could see the disinfectant and sanitizer they are using to clean surfaces before and after activities. Ms. Millen informed me that the children wash hands before and after meals, when they come back in form outside and after activities that require hand washing. I asked Ms. Millen if the children wash hands upon arrival. She stated that children come in and put away their belongings and then join the group in their activities. I reminded Ms. Millen that children are required to wash their hands upon arrival at the center. We also discussed illnesses throughout the center. She informed me that the center keeps a log of illnesses in each class. When they see that certain illnesses are impacting the center, they send out a message to parents informing them of the signs and symptoms of the illness. Based on information received from interviewed staff and my observations, the finding regarding the allegation sanitation was substantiated. The following violation(s) were observed during today’s inspection. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The center had a fire inspection on August 23, 2023 but a division fire inspection form was not filled out during the time of the inspection. 10A NCAC 09 .0304(a) 608 Children did not wash their hands upon arrival at the center, after each visit to the toilet, before eating, before and after water activity play, after outside play, and after handling animals or animal cages. Children did not wash hands upon arrival at the center. This was corrected during the visit by the administrator notifying staff and parents to have children wash hands upon arriving at the center. 15A NCAC 18A .2803(c) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before December 18, 2023, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. A violation was documented for failure to maintain compliance with sanitation requirements. Violations of this nature directly impact the safety of the children while in your care. In addition, these types of violations have the greatest negative impact on your compliance history. During the visit, we discussed that repeated violations of this nature during consecutive visits may lead to an administrative action against your license.
