Lilia Purington, administrator/owner, assisted me with the visit. This program operates with a three (3) star license, issued September 1, 2017, earning four (4) points for education, two (2) points for program and one (1) for quality point for complying with the education option. License restrictions include daytime and overnight, capacity of 12 on each shift, meets enhanced space. The last annual compliance visit was conducted May 15, 2025. The last sanitation inspection was conducted on September 25, 2025. The last fire inspection was conducted on November 4, 2025. The program compliance history was ninety-three (93) percent as of 5/3/2026 and was reviewed with Mrs. Purington. I visited each indoor and outdoor space. Children enrolled were observed in free play in the centers, outdoor play, participating in teacher directed activities and personal care routines. Infants enrolled were observed during tummy time, personal care routines, free play and teacher directed activities. Teachers were interacting with the children and attending to their needs. Breakfast consisted of pancakes, scrambled eggs, bananas and milk. The following violation was observed during the visit: Violation Number Comment Rule 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The facility information has not been entered into the ABCMS portal. G.S. 110-90.2 & .2703(r) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before May 19,2026, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. Technical assistance was also offered on completing the ABCMS portal training and entering the facility’s roster in the portal. All current staff should be entered and new staff or staff who terminate their employment should all be entered with five business days of the additions or changes. You must complete the ABCMS portal training in Moodle prior to being allowed access to the portal to enter your current staff. This pathway includes the following: • Enhanced Ratio(required) and/or Enhanced Space (optional) • 50% of all Lead Teachers and 50% other educators meet 5 Star Education Standards • Family and Community Engagement Foundational Practices AND 3 additional choices • CQI Plan for Facility and Individuals • Approved curriculum for all ages served • Child Observations and assessments • Coaching/training options for administrators and lead teachers. • NCRLAP Self Study I again left copies of the Continuous Quality Improvement and Professional Development Plan, work experience form, Pathway #2 -Classroom and Instructional Quality for Child Care Centers and Centers Located in a Residence (keep those copies and continue working on completing them prior to your next visit.) You stated that you will review the approved curricula and assessment tools at a later date and decide if you are going to continue using your current curriculum or go with another option. Mrs. Purington stated that they have already begun reviewing on the Facility CQI plan for both the center and staff. This program operates with a three (3) star license, issued September 1, 2017, earning four (4) points for education, two (2) points for program and one (1) for quality point for complying with the education option. License restrictions include daytime and overnight, capacity of 12 on each shift, meets enhanced space. The owner/administrator, Lilia Purington, assisted me with the visit. The last annual compliance visit was conducted on May 15, 2025. The facility’s sanitation and fire inspections are current and up to date. The program’s compliance history was ninety-seven (97) percent as on 11/30/2025 and was reviewed with the administrator. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, and all indoor and outdoor areas were monitored. I observed children in the indoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in free play in activity areas, personal care routines such as handwashing and toileting and transitions. The caregivers were interacting and meeting the developmental needs for each of the children. Two infants enrolled were observed actively participating in free play. Rated license requirements were reviewed to ensure compliance was maintained with current program and education components as well as the quality point. Breakfast consisted of whole wheat cereal, whole wheat pancakes, pears, carrots and milk. The following violations were cited during today’s visit: Violation Number Comment Rule 451 For children under three years of age, an open area that allows freedom of movement was not available, both indoors or outdoors. Upon my arrival, there were four (4) play pens in the area used by the children that restricted their movement to freely move. .0510(e)(4) 860 Balloons were accessible to children. Four (4) balloons that were full of air where in the children's play area. .0604(q) 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. Operator was not able to show documentation for the entire month of November for two infants ages nine (9) and eleven (11) months. .0606(g) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. When the playpens are kept in the main space used by the children through out the day, it can make accessible areas to the children seem in accessible. It also does not leave them very much room to move around the space freely. Technical assistance was also provided on ensuring that balloons are not available or accessible to the children. Ballons are considered a choking hazard for small children. Other alternatives to balloons could be playing with bubbles or pinwheels with teacher direction As a reminder, children under three are not allowed screentime. It would be best practice for you since all of your current children enrolled are one and under, to remove the TV from the area used for care. During this visit we discussed the QRIS Modernization Pathways to the Stars. Mrs. Purington has decided to follow the option #2 Program Assessment for Child Care Centers and Centers Located in a Residence (CLIAR). Studies have shown that excessive screentime for children under the age of three can negatively impact a child’s development. It can lead to issues with language, attention, social skills, and emotional well being. It can also affect sleep, vision and physical health. All children enrolled need to have open space that allows them to freely move through out the class. When you place materials or items such as playpens, within the interest areas or the areas that the children use and it does not have an intended purpose at that time, you prohibit or send the message to the children that that area or space is not available for use. If it is not nap or rest time, put the play pens up until it is time for their use. Lilia Purington, administrator/owner, assisted me with the visit. This program operates with a three (3) star license, issued September 1, 2017, earning four (4) points for education, two (2) points for program and one (1) for quality point for complying with the education option. License restrictions include daytime and overnight, capacity of 12 on each shift, meets enhanced space. The last annual compliance visit was conducted June 20, 2024. The last sanitation inspection was conducted on September 18, 2004, earning a superior rating. The last fire inspection was conducted on November 5, 2024. The program’s compliance history was one hundred (100) percent as of 5/14/20254. I visited each indoor and outdoor space. Children enrolled were observed during teacher directed activities, free play, watching television, routines such as diapering, toileting and handwashing and outdoor play. The following violations were cited during the visit: Violation Number Comment Rule 451 For children under three years of age, an open area that allows freedom of movement was not available, both indoors or outdoors. Seven (7) play pens were placed throughout the space used by all of the children who ranged in ages from five (5) months to 3 years of age and did not allow for free movement. .0510(e)(4) 544 Screen time was offered to children under three years of age. Upon my arrival, seven (7) children ranging in ages from to five (5) months to 3 years of age were watching a children's program on tv. .0510(f) 871 Center staff did not comply with the safe sleep policy.
