Laura Ingram, administrator, assisted me with the visit. This is a religious sponsored that operates under a G.S. 110-106. The Notice of Compliance was issued February 17, 2011. The current restrictions include daytime care only and uses corporal punishment. Age range 0-5 years; capacity is 27. The program’s compliance history was ninety-four (94) percent as of June 7, 2026, and was discussed with the administrator. The NC Secretary of State website was reviewed during the visit and Calvary Memorial Church. was listed as current-active. The Notice of Compliance was posted and the restrictions were in compliance. The last annual compliance visit was conducted on August 20, 2025. The last sanitation inspection was conducted on 1/6/2026 and earned a superior rating. The last fire inspection was conducted on 9/3/2025. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. Children present during the visit were observed participating in outdoor play. The caregivers were interacting and meeting the developmental needs for each of the children. Supervision and staff/child ratios were in compliance. No new staff have been employed since my last visit. The children brought meals from home that met the USDA guidelines. The following violation was observed during today’s visit: Violation Number Comment Rule 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The facility not the staff have been added to the ABCMS portal as of today's visit. G.S. 110-90.2 & .2703(r) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before June 23, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Children enrolled were observed in free play in the centers, outdoor play, participating in teacher directed activities and routines such as diapering, toileting and handwashing. Lunch consisted of pizza, cucumbers, ice cream and milk. The following violation(s) were documented during today’s visit: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The fire inspection conducted on 6/21/2023 was not submitted within 7 days of completion. 10A NCAC 09 .0304(a) 114 A summary of the NC Child Care Law was not given to a parent of every child enrolled in the center. There was no documentation on file that summary was given for one child (D.C.). GS 110-102 811 Potentially hazardous items including but not limited to power tools, nails, chemicals, propane stoves, lawn mowers, gasoline, or kerosene were not stored in locked areas, removed from the premises, or made inaccessible to children. Hand sanitizer was not stored five feet or higher when not in use. .0604(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Aerosol cans and cleaners not locked in storage. .2820(b) 1834 Application did not have a medical action plan attached for any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services. No medical action plan was on file for one child (D.C.) that had emergency medicine at the facility. .0801(b) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately on or before October 28, 2023, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Laura Ingram, administrator, assisted me with the visit. Janet Edwards, Licensing Supervisor, accompanied me during the visit. This program currently operates under a GS 110-106. The Notice of Compliance was issued February 17, 2011. The Notice of Compliance restrictions include daytime care only and uses corporal punishment. Age range 0-5 years; capacity 40. The last annual compliance visit was conducted on November 29, 2022. The last sanitation inspection was completed on November 28, 2022 with a superior classification. The last fire inspection was conducted June 21, 2023 and the facility was approved for daytime care only. The program’s compliance history was one hundred (100) percent as of October 10, 2023 and was reviewed with the administrator.
