Applicable childcare requirements were discussed with you, Shakeeta Utley, Operator. Your four-star rated license was posted, and you were in compliance with all restrictions noted on the license. During this visit I monitored the indoor and outdoor areas approved for child care. I observed children sleeping on individual cots. One (1) child was not sleeping so they were provided with quiet activities. During this visit children were adequately supervised. In addition, the operator’s file, program records, and all children’s files were monitored. For afternoon snack the children were served 100% apple juice, crackers, applesauce, and water. Overall enrollment and attendance information were collected. The attached Family Child Care Enrollment Worksheet reflects this information. At the conclusion of the visit, the visit summary was reviewed with you, and you were provided with an opportunity to ask questions, seek clarification, and request technical assistance. You were also provided with a copy of this visit summary for your records. Violation Number Comment Rule 908 Health questionnaire was not completed annually. This was not completed annually. The operator completed the health questionnaire during the visit; therefore, it was corrected during the visit. .1703(a)(1) 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The operator didn't complete the ABCMS Portal/Roster. G.S. 110-90.2 & .2703(r) The two (2) violations documented above must be corrected immediately. A signed and dated letter of compliance must be submitted to me no later than April 14, 2026, clearly detailing how each violation has been corrected. You may email your compliance letter to me at leisa.benson@dhhs.nc.gov. Failure to submit your compliance letter by the stated deadline or failure to contact me regarding a revised timeline may result in a follow-up visit, and violations may be cited. If you are unable to meet the required deadline of April 14, 2026, you must contact me prior to that date with a proposed timeline for completing the required corrections. In some cases, this time frame may be extended. Important Notice: If your compliance letter states that corrections have been made and it is later determined they were not, this will be considered falsification of information, and an administrative action may be taken against your license. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen (18) months or during the length of time the facility has operated, whichever is less. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. This program currently operates with a temporary license effective April 8, 2025 to October 8, 2025. Applicable childcare requirements were discussed with you, Shakeeta Utley, Operator. Later during the visit a representative from the Cumberland County Partnership arrived to provide technical assistance. Upon arrival there were two (2) three-year-olds and the operator present in the kitchen eating breakfast, which consisted of waffles, applesauce, and milk. The operator does participate in the food program. After eating the children and operator were observed washing their hands. The children were observed during free choice activities, group time, and playing outside. The children were adequately supervised. Appropriate interactions were observed between the children and operator. All indoor and outdoor areas approved for child care were monitored during the visit. I also monitored program records and operator file. I didn’t review children files because no new children have enrolled since the last visit conducted on May 29, 2025; however, I did monitor to ensure that children’s files were available for review. During the visit you requested to be approved for transportation. I reviewed the transportation requirements with you. I also reviewed with you that you can’t transport children until all requirements have been met and you can only use the vehicle that is approved. During the visit I observed the vehicle that will be used for transportation. You met all transportation requirements except you didn’t have a first aid kit in the vehicle. Also, the tire tread on the left back tire on the driver side was low. Once you have corrected the items listed above and I have received your compliance letter I can approve you for transportation. At the completion of the visit, the Visit Summary was reviewed with you, and you were also given a computerized copy for your records.
