A checklist was used to note the requirements I monitored today. The staff and training worksheets were provided prior to today’s visit. The visit was completed with G. Creech, Owner/Administrator. You were notified of the purpose of the visit upon my arrival, and you were available for consultation at the conclusion of the visit. Kim Barnes, Child Care Consultant, assisted with today's visit. This center currently operates with a three-star license, earning four points in education standards, two points in the program standards and 1 quality point for the administrator having at least (10) years of documented child care and administrative work experience in a license child care facility. This program continues to meet enhanced ratio requirements. The last annual compliance visit was completed on March 18, 2025. The last sanitation inspection was completed on November 3, 2025, earning a “Superior” classification. The last fire inspection was completed on May 1, 2025, and the center was approved for daytime care only. Prior to today’s visit, the facility earned a Compliance History score of 94% over an 18 - month time- period. A copy of the compliance history was provided to you. Children were observed participating in group activities, free play in the indoor and outdoor environment, completing transitions, completing routines such as handwashing and toileting and eating lunch. All staff provided care in a nurturing manner. Staff child ratios were in compliance and adequate supervision was provided for all children. Infants were observed participating in tummy time, interacting with caregivers, and having routine care needs met. I used the Annual Compliance Monitoring Checklist for Child Care and the Child Care Center Item Number Listing (DCD 0357) as a basic monitoring tool to assess compliance with all applicable child care requirements. A computer-generated visit summary was reviewed with you, and a copy was provided to you. All violations were reviewed with you at the conclusion of the visit. The following violation was documented: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. On the preschool playground there is an area of the fence that has been patched. The patch has some sharp points that are accessible to children. The black borders that are used for containing surfacing material, are cracked exposing holes and sharp edges. There are briars running along the side of the fence and are accessible to children. There are several tree roots that are raised causing tripping hazards. 10A NCAC 09 .0601(a) CONSULTATION AND TECHNICAL ASSISTANCE: The violations documented above must be corrected immediately. A signed and dated letter of compliance must be mailed to me by March 19, 2026, describing how and when the violations were corrected. This two-week time frame is established to allow you time for submitting your compliance letter and is not intended to be used as a guide for correcting violations, as they should be corrected immediately. G. Creech, Owner was present and assisted with the visit. This center currently operates with a three-star license, earning four points in education standards, two points in the program standards and 1 quality point for the administrator having at least (10) years of documented child care and administrative work experience in a license child care facility. This program continues to meet enhanced ratio requirements. The last annual compliance visit was completed on April 2, 2024. The last sanitation inspection was completed on February 25, 2025, earning a “Superior” classification. The last fire inspection was completed on May 2, 2024, and the center was approved for daytime care only. Twenty (20) children ranging from ages zero (0) to five (5) years old were observed in care today. Children were engaged in morning routine/activities, free-choice play, eating lunch, completing hand washing and toileting routines, and interacting with caregivers. Lunch consisted of chicken sandwiches on a bun, beans, bananas, and milk. The following violations were documented. Violation Number Comment Rule 1911 An incident report was not completed and mailed to a Division representative within seven days after the incident when medical treatment was required. An incident report completed on February 6, 2025, was not submitted to the Division within seven days as required. .0802(f) The violation documented was corrected during the visit. You gave me a copy of the incident report dated February 6, 2025, today during the annual compliance visit. I asked that you submit a compliance letter stating your understating of the child care requirements regarding submission of incident reports. You should also include what will be done in the future to ensure incident reports are submitted as required. The center’s compliance history score was reviewed with the operator. The program’s compliance history was 94% as of March 17, 2025. OWNERSHIP STATUS: Gloria F. Creech is listed as the current owner of this center. Prior to making any changes in the ownership status, contact me to discuss changes of ownership procedures. Failure to comply could impact the program license status. FLOOR PLAN/SPACES: The floor plan shows two separate spaces for classroom #3 which is labeled 3a and 3b; however, the room has been changed to one space. I informed you the floor plan would be updated to reflect the accurate space instead of two separate groups. I will send the updated floor plan to you via postal mail. If you have questions once received contact me to discuss. INCIDENT REPORTS: During my review of the incident log, you informed me of an incident that occurred at the facility on February 6, 2025. You stated a child had on boots to large which caused a fall. You added a person that works with the Emergency Medical Team was onsite at the time and checked the child as a precautionary measure. You stated the parent was present during this time and was advised by you to take the child to the emergency room for further evaluation. According to you, the child’s parent called the center to report no injuries were sustained. You showed me the incident report that was completed for this incident. I informed you that the incident report should have been submitted to me within seven days from the date the incident happened, because child was taken to the hospital for evaluation. You explained you thought submission was only required if treatment was provided. I clarified that any visit to the hospital due to an incident at a child care program necessitates the submission of an incident report. I advise you to ensure all future incident reports are reported in accordance with the required timeline. TRANSPORTATION: You shared the vehicle (2003) Chevrolet Express van used for transportation was in the shop for repairs. You reported that children are now being transported using your personal vehicle (2023 Honda Odyssey) van. You stated children are transported in separate groups to each individual school to ensure safety laws are always met. I notified you the primary vehicle will be monitored for compliance with transportation requirements during the next visit. I recommended you check the vehicle for safety and transportation requirements prior to transported with children. QRIS PUBLIC COMMENT PERIOD: On March 26 at 1:00 p.m., the N.C. Child Care Commission will hold a special meeting including a Public Hearing at 1:15 p.m. to hear from the public about its proposed rules to modernize the state Quality Rating and Improvement System (QRIS). Members of the public wishing to comment on any of the proposed rules may sign up to speak upon arrival. Instructions for webinar attendees to sign up to speak will be shared once the webinar begins. On March 27 at 10 a.m., the Commission will meet to continue considering public comments. CRIMINAL BACKGROUND CHECK REQUIREMENTS: As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Children were engaged morning activities, free choice play with developmentally age-appropriate materials, interacting with the caregivers, and completing toileting, diapering and hand washing routines. Currently this program operates with a three-star license. The last sanitation inspection was completed on September 25, 2024, earning a superior classification. HAND WASHING: We discussed ensuring staff and children’s hands are washed as required after diapering. I talked to you about the diaper changing poster on the wall. I explained two steps of the diaper changing procedures were missing. It was recommended retrieving an updated hand washing sign to post in the infant classroom. SAFE ENVIRONMENT: I informed you the bolts were protruding from the flush of the base on the black border enclosing the surfacing on the playgrounds. You stated you would have the bolts hammered down. It was suggested checking the bolts during outdoor play inspections to monitor protrusion. If identified, the bolts should be reinserted. SCREEN TIME: I reminded you screen time should be prohibited for children under the age of three. Staff turned the televisions off soon after I entered the classroom. You stated staff use the television to play music for children to dance to. It was suggested covering the screen when playing the music to avoid providing screen time to children.
