Jennifer Garner, Lead Child Care Consultant, assisted during today’s visit. This facility operates with a new Notice of Compliance since July 1, 2020. The last annal compliance visit was conducted March 25, 2025. A Fire Inspection was completed on February 16,2026; the facility was approved for daytime care only. An “Approved” sanitation inspection and 27 demerits was completed on December 15, 2025. The center's compliance history was reviewed with the operator. As of today, the program’s compliance history was 83% percent. The status of the Church which owns the facility, New River Community Church of God was listed, was verified as current and active with the NC Secretary of State’s office today. You, Yesica Coceres and Jane Allen, administrators, was present today. During the visit, you accompanied me during the walk through and observations in the classroom. All indoor and outdoor spaces were monitored today. Five spaces are approved for use. Today, there were 46 children enrolled and 31 were present, ranging in age from infants to five-year-olds. Upon arrival, in space 3 children were playing in the gymnasiums participating in gross motor play. They then transitioned back into the classroom. Children were engaging in free play participating in center play including but not limited to blocks and dramatic play. Infant sleep charts were completed and in compliance. Infants were engaging in personal care routines. In space 1, 2, and 4, children were playing in free play activities including but not limited to playing with playdoh, cutting with scissors, reading books, playing in dramatic play, and playing with magnetic tiles. For lunch, goulash, salad, peaches, and milk was served. There were no new staff members employed, and a percentage of the veteran staff files were reviewed today and found in compliance. Five of the children’s files were reviewed today and found in compliance. The following violations were observed and documented during today’s visit. All violations must be corrected immediately. When diaper cream forms are filled out by parents, you should check that the correct expiration date is noted on the topical ointment permission form and that it matches the expiration date on the medication. If a parent brings in a diaper cream to replace another diaper cream, a new form must be filled out for that diaper cream. I strongly encourage you to review the child care rules regarding medication and share them with staff. It may be helpful to create a checklist for staff to assist them in making sure requirements regarding permission, storage and administration are met. I also encourage you to check the classroom medicine boxes on a regular basis as an additional monitoring measure. Regarding medication authorizations, I encouraged you today to make lists of medications brought in by parents which include children’s names, types of medications, expiration dates of medications and expiration dates of authorization forms. This will allow staff members to easily see when medications or authorization forms are about to expire so they can return them to parents or get new authorization forms within 72 hours as required. Staff Nutrition- Child Care Rule 10A NCAC 09 .0901(j) GENERAL NUTRITION REQUIREMENTS states that staff shall role model appropriate eating behaviors by consuming only food or beverages that meet the nutritional requirements specified in this Rule in the presence of children in care. Fast Food or coffee drinks that staff might consume do not follow these requirements and should not be consumed in the classroom or in the presence of children. Today we discussed ensuring that staff model appropriate eating and drinking habits while in the presence of children. Storage of Aerosols- Please review child care rules regarding storage and accessibility of potentially hazardous items. Items labeled “Keep out of reach of children” with no other warnings must be stored at a minimum of five feet off the floor. Hazardous cleaning supplies, including those kept under pressure in an aerosol can, as well as medications, must be kept in locked storage. The definition of locked storage requires that the lock be unlocked with some type of device such as a key or combination. Regular childproofing locks do not meet this requirement. Ensure that all cleaning supplies and other items labeled “Keep out of reach of children” are kept inaccessible while children are in care. Today I suggested only having sunscreen that is cream to use for children. Playground Safety- Child Care Rule 10A NCAC 09 .0604 (p) SAFETY REQUIREMENTS FOR CHILD CARE CENTERS states that once a day, prior to initial use, the indoor and outdoor premises shall be checked for debris, vandalism, and broken equipment. Continuing to ensure that debris shall be removed and disposed of. Before children enter the outdoor area each day, you must take time to observe the outdoor area and ensure there are no safety hazards or debris in the area. If there are any hazards, they must be addressed before children can enter the area. Today there are exposed bolts underneath the plastic climbing structure on the preschool playground. Today we discussed considering cutting the bolts or ensuring that the bolts are covered to ensure that the children cannot hit the bolts. Infant Feeding- Upon arrival an infant from space 3 was being fed a bottle in a bouncy chair in space 5. Infants can only be fed in the arms of the caregivers or in approved feeding seats which include highchairs and at a table. Transportation- For each child being transported, identifying information, including the child's name, photograph, emergency contact information, and a copy of the emergency medical care information form required by Rule .0802(c) of this Chapter, shall be in the vehicle. Today, the children’s identifying information did not have a photograph for the children being transported. CLEAN WATER FOR CAROLINA KIDS- Your facility’s status, according to Clean Classrooms for Carolina Kids website, shows the following: You completed the lead water test and enrollment on April 19, 2024. Water testing must be completed every three years. The lead-based paint and asbestos have been started with no results yet. Therefore, you will need to follow up to ensure you have completed all steps for these two items. To meet requirements, you must enroll your program through Clean Classrooms for Carolina Kids in ALL THREE TESTING PROGRAMS. Your facility may be exempt from testing lead-based paint and/or asbestos based on the year built, however it is still required that you enroll in the programs and provide documentation to request exemption. At the completion of the visit, this summary was printed, reviewed, and a copy was left with you. As of today, the program’s compliance history was 83% percent. The status of the Church which owns the facility, New River Community Church of God was listed, was verified as current and active with the NC Secretary of State’s office today. You, Yesica Coceres and Jane Allen, administrators, was present today. During the visit, you accompanied me during the walk through and observations in the classroom. All indoor and outdoor spaces were monitored today. Five spaces are approved for use. Today, there were 46 children enrolled and 31 were present, ranging in age from infants to five-year-olds. Upon arrival, in space 3 children were playing in the gymnasiums participating in gross motor play. They then transitioned back into the classroom. Children were engaging in free play participating in center play including but not limited to blocks and dramatic play. Infant sleep charts were completed and in compliance. Infants were engaging in personal care routines. In space 1, 2, and 4, children were playing in free play activities including but not limited to playing with playdoh, cutting with scissors, reading books, playing in dramatic play, and playing with magnetic tiles. For lunch, goulash, salad, peaches, and milk was served. There were no new staff members employed, and a percentage of the veteran staff files were reviewed today and found in compliance. Jennifer Garner, Lead Child Care Consultant, assisted during today’s visit. This facility operates with a new Notice of Compliance since July 1, 2020. The last annal compliance visit was conducted March 25, 2025. A Fire Inspection was completed on February 16,2026; the facility was approved for daytime care only. An “Approved” sanitation inspection and 27 demerits was completed on December 15, 2025. The center's compliance history was reviewed with the operator. As of today, the program’s compliance history was 83% percent. The status of the Church which owns the facility, New River Community Church of God was listed, was verified as current and active with the NC Secretary of State’s office today. You, Yesica Coceres and Jane Allen, administrators, was present today. During the visit, you accompanied me during the walk through and observations in the classroom. All indoor and outdoor spaces were monitored today. Five spaces are approved for use. Today, there were 46 children enrolled and 31 were present, ranging in age from infants to five-year-olds. Upon arrival, in space 3 children were playing in the gymnasiums participating in gross motor play. They then transitioned back into the classroom. Children were engaging in free play participating in center play including but not limited to blocks and dramatic play. Infant sleep charts were completed and in compliance. Infants were engaging in personal care routines. In space 1, 2, and 4, children were playing in free play activities including but not limited to playing with playdoh, cutting with scissors, reading books, playing in dramatic play, and playing with magnetic tiles. For lunch, goulash, salad, peaches, and milk was served. There were no new staff members employed, and a percentage of the veteran staff files were reviewed today and found in compliance. Five of the children’s files were reviewed today and found in compliance. The following violations were observed and documented during today’s visit. All violations must be corrected immediately. Water Test- It’s time to enroll your facility in the Clean Classrooms for Carolina Kids program to identify and eliminate exposure to lead and asbestos hazards in building infrastructure. This program is an expansion of Clean Water for Carolina Kids, which previously tested all child care centers in the state for lead in water at drinking and food preparation taps. As this is a legislatively mandated effort, facilities are required to meet rule requirements to have tap water tested every three years and to have inspections for lead paint and asbestos if applicable. Steps: 1. Sign Up – Pre-enrollment webinars are available at cleanwaterforUSkids.org/carolina or paste https://bit.ly/3CK-webinar into your web browser to see available dates and times for the next month. 2. Enroll – At www.cleanwaterforUSkids.org/carolina using the PIN and complete the enrollment surveys for three program sections: 1) lead in water, 2) lead-based paint, and 3) asbestos. 3. Evaluate Hazards - The program will ship your facility a water sample kit and may coordinate an on-site visit by a professional to assess for lead-based paint and asbestos hazards, if needed. 4. Receive Results – Receive your results, recommendations, and water mitigation support, if needed. 5. Request Reimbursement - If you choose to conduct lead-based paint or asbestos mitigation, you can request reimbursement from NC DHHS.
