Your program currently operates with a Provisional license issued May 1, 2025. The last annual compliance was conducted on May 16, 2025. The sanitation inspection was completed May 12, 2025, with a Provisional classification. The last fire inspection was conducted on October 15, 2025. Your facility was approved for 1st shift care, enhanced ratios, enhanced space, and reduced ratios by one in each group. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-eight (98%) percent as of February 17, 2026. The NC Secretary of State website was reviewed on February 17, 2026, and Carteret County Public School Foundation was listed as current-active. Reba Lewis, Administrator, was present and available for consultation. Sixteen (16) children, between the ages of four (4) and five (5) years old are enrolled and fourteen (14) were present today. There is one (1) approved indoor space and one (1) approved outdoor space. At least five (5) activity areas including language, blocks, manipulatives, dramatic play, and art were available in the classroom. Age-appropriate materials and activities were provided for all children. Upon entering your classroom, you and the children were seated on the carpet for morning group time. You played the ukulele and sang a good morning song to the children. After the song, you and the children sang the days of the week and months of the year songs. The children transitioned from the morning group time to free play activities which consisted of stacking small legos, writing letters, cutting paper, coloring with markers, building with cardboard brick blocks, viewing insects with magnifying glasses, pushing dump trucks, pretending with puppets, and individual electronic tablet games. Tablet games included math matching and story book readings. Screen time logs were monitored, and requirements were met. The outdoor space had a variety of gross motor equipment which included riding cars, tricycles, basketball goals, a large climber with two slides, balls, and a parachute. Other activities available included a sand box with sand toys, two playhouses, and a large gear and maze structure. Today, you reported that the sandbox cover was destroyed during a recent winter storm and a new one is expected for delivery this week. Health and safety trainings were monitored, and requirements were met. Your program currently operates with a 5-Star license issued March 6, 2019. The last annual compliance was conducted on May 20, 2024. The sanitation inspection was completed May 12, 2025. The last fire inspection was conducted on April 29, 2025. Your facility was approved for 1st shift care, the facility meets enhanced ratios and enhanced space, and enhanced ratios minus one in each age group. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-five (95%) percent as of May 16, 2025. The outdoor space had a variety of gross motor equipment which included riding cars, tricycles, basketball goals, a large climber with two slides, balls, and a parachute. Your program currently operates with a 4-Star license effective March 6, 2019. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-nine (99) percent as of October 30, 2024. The NC Secretary of State website was reviewed on October 30, 2024, and Carteret County Public School Foundation was listed as current-active. The license was posted, and the restrictions were in compliance. Administrator, Reba Lewis, was available for consultation during the visit. There is one (1) approved indoor space and one (1) approved outdoor space. Age-appropriate materials and activities were provided for all children. A walk-through of the facility was completed today. All indoor and outdoor areas were monitored. A current activity plan was observed posted in the classroom. Upon entering your facility, you and the children were in the outdoor space. The children were riding tricycles, playing with toys in the sand box, taking turns swinging at the punching ball, running, sliding, and riding cars. The children transitioned into the indoor space for lunch. Handwashing and toileting routines were observed, and requirements were met. Lunch consisted of chicken tenders, carrots, peach cobbler, and milk. Food service requirements were found to be in compliance. After lunch, the children transitioned to rest time. Each child had their individual cot with linens. Cot spacing was monitored, and requirements were met. During indoor space monitoring, three (3) aerosol cans were located over the sink not in locked storage. The outdoor spaces had a variety of gross motor equipment including tricycles, riding cars, a 2-seat see saw, 3-hoop basketball goal, a variety of balls, covered sand box with large shade umbrella structure, two (2) playhouses, sand toys, two (2) large manipulative structures with a maze and gears, and a large climber with a double slide. Health and Safety trainings and the Shaken Baby and Abusive Head Trauma Policy were monitored today and found to be in compliance. Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Items labeled “Keep out of reach of children” with no other warnings must be stored at a minimum of five feet off the floor. Hazardous cleaning supplies, including those kept under pressure in an aerosol can, as well as medications must be kept in locked storage. The definition of locked storage requires that the lock be unlocked with some type of device such as a key or combination. Regular childproofing locks do not meet this requirement. Ensure that all cleaning supplies and other items labeled “Keep out of reach of children” are kept inaccessible while children are in care. Today I suggested that all cleaning supplies remain in a locked cabinet while children are in care. The DCDEE and the North Carolina Rated License Assessment Project (NCRLAP) are collaboratively working on preparations for the transition to the third editions. Your program currently operates with a 4-Star license issued March 6, 2019. The last annual compliance was conducted on May 23, 2023. The sanitation inspection was completed March 11, 2024, with a “Superior” classification. The last fire inspection was conducted on April 25, 2024. Your facility was approved for 1st shift care, the facility meets enhanced ratios, enhanced space, and minus 1 child in each group. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-eight (98) percent. The NC Secretary of State website was reviewed on May 20, 2024, and Carteret County Public School Foundation, was listed as current-active. Fourteen (14) children, between the ages of four (4) and five (5) years old are enrolled and nine (9) were present today. There is one (1) approved indoor space and one (1) approved outdoor space. At least five (5) activity areas including language, blocks, manipulatives, dramatic play, and art were available in the classroom. Age-appropriate materials and activities were provided for all children. Upon arrival at your classroom, preschool children were engaged in a variety of free play activities which included cutting and gluing in the art center, DIY plant creations in science, magnetic shapes in manipulatives, and building with cardboard brick blocks, wooden street signs, wooden trees, people, and buses in the block area. The children transitioned to morning snacks. Food service, toileting, and handwashing routines were monitored, and requirements were met. The outdoor space had a variety of gross motor equipment including tricycles, riding cars, a 2-seat see saw, 3-hoop basketball goal, a variety of balls, covered sand box with large shade umbrella structure, two (2) playhouses, sand toys, two (2) large manipulative structures with a maze and gears, and a large climber with a double slide. Health and safety trainings were monitored, and requirements were met. Violation Number Comment Rule 1867 The depth of the loose surfacing was not based on critical height of the equipment. The depth of the mulch in the outdoor space under the large climbing structure was not nine (9) inches. .0605(k)(1-4) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The program follows the Carteret County school calendar. The length of the day, 8:00am until 3:00pm, meets the instructional day requirement of 6.5-10 hours. Child Assessment and Required Screenings: The assessment tool, GOLD, Teaching Strategies 2010-2011, is in place. Portfolios for each child are maintained electronically on Teacher Strategies Gold online application. The facility uses the developmental screening tool, TS Gold for pre-screening and ongoing assessments. Documentation of health assessments, vision, hearing, dental screenings, and DIAL-4 pre-screenings were on file for each child. Curriculum: The Creative Curriculum for Preschool, 6th Edition, Teaching Strategies, 2016 has been implemented and the classroom has the required resources available for use. Materials to support the current activity plan were displayed throughout the classroom. Family Engagement: Parent involvement includes open house, parent-teacher conferences, staggered enrollment, daily communication, and TS Gold. Staff Requirements: The administrator and the lead teacher meet NC Pre-K education requirements. The teacher assistant is currently a long term substitute.
