Upon my arrival I was greeted by assistant director, K. Molina. A checklist was used to note the requirements I monitored today. C. Welch, director, arrived a short time later. I was unaccompanied as I completed a general walkthrough of the indoor environment which consisted of thirteen (13) classrooms, kitchen and the outdoor environment. The children were observed free choice activities, participating in teacher-directed activities, outdoor play and completing routine care tasks. Today’s meal was in compliance with the Meal Patterns. ** License Status: This program currently operates on a GS 110-106 Notice of Compliance issued on July 17, 2008. ** Annual Inspections: The last sanitation inspection was completed May 19, 2026, and received a ‘Superior’ classification with 2 demerits. The last fire inspection was completed on March 26, 2026. The last documented fire drill was conducted on June 30, 2026, the last documented lockdown drill was completed on June 29, 2026, and the last documented playground inspection was completed on June 30, 2026. During today’s visit, a full assessment of Child Care Requirements was conducted. I observed all required postings, attendance logs, transportation and safe sleep checks. I reviewed a sample of children’s records, staff files and all applicable program records. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Electrical power strip was not covered with safety plugs. 10A NCAC 09 .0604(c) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. A box of borax was located on the cabinet in the classroom designated for four and five year old children. .2820(b) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. Expired diaper cream and sunscreen was not discarded after expiration date. .0803(12) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. Plastic grocery bag was accessible to children in the classroom designated for two year old children. .0604(q) 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. Photographs were not included on emergency and identifying information for the children being transported on buses. 10A NCAC 09 .1003(d) 1850 Signage was not posted regarding the smoking and tobacco restriction at the entrance of the center and/or in vehicles used to transport children. Once pre-packaged food has been opened, the food shall be stored in clean, resealable bags or containers made of plastic or glass that are tightly covered and manufactured for the purpose of food storage.15A NCAC 18A .2806(a) cheerios were not stored in an airtight container. The violations cited during today’s visit were corrected therefore a compliance letter is not needed. Prior to today’s visit, the 18-month compliance history for this facility was 97%. Technical Assistance/ Consultation: All medications, including topical ointment, require authorization to administer medication form. The forms must be completed in their entirety, including the full name of the child, dates the medication is to be administered, dosage, frequency and parent’s signature and date. Authorization forms are good for a period of twelve (12) months, so please be sure to include the expiration date. Any medication remaining after the course of treatment is completed, after authorization is withdrawn or after authorization has expired must be returned to the child’s parents. Any medication the parent fails to retrieve within 72 hours of completion of treatment, or withdrawal of authorization, shall be discarded. During today’s visit, I observed a box of borax laundry booster on top of a cabinet in the classroom designated for four- and five-year-old children. Borax should be kept inaccessible to children and locked up at all times because it is a hazardous chemical and poses a risk to children. During today’s visit, plastic grocery bags were observed in cubbies located in the space designated for two-year-old children. Plastic bags or small pieces of material that can be torn apart and swallowed must be inaccessible to children under three (3) years of age. K. Hopkins greeted me and assisted with the visit. I monitored the indoor and outdoor environments for compliance with child care requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The sanitation placard, evacuation plan, safe arrival and departure procedures, first aid chart, menus, daily schedules, staff/child ratios, emergency medical care plan, emergency telephone numbers and a summary of NC Child Care Law were all posted at the time of today's visit. The files for five new staff members and a percentage of the other staff files were reviewed and the information on the Staff and Training worksheets was verified. A sample of children's files were reviewed during the visit and the worksheet was completed. The storage of medication and hazardous products was also monitored for compliance during today's visit. The last fire drill was documented as completed on 6-5-25 at 9:47 am with an evacuation time of two minutes. The last outdoor inspection was documented as completed on 7-10-25. A lockdown drill was documented as completed on 5-8-25 at 10:00 am. An approved fire inspection was completed last on 3-12-25. A sanitation inspection was last completed on 12-6-25 with two demerits and a superior classification. During today's visit, the children were observed napping and playing in the classrooms and eating a snack. For snack the children had pizza rolls and white grape juice. The corporation status of Wilson’s Mills Baptist Church Child Care was verified as current and active during the visit using the Secretary of State's website. You are currently licensed under a GS 110-106 Notice of Compliance. At the time of today's visit, you continued to meet the requirements previously met to obtain the Notice of Compliance. Please be reminded that you may apply for a star rated license at any time by submitting an Application for Assessment of a Two Component Rated License to me. Violation Number Comment Rule 601 Refrigerator(s) did not maintain a temperature of 45 degrees F. or below. A thermometer was not present in the refrigerator in the classroom for the infant children. 15A NCAC 18A .2806(j)(2) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. A container of Vaseline was not in locked storage and a tube of sunscreen was on a desk in a classroom for the school age children. 15A NCAC 18A .2820(d) Today, I cautioned you about having the rooms so dark at nap time. I reminded you today that you need to be able to see the children in order to adequately supervise them and to respond quickly in the even of an emergency. At the time of today's visit, our area was under a heat advisory with temperatures of near 100 degrees expected and a heat index of 104 to 109. We discussed several concerns presented by these high temperatures and ways to address them: - Outdoor time must be limited to no more than fifteen minutes and must occur in the early hours of the day. Please refer to your weather watch chart for further instructions for your youngest children in care, as well as those with special health concerns that may be made worse when exposed to high temperatures. - While outside, please pay special attention to all children and staff members to watch for signs of heat exhaustion including heavy sweating, pale or clammy skin, headaches, dizziness, weakness, vomiting and/or a fast, weak pulse. Heat strokes are considered a medical emergency and require immediate medical attention. Symptoms of heat stroke include a dangerously high body temperature (104 or higher), hot and dry skin, confusion, rapid pulse and potential loss of consciousness. If you suspect a child or staff member is experiencing either of these conditions, please seek medical attention. - Drinking water must be freely available to all children and should be provided during outdoor play, as well as immediately upon re-entry into the facility following outdoor play. - Your building must remain at 85 degrees or less based on Environmental Health requirements. Please refer to your Safe Sleep Policy regarding the temperature requirement for classrooms where infants are present. If you are not able to maintain these temperatures, you would need to move the children to spaces where the temperature can be maintained or consider closing until the air conditioning can be repaired. - Before transporting children, please ensure that the air conditioning on each vehicle is working and that the vehicles can be adequately cooled. I recommend that you limit off-premises activities that include outdoor activities during this extreme weather. ****Child care licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. I monitored the indoor environment for compliance with child care requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The sanitation placard, evacuation plan, safe arrival and departure procedures, first aid chart, menus, staff/child ratios, emergency medical care plan, emergency telephone numbers and a summary of NC Child Care Law were all posted at the time of the visit. The storage of medication and hazardous products was also monitored during the visit. During today's visit, the children were observed in teacher directed group activities, playing inside and playing outside, and going on a buggy ride and eating lunch. Lunch consisted of chili with beans and beef, ritz crackers, summer squash, pineapple and milk. The children were observed during center activities, teacher directed activities, and completing routine care tasks. Today’s meal was observed and in compliance with the Meal Patterns. ** License Status: This program currently operates on a GS 110-106 Notice of Compliance. ** Annual Inspections: The last sanitation inspection was completed on June 14, 2023, and received a ‘Superior’ classification with 6 demerits. The last fire inspection was completed on March 29 2023. The last documented fire drill was conducted on August 22, 2023, the last documented shelter-in-place drill was completed on August 27, 2023, and the last documented playground inspection was completed on August 18, 2023. During today’s visit, a full assessment of Child Care Requirements was conducted. I observed all required postings, attendance logs, safe sleep logs, transportation, and I reviewed a sample of children’s records, staff files and all applicable program records. The following violation was observed, and corrected during the visit. Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet.
