Upon my arrival, I was greeted by assistant director K. Baker. K. Hollifield director arrived a short time later. A checklist was used to note the requirements I monitored today. I was accompanied by Ms. Baker as I completed a general walk-through of the indoor and outdoor areas, which consisted of five (5) classrooms, the kitchen, and the outdoor play environment. The children were observed during free choice activities and completing routine care tasks. Today’s meal was observed and in compliance with the Meal Patterns. **Current License Status Currently this center operates with a three-star rated license issued March 4, 2024. ** Annual Inspections: The last sanitation inspection was completed May 4, 2026, and received a ‘Superior’ classification with 4 demerits. The last fire inspection was completed on October 24, 2025. The last documented fire drill was conducted on June 9, 2026, the last documented lockdown drill was completed on March 10, 2026, and the last documented playground inspection was completed on May 11, 2026. Prior to today’s visit I reviewed the NC Secretary of State’s website and observed that the owner of this facility is listed as current/active. During today’s visit, a full assessment of Child Care Requirements was conducted. I observed all required postings, attendance, and transportation. I reviewed a sample of children’s records, staff files and all applicable program records. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. A current activity plan was not posted in the classroom designated for three year old children. GS 110-91(12); .0508(a) 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. Health questionnaire were not completed for all staff. .0701(a) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. Emergency information was not updated annually for all staff. .0701(a) 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. Photograph of one child was missing from emergency and identifying information for a child being transported. 10A NCAC 09 .1003(d) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last lockdown drill was completed on 3/10/26 .0604(u);.0302(d)(8) To comply with the NC Laws and Rules, any violations cited today must be corrected immediately. A compliance letter must be sent to me by July 21, 2026. The letter must address each violation, explain how it has been corrected and how it will be maintained in the future. If the compliance letter is not received by the date listed a return visit will be made and the violations cited again. Technical assistance/ Consultation: The last emergency drill was completed on March 10, 2026. As a reminder, set notifications to remind yourself when emergency drills need to be complete. Regular practice of emergency drills reduces panic, uncovers safety plan gaps, builds muscle memory and ensures a coordinated response during actual emergencies. During today’s visit, I observed on the playground designated for four- and five-year-old children many tree roots that can cause a tripping hazard. Ms. Hollifield stated that she has a maintenance guy currently working on cutting the tree roots from the playground. She is moving rubber mats to put on top of the roots to ensure that children do not trip over them until all the roots are cut. K Hollyfield was present and assisted with the visit. I monitored the indoor and outdoor environments for compliance with child care requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The permit, sanitation, evacuation, safe arrival and departure procedures, first aid chart, menus, daily schedules, activity plans, summary of NC Child Care Law and staff/child ratios were posted at the time of the visit. The storage of medication and hazardous products was also monitored for compliance. One new staff file was reviewed today. A sample of children's records were reviewed, and the worksheets were completed. The last outdoor inspection was documented as conducted on 6-20-25. A fire drill was documented as conducted on 6-23-25 at 9:30 am with an evacuation time of sixty one seconds. A lock down drill was documented as conducted on 6-23-25. An approved fire inspection was completed on 10-15-24. A sanitation inspection was last completed on 4-14-25 with zero demerits and a superior classification. The children were observed during naptime and playing outside. I did not observe a meal or snack being served today. According to the menu, lunch consist of chicken and rice, mixed vegetable, applesauce and milk. A four star child care center with seven points in education, two points in program and one quality point was issued to you on 3-2-24. Based on the information provided and on observations made during the visit, you continue to meet the requirements previously met to obtain a license. GS 110-91(12); .0508(a) 716 All stationary equipment, more than 18 inches high, was not installed over protective surfacing. The surfacing under the play structure on the playground for the PreK-school age children was not adequate. .0605(j) Child care licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the childcare law and rules. I explained that you have access to the on-line version of the Child Care Rule Book, as well as a hard copy of the rules. I recommended that you visit the Division of Child Development's website (www.ncchildcare.nc.gov) on a regular basis to find out what's new in childcare. You could also find any needed Child Care forms by going on the web site and clicking on the "Provider" tab and then clicking on the "Provider Documents" tab. I monitored the indoor and outdoor environments for compliance with child care requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The permit, sanitation, evacuation, safe arrival and departure procedures, first aid chart, menus, daily schedules, activity plans, summary of NC Child Care Law and staff/child ratios were posted at the time of the visit. The storage of medication and hazardous products was also monitored for compliance. Files for new staff and files for a percentage of existing staff were reviewed and the information on the Staff and Training Worksheet was verified. The corporation status of Playmates Child Care and Development Center was verified as current and active during the visit using the Secretary of State's website. Based on the information provided and on observations made during the visit, you continue to meet the requirements previously met to obtain a license.
