Karen Wall, Director, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Wall and additional staff members. Limited monitoring of child care requirements occurred during today’s visit. A violation was confirmed during today’s visit. Violation Number Comment Rule 1757 A valid qualification letter was not on file and available to review at the facility. A staff member did not have a qualification letter on file.
