I monitored the indoor environment for compliance with child care requirements. Smoke detectors, fire extinguishers, first aid kit, refrigerator thermometer, storage of medication and hazardous products, outlets, garbage storage, toys and materials and the bathroom the children use were all monitored for compliance. The permit, schedule, activity plan, SIDS information, emergency telephone numbers and a summary of NC Child Care Law were all posted at the time of today's visit. Qualifying letters, CPR/ first aid, ITS-SIDS, health questionnaire, emergency procedures and the children's records were also monitored for compliance. The last fire drill was documented as completed on 5-11-26 at 12:00 pm with an evacuation time of twenty nine seconds. A lock down drill was documented as conducted on 5-5-26. The children were observed during naptime. A meal or snack was not observed during the visit. At the time of the 3-5-26 visit, you stated that you would like to go ahead and apply for a new rated license and that your will be applying for the Classroom and Instructional Quality Pathway. During that visit, we reviewed the Application for a Rated License, the Family and Community Engagement forms and the Continuous Quality Improvement Forms. You stated that you have not yet completed all of the paperwork, but would do so before the end of July and submit to me. As long as your paperwork is received before 9-5-26, an additional visit will not be needed. Once I receive the paperwork, your new license will be mailed to you. Violation Number Comment Rule 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. A completed health assessment was not on file for one child with an enrollment date of 4-13-26. GS 110-91(1); 10A NCAC 09.1721(a)(1) During today's visit, you stated that you will be taking an extended leave and will be closed from 6-29-26 to 7-31-26. As we discussed, DCDEE no longer places family child care home licenses on inactive status when a provider takes an extended leave. You will not be due to another monitoring visit during this timeframe. However, if a complaint report is received by our office, we are required by law to follow up on the report. If this were to happen while you are closed, I will contact you to either conduct the visit by phone or to schedule a time to conduct the visit in your home. Child care licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the childcare law and rules. I explained that you have access to the on-line version of the Child Care Rule Book, as well as a hard copy of the rules. A rated license assessment was completed during the visit to transition your license to a three star under the new rated license requirements. I monitored the indoor and outdoor environments for compliance with child care requirements. Smoke detectors, fire extinguishers, first aid kit, refrigerator thermometer, storage of medication and hazardous products, outlets, garbage storage, toys and materials and the bathroom the children use were all monitored for compliance. The permit, SIDS information, emergency telephone numbers and a summary of NC Child Care Law were all posted at the time of today's visit. Qualifying letters, CPR/ first aid, ITS-SIDS, health questionnaire, emergency procedures and the children's records were also monitored for compliance. The last fire drill was documented as completed on 2-4-26 at 9:00 am with an evacuation time of twenty nine seconds. The last outdoor inspection was documented as completed on 2-4-26. A lockdown drill was documented as conducted on 1-21-26 at 9:45 am. Four preschool children were present and were observed during free play and naptime. Lunch on the day of the visit consisted of chicken tenders, garden peas, macaroni and cheese, raisins and milk. You attended the Family Child Care Home Provider meeting held in September, 2025 to review the changes to the rated license requirements now found in section .3200 of the child care rules. During that meeting, we reviewed the various options available to family child care home providers. At the time of today's visit, you stated that you would like to go ahead and apply for a new rated license and that your will be applying for the Classroom and Instructional Quality Pathway, also known as pathway 2. We reviewed the Application for a Rated License, the Family and Community Engagement forms and the Continuous Quality Improvement Forms. Each of these forms was left with you today. Upon receipt of your completed forms, your new rated license will be printed and mailed to you. Violation Number Comment Rule 930 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years from the date of issuance). Prior to the 9-28-25 expiration date, two household members did not obtain new qualification letters. G.S. 110-90.2 & .2703(n) & (o) A violation was documented as two of your household members, Weldon and Doris Cole, did not obtain a new qualification letter prior to the 9-28-25 expiration date of their previous letters. You stated that both of them were fingerprinted on 2-26-26; however, you have not been able to complete the application process. I provided you with a contact person in the Criminal Background Check Unit and you stated that you would contact her following the visit. As we discussed, this violation must be corrected on or before 3-19-26. If the violation is not verified as corrected by this date, your license will be placed on inactive status until the qualification letters are issued for both household members. You will not be able to provide care in your home if your license is placed on inactive status until the violation is corrected and your license is returned to active status.
