The last annual compliance visit was conducted on September 3, 2025. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-five percent as of May 5, 2026. You visited each indoor and outdoor space with me. During today’s visit, I observed children in the indoor learning environments. Children were participating in general personal care routines and free play activities. The operator was engaging with children during play. I found supervision, capacities, and interactions to be in compliance. The following were also inspected during today’s visit: program requirements, equipment, furnishings, capacity, supervision, medication, medication administration, outdoor learning environments, and program records including staff records and children records. The following was cited during today’s inspection. Violation Number Comment Rule 905 Record of monthly fire drills, giving the date, time of day and length of time to evacuate, and operator's signature, were not available. Fire drills were not documented for December and February. .1721 (e )(2) 908 Health questionnaire was not completed annually. BW and SW had expired health questionnaires. This was corrected during the visit by having then sign the health questionnaire. .1703(a)(1) 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. You must register by May 1, 2024, to be a part of the program. We discussed requesting Environment Rating Scale (ERS) assessments in an effort to increase the program's star rating. Resources such as Child Care Resource and Referral and Smart Start can be valuable when preparing for the ERS. “All about FCCERS-R” and the NCRLAP website (www.ncrlap.org) can also be helpful as staff prepare for the ERS. On the NCRLAP website, there are training videos, sample questions and clarifying information regarding the assessments you may find helpful. You are in Cohort 1. Your preparation year is July 1, 2023 – June 30, 2024. Your assessment year is July 1, 2024 – June 30, 2025. If you wish to complete the assessment in your preparation year, please submit the assessment application to me. The application can be found on the division website under provider documents. We also discussed changing the batteries in your smoke detectors. During the visit one smoke detector was beeping. Your program currently operates with a three-star license, issued March 14, 2018, earning 1 point in the education component, 6 points in the program standards component meeting minimum requirements, having written operational policies, no more than four children less than one year of age, and 1 quality point for restricting infant capacity to no more than two children under one year of age. The last annual compliance visit was conducted September 20, 2023. The program’s compliance history was ninety-six percent as of September 13, 2023. Children enrolled in were participating in free choice activities. The operator was talking to the children and ensuring the safety of the children in care. All program records were reviewed today including emergency drills, staff records, children records, all required postings, license and permit restrictions. There were violations sited today regarding staff records. The following violation(s) were cited today. Violation Number Comment Rule 908 Health questionnaire was not completed annually. A health questionnaire was completed during today's visit. .1703(a)(1) 923 Operator did not keep records of monthly fire drills, quarterly lockdown or shelter-in-place drills, incident reports, incident logs, accurate attendance, emergency numbers, emergency preparedness and response plan, field trip/transportation permission, pet vaccinations records and/or playground inspections monthly outdoor safety checks for a minimum of one year. The last fire drill was documented July 1, 2023 .1721 (e)(7) 2012 Individual who provides care for 5 hours or more a week, did not complete Recognizing and Responding to Suspicions of Child Maltreatment training. SW did not complete their Recognizing and Responding to Suspicions of Child Maltreatment training. .1729(a)(9) C4 2022 Operator and/or staff members did not complete the required health and safety trainings within the specified time frame. SW did not complete their Health and Safety Trainings. .1703(b) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before September 27, 2023, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected.
