Your program currently operates with a five-star license, issued October 15, 2024, earning 7 points in the education component, 6 points in the program standards component meeting all enhanced requirements and 1 quality point by offering a staff benefits package and an infrastructure of parent involvement. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-five percent as of June 12, 2025. The NC Secretary of State website was reviewed on June 12, 2025, and Sonflower Seeds-Preschool & Learning Center, LLC was listed as current- active. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, transitions, and personal care routines. The caregivers were interacting and meeting the developmental needs for each of the children. Files for new staff were reviewed. Rated license requirements were reviewed to ensure compliance was maintained with current program and education components as well as the quality point. The most recent fire inspection was June 26, 2024, and the most recent sanitation inspection was March 4, 2025. During today’s visit, I remeasured spaces 2 and 3 to make a combined classroom after you made the decision to remove the wall that was separating the two classrooms. The measurements are now as follows. Classroom 2: Total Square footage: 348.65 sq ft. Capacity at 25 sq ft per child: 14 children Capacity at 30 sq ft per child: 12 children You have chosen to meet all enhanced requirements. This means that you would use enhanced space standards at 30 sq ft per child. The maximum number of children that can be in the room at one time is 12 children. The following was cited during today’s visit. Violation Number Comment Rule 718 Swings did not have surfacing that extends two times the length of the pivot point to the surface below beyond the external limits of the equipment. The swing set on the school age playground did not have surfacing that extends two times the length of the pivot point to the surface below beyond the external limits of the equipment. .0605(m)(2) 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The ABCMS Provider Portal is not current with all teachers. G.S. 110-90.2 & .2703(r) 1866 Other surfacing materials not identified in rule were used that did not meet the ASTM standards, were not installed, maintained, and/or replaced according to manufacturer's instructions. The mats under the swings have grown grass trough them and are no longer meet the standard. Verification of the mats meeting the ASTM standards was not observed. Please keep on record verification that the mats meet the ASTM standard and maintain them to prevent other surfacing from taking over. .0605(j)(2) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Your program currently operates with a five-star license, issued July 9, 2018, earning 7 points in the education component, 7 points in the program standards component meeting all enhanced requirements and 1 quality point for having enhanced policies and infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted October 24, 2023. The sanitation inspection was completed April 15, 2024, with a “Superior” classification. The last fire inspection was conducted June 26, 2024, and your facility was approved for daytime care only. During today’s visit I monitored all indoor and outdoor learning environments, staff records, child records, emergency drills, transportation, medication administration, staff/child ratios, interactions, supervision, and all child care requirements. Teachers were seen interacting with children during a bible story. Children were observed sitting on a mat listening to the story. Today’s lunch consisted of cheese tortillas, corn, beans, and milk. ERS summaries were reviewed today and items scoring less than a three could constitute a violation of child care requirements and therefore were monitored. These items are noted below: ECERS-R assessment – May 21, 2024, Total score – 5.57 pts Item 7 Indicator: 3.2 (see pages: 18-19 in the ECERS-R) Score: 2 pts Assessor Comments: This group used the outdoor area with a swing, rock climbing wall, and wooden step logs. One of the wooden step logs measured 4 feet tall and was placed over grass, rather than an approved cushioning surface. Refer to the document, NCRLAP's Requirements for Gross Motor Space and Equipment, for more information. Observations today: The log is not a piece of stationary equipment and does not require surfacing. Item 12 Indicator: 3.1 (see pages: 28-29 in the ECERS-R) Score: 2 pts Assessor Comments: The children and teachers used the sink in the dining area to wash their hands after some toileting routines, outdoor play, and after breakfast and before and after lunch. The sink was not sprayed with a disinfectant after toileting routines before it was used for other purposes. Observations today: We discussed the rule and I observed teacher sanitizing and disinfecting the tables and sinks in the activity space. Item 13 Indicator 3.1 (see pages: 30-31 in the ECERS-R) Score: 2 pts Assessor Comments: This indicator requires that teachers, staff members, and children wash their hands at least 75% of the time when needed upon arrival/reentry, before and after water play and after messy play, after contact with bodily fluids, and after touching contaminated objects. The children washed their hands as needed. The teachers and staff members washed their hands 50% of the time. The staff members did not wash their hands upon arrival or reentry to the classroom and a teacher did not wash her hands after outdoor play. Observations today: Today all children and teachers washed their hands throughout the day. We discussed reminding teachers that all teachers and children must wash hand upon re-entry to the program space. Item 14 Indicator 3.1 (see pages: 32-33 in the ECERS-R) Score: 2 pts Assessor Comments: Outdoors, the safety concern mentioned in item 7 applies to this indicator as well. Observations today: The equipment is not stationary and does not require surfacing. SACERS-U assessment – May 23, 2024, Total score – 5.42 pts Item 17 Indicator: 3.2 (see page: 24 in the SACERS-U) Score: 2 pts Assessor Comments: For snack, most of the children were served banana muffins with milk. However, four children were served water with their snack instead of milk. Since each child did not have all of the required meal components, credit was not earned. Please see the USDA Meal Guidelines for more information. Observations today: Children are offered milk and then receive water if they do not drink the milk. Item 19 Indicator: 3.1 (see page: 26 in the SACERS-U) Score: 2 pts Assessor Comments: Categories of art materials include drawing, paints, collage, sculpture, and crafts. To earn credit, at least one example from two different categories should be provided for children’s daily use. There were drawing materials outdoors such as pens, colored pencils, markers, chalk, and paper present. Although wood glue was also present, there was nothing to use with the glue to make this material functional. Materials representing other categories were not present outdoors. Please note, additional art materials were present indoors; however, on the day of the observation, the children played outdoors for most of the observation. The teacher reported that children will play indoors on inclement weather days or during Summer Camp. Therefore, the additional art materials were not considered for this indicator. Observations today: The wood glue was stored in a cabinet and the other supplies that goes with the wood glue is in a separate container located on the shelf of the cabinet. Item 23 Indicator 3.1 (see page: 30 in the SACERS-U) Score: 2 pts Assessor Comments: There were no books or language games present outdoors. Although enough books and games were present indoors; as previously described in item 19, children could use these materials on inclement weather days or during Summer camp, instead of daily. Observations today: Books are not stored outside but children are given the opportunity to bring a book outside with them and read in the hammocks. Item 25 Indicator 3.1 (see page: 32 in the SACERS-U) Score: 2 pts Assessor Comments: Science and nature materials such as different types of plants, seashells, as well as gardening supplies such as gloves, shovels were present outdoors. However, there was no science equipment present to meet the requirement of this indicator. Additional science materials were present indoors; however, as previously mentioned, children will use these materials on inclement weather days or during Summer camp, rather than daily. Observations today: The center uses their nature-based learning environment as their science center. The children actively maintain center gardens, talk about the process of gardening and the science behind the environment around them. The following was cited during today’s inspection. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. A bolt was sticking up about 2 inches out of the base of the shelter in the outdoor learning environment. This is a tripping hazard to children. The soil around the bolt has washed away in the recent rainfall making it now a tripping hazard. 10A NCAC 09 .0601(a) 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. Child 1 did not have a health assessment in their file for review. Technical Assistance: We discussed the tripping hazard in the outdoor learning environment. We discussed tightening the bolt to eliminate the tripping hazard. We also discussed ensuring that all child medical report is received prior to starting or within 30 days of the child staring. The children washed their hands as needed. The teachers and staff members washed their hands 50% of the time. The staff members did not wash their hands upon arrival or reentry to the classroom and a teacher did not wash her hands after outdoor play. Observations today: Today all children and teachers washed their hands throughout the day. We discussed reminding teachers that all teachers and children must wash hand upon re-entry to the program space. However, four children were served water with their snack instead of milk. Since each child did not have all of the required meal components, credit was not earned. Please see the USDA Meal Guidelines for more information. Observations today: Children are offered milk and then receive water if they do not drink the milk. Your program currently operates with a five-star license, issued July 9, 2018, earning 7 points in the education component, 7 points in the program standards component meeting all enhanced requirements and 1 quality point for having enhanced policies and infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted October 24, 2023. The sanitation inspection was completed April 15, 2024, with a “Superior” classification. The last fire inspection was conducted June 26, 2024, and your facility was approved for daytime care only. During today’s visit I monitored all indoor and outdoor learning environments, staff records, child records, emergency drills, transportation, medication administration, staff/child ratios, interactions, supervision, and all child care requirements. Teachers were seen interacting with children during a bible story. Children were observed sitting on a mat listening to the story. Today’s lunch consisted of cheese tortillas, corn, beans, and milk. ERS summaries were reviewed today and items scoring less than a three could constitute a violation of child care requirements and therefore were monitored. These items are noted below: ECERS-R assessment – May 21, 2024, Total score – 5.57 pts Item 7 Indicator: 3.2 (see pages: 18-19 in the ECERS-R) Score: 2 pts Assessor Comments: This group used the outdoor area with a swing, rock climbing wall, and wooden step logs. One of the wooden step logs measured 4 feet tall and was placed over grass, rather than an approved cushioning surface. Refer to the document, NCRLAP's Requirements for Gross Motor Space and Equipment, for more information. Observations today: The log is not a piece of stationary equipment and does not require surfacing.
