Your program currently operates with a five-star license, issued November 21, 2024, earning 7 points in the education component, 6 points in the program standards component meeting all enhanced requirements and 1 quality point by the child care administrator having at least 10 years of documented child care administration work experience in a licensed program. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-seven percent as of June 12, 2025. The NC Secretary of State website was reviewed on June 12, 2025, and Sonflower Seeds-Preschool & Learning Center, LLC was listed as current- active. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, transitions, and personal care routines. Infants were engaged in tummy time, napping, and diapering routines. Sleep checks were monitored during today’s inspection. The caregivers were interacting and meeting the developmental needs for each of the children. Rated license requirements were reviewed to ensure compliance was maintained with current program and education components as well as the quality point. The most recent fire inspection was June 26, 2024, and the most recent sanitation inspection was March 4, 2025. The following was cited during today’s visit. Violation Number Comment Rule 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The center's roster is in the ABCMS Provider Portal is not current with all teachers. G.S. 110-90.2 & .2703(r) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before June 26, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. Name of Operation: LITTLE SPROUTS CHILD DEVELOPMENT CENTER Facility ID: 19000361 Consultant: BRANDON FRY Operation Type: Center Case Number: 0624-038L Visit Date: 6/12/2024 Number Present: 16 Completed Date: 6/12/2024 Age: From 0 To 4 Total Minutes: 95 Time In: 09:25 AM Time Out: 11:00 AM Time In: Time Out: List to Use: Center Type Of Visit: Complaint Visit Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to obtain information regarding alleged violations of child care requirements. The allegations are as follows: There are concerns of inadequate supervision. Based on information provided, on June 5, 2024, a child was left unattended for approximately five to ten minutes. Staff/child ratio, group size, supervision, use of licensed space, space capacity, license restrictions, and the outdoor environment were monitored. The license and emergency care plan were posted. I observed the teachers interacting with children as the children participated in free play activities. Staff files for new staff were reviewed. Upon arrival I spoke with Jennifer Walters, Assistant Director, and informed her the nature of today’s visit. I asked Ms. Walters to describe the incident to me. She informed me that the children were going outside in the afternoon and one of the children were left inside for about five minutes. She stated that they discovered that the child was inside after the parent arrived at the center to pick up their child. They came back into the room and found the child standing in the room. She stated that since then the teacher of the classroom has not worked alone, and the center has implemented a name to face check for every transition outside and inside. They have a sheet in the classroom with the children’s name and they visually check each time they go outside and inside to ensure that children are accounted for. I then spoke with the teacher of the classroom where this incident happened. They informed me that they had the children lined up to go outside. The child was behind them and didn’t realize that the child was there. They believed that the child had already exited the side exit door to the playground. She closed the door behind her, leaving the child in the classroom. They stated that roughly three to five minutes later the child’s parent arrived and that is when she noticed that the child was not outside. They confirmed that they are now using a name to face check process during transitions to help prevent this incident from happening in the future. Based on information received from interviewed staff and my observations, the finding regarding the allegation of inadequate supervision was substantiated. The following violations were observed during today’s visit. Violation Number Comment Rule 303 Children were not adequately supervised at all times. A child was left inside unattended for approximately five minutes while transitioning the group of children outside. .1801(a)(1-5) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. Teacher SK did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. They were hired September 18, 2023, and the training was completed April 23, 2024. This was corrected during the visit as the training was completed but was not done in the required timeline. .1102(g) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. You may also want to have staff review and acknowledge the centers expectations for supervision and provide additional coaching around transition times. Some ideas may be for the teacher to give a high five to every child while they exit the door. When the teacher gives them a high-five, they say their name so that they know each child as now exited the classroom and is now outside. A violation was documented for failure to maintain compliance with supervision requirements. Violations of this nature directly impact the safety of the children while in your care. In addition, these types of violations have the greatest negative impact on your compliance history. During the visit, we discussed that repeated violations of this nature during consecutive visits may lead to an administrative action against your license. A follow up visit may be conducted in the near future to monitor compliance with child care requirements.
