Your program currently operates with a five-star license, issued October 6, 2025, using the program assessment pathway. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted on June 24, 2025. The sanitation inspection was completed March 19, 2026, with a “Superior” classification. The last fire inspection was conducted February 19, 2026, and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-four percent as of June 8, 2026. The NC Secretary of State website was reviewed on June 8, 2026, and Little Blessings Christian Child Care Center LLC was listed as current- active. You visited each indoor and outdoor space with me. During today’s visit, I observed children in the indoor learning environments. Children were participating in general personal care routines and free play activities. Teachers were seen walking around the room engaging with children during play. I found supervision, ratios, and interactions to be in compliance. The following were also inspected during today’s visit: program requirements, equipment, furnishings, staff/child ratios, supervision, medication, medication administration, outdoor learning environments, and program records including staff records and children records. The following was cited during today’s inspection. Violation Number Comment Rule 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. Shelter-in-place or lockdown drills were documented every three months. The last documented drill was February 11, 2026. A shelter-in-place drill was conducted in May but was not recorded. This was corrected during the visit by updating the log with the May drill date. .0604(u);.0302(d)(8) Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Technical Assistance: During today’s visit, I inspected your Lincoln Navigator, license plate HKT-5506, for transportation. The last state inspection was completed July of 2025, and the insurance is current and expired December 4, 2026. You have a first aid kit and fire extinguisher in the car. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor learning environments and found supervision and interactions to be in compliance. Staff/child ratios were inspected. Children enrolled were singing songs, participating in crafts, transitioning from one activity to another as well as personal hygiene activities. All teachers were engaged and speaking with the children on the child’s level. I also observed programs records and observed two violations regarding staff records. Three staff did not renew their health and safety training within the five-year requirement and one newly hired staff member did not have a medical report that was completed within the last 12 months. Lunch consisted of bologna and cheese sandwiches, corn, pineapple and milk. The following violations were cited during today’s inspection. The NC Secretary of State website was reviewed on July 14, 2023, and Little Blessings Christian Child Care Center LLC was listed as current- active. You visited each indoor and outdoor space with me. Children enrolled were singing songs, participating in crafts, transitioning from one activity to another as well as personal hygiene activities. All teachers were engaged and speaking with the children on the child’s level. I also observed programs records and observed two violations regarding staff records. Three staff did not renew their health and safety training within the five-year requirement and one newly hired staff member did not have a medical report that was completed within the last 12 months. Lunch consisted of bologna and cheese sandwiches, corn, pineapple and milk. The following violations were cited during today’s inspection. Violation Number Comment Rule 532 All children were not held or placed in feeding chairs or other appropriate apparatus to be fed. A bottle fed ten-month old was not held or placed in a feeding chair while being fed. The ten-month old infant was observed feeding lying on the floor. This violation was corrected during the visit when a staff member immediately picked up the child. 10A NCAC 09 .0902(b) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. Staff member EL was hired July, 17, 2023. The medical report on file was dated March 4, 2020. The medical report must be within the last 12 months. 10A NCAC 09 .0701(a) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. Staff CB, HB, and JB did not renew their health and safety training within the last five years. The last training was completed January 3, 2018. .1103(b) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before August 2, 2023, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. Your program currently operates with a five-star license, issued October 15, 2019, earning 5 points in the education component, 7 points in the program standards component meeting enhanced requirements and reduced ratios and 1 quality point for offering a staff benefits package and an infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted August 9, 2022. The sanitation inspection was completed April 18, 2023, with a “Superior” classification.
