Your program currently operates with a three-star license, issued April 17, 2023, earning 1 point in the education component, 6 points in the program standards component and 1 quality point for limiting infant capacity to no more than two children under age one. The last annual compliance visit was conducted on December 17, 2024. The center's compliance history was reviewed with the operator. The program’s compliance history was one hundred percent as of November 18, 2025. The NC Secretary of State website was reviewed on November 18, 2025, and Deborah's Day Care, Inc was listed as current- active. During today’s visit, I observed children in the indoor learning environments. Children were participating in general personal care routines and free play activities. The operator was seen engaging with children during play. I found supervision, capacity, and interactions to be in compliance. The following were also inspected during today’s visit: program requirements, equipment, furnishings, staff/child ratios, supervision, medication, medication administration, outdoor learning environments, and program records including staff records and children records. One violation was cited during today’s inspection. Violation Number Comment Rule 1103 For each child being transported, the operator or other transportation provider did not have identifying information, including the child's name, photograph, emergency contact information, and a copy of the emergency medical care information form required by Rule .1721(a )(3) in the vehicle whenever children were transported. The provider had the required paperwork but it was not located in the car like they thought it was. The documentation was located inside the home with the other records. This was corrected during the visit by putting the required documents in the care. .1723(13) Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. Technical Assistance: We discussed the importance of having emergency and identifying information of children while providing transportation. The provider had all the information on hand and thought it was in the car but it was inside the home with the other records. This violation was corrected during the visit by putting the information in the vehicle. We also discussed the new pathway to the stars. We discussed scheduling a technical assistance visit to discuss the new Pathway to the Stars. At the completion of the visit, this visit summary was printed, reviewed, and a copy was left with you.
