    Name of Operation: CHATHAM COUNTY HEAD START Facility ID: 19000270 Consultant: BRANDON FRY Operation Type: Center Case Number: 0125-229L Visit Date: 1/30/2025 Number Present: 105 Completed Date: 1/30/2025 Age: From 2 To 5 Total Minutes: 130 Time In: 09:10 AM Time Out: 11:20 AM Time In: Time Out: List to Use: Center Type Of Visit: Complaint Visit Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to obtain information regarding alleged violations of child care requirements. The allegations are as follows: There is a concern of inadequate supervision as it relates to transportation. Staff/child ratio, group size, supervision, use of licensed space, space capacity, and license restrictions were monitored. The license and emergency care plan were posted. I inspected each classroom. I observed children participating in daily routines such as free play, circle time, personal care, etc. Teachers were seen walking around the room and interacting with the children. I found supervision, interactions, and ratios to be in compliance. Based on information provided, on January 16, 2025, a four-year-old child was left unattended on the bus upon dropping off at the school in the morning. During the inspection I spoke with the education specialist and the Bus Driver. They reported that after all of the children stepped off of the bus and after completing their head counts, one child stepped back on the bus to retrieve a folder. The Bus driver said that this happened after she had already checked the seats for any remaining children. They started they were focused looking at the alarm signal lights on the ceiling of the bus walking back up the aisle after doing their check and did not see that they child came back on the bus. The group of children then proceeded to walk up the hill to the school. The bus driver stayed back to lock up the bus and then they proceeded to walk up the hill to the bus. Upon walking up the hill, they were informed that a child was still on the bus and they went back and helped the child out of the bus. The director reviewed video footage and was able to determine that the child was left unattended for approximately 47 seconds. Based on information received from staff interviewed and my observations, the finding regarding the allegation of inadequate supervision was substantiated. The following violation was cited during today’s inspection. Violation Number Comment Rule 1118 Children were left in a vehicle unattended by an adult. A child was left unattended on the bus for approximately 47 seconds. GS 110-91 (13); .1003(g) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation documented may impact the compliance history score. The violation documented must be corrected immediately. On or before February 13, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware, any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. You have now moved drop-off to the main entrance of the school within feet of the front door. When the bus arrives at the school, the office is notified and additional staff come out to assist with unloading children. The bus is then checked by one of the bus monitors to ensure all children are off the bus. The bus driver then does an additional check to double ensure all children are off the bus. All bus monitors and the driver have been trained in the new procedure. I recommend adding a face to name check when unloading and loading the bus. This will also help ensure that all children are off the bus and accounted for. A violation was documented for inadequate supervision during transportation. Violations of this nature directly impact the safety of the children while in your care. In addition, these types of violations have the greatest negative impact on your compliance history. During the visit, we discussed that violations of this nature during consecutive visits may lead to an administrative action against your license. A follow-up visit may be conducted in the near future to monitor compliance with child care requirements. Your program currently operates with a five-star license, issued December 18, 2019, earning 7 points in the education component, 7 points in the program standards component meting all enhanced requirements plus reduced ratios and 1 quality point by offering a staff benefits package and an infrastructure of parent involvement. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-nine percent as of October 17, 2024. The NC Secretary of State website was reviewed on October 17, 2024, and Telamon Corporation was listed as current- active. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, transitions, and personal care routines. The caregivers were interacting and meeting the developmental needs for each of the children. Files for new staff were reviewed. We discussed requesting Environment Rating Scale (ERS) assessments in an effort to increase the program's star rating. Your program currently operates with a five-star center license, issued December 18, 2019, earning 7 points in the education component, 7 points in the program standards component meeting enhanced requirements and reduced ratios and 1 quality point by offering a staff benefits package and an infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. The center's compliance history was reviewed with the operator. The program’s compliance history was one hundred percent as of February 28, 2024. The NC Secretary of State website was reviewed on February 28, 2024, and Telamon Corporation was listed as current- active. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, transitions, and personal care routines. The caregivers were interacting and meeting the developmental needs for each of the children. Files for new staff were reviewed. Rated license requirements were reviewed to ensure compliance was maintained with current program and education components as well as the quality point. Fire and sanitation inspections have not occurred since the last visit and remain current. Today's lunch consists of Chicken Stir Fry, peppers, tomatoes, onion, peaches, noodles, and milk. One violation was cited during today's inspection.
