The visit was conducted with you, Tracy Mintz, Owner/Administrator, and Judy Thompson, Lead Teacher. The facility currently operates with a five-star rated license issued on October 14, 2019, earning six (6) points in staff education, seven (7) points in the program standards, and one (1) quality point. The program's license restrictions include the following: daytime care, meets enhance space, and meets enhance ratios minus 1. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year -old children are enrolled. The last annual compliance visit was conducted December 16, 2024. The most recent sanitation inspection for your facility was conducted on May 27, 2025. A superior sanitation classification was issued with three (3) demerits noted on the grade card. The most recent approved fire inspection for your facility was conducted on January 10, 2025, for daytime care only. The most recent fire drill was conducted on November 7, 2025, at 9:45am and recorded. The most recent shelter in place emergency drill was conducted on October 8, 2025, at 10:05am and recorded. The most recent playground safety inspection was conducted on November 7, 2025, by Tracy Mintz. You stated one (1) new staff have been hired since your most recent annual compliance visit conducted on December 16, 2024. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-seven percent (97%) as of December 1, 2025. The Secretary of State website was checked on December 1, 2025, and your business Brothers in Christ Ministry, Inc. is active and in good standing. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me, Tiffani Sims, at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. The facilities contact information was reviewed and confirmed with you. I visited each indoor and outdoor space. A walk-through of the facility was completed today; all indoor and outdoor areas were monitored. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in free play activities indoors, handwashing routines, lunch, and nap. Children in Space #3a were observed during group time, story time, indoor free play, hand washing, lunch, and naptime. Children in Space 3c were observed during indoor free play, hand washing, lunch, and naptime. Children in Space 1a were observed during indoor free play, bathroom routines, hand washing, lunch, and naptime. A checklist was used to note the requirements I monitored today. The analysis date for the most recent lead water test was May 31, 2024. Lead water testing must be completed every three years. This website also indicated there were no information available for required asbestos and lead-based paint testing. I suggested that you or the individual responsible contact RTI to start this process on completing and submitting the facilities required documentation for asbestos and lead based paint testing. These tests must be completed by December 31, 2025. You provided me with applicable program, staff, and children’s records for review. Files for one (1) new staff, one (1) existing staff, and applicable records for all staff were reviewed. All staff were listed on the ABCMS Provider Portal roster for this facility. A sample of five (5) children’s records were reviewed. NC Pre-K requirements were monitored today. Violations observed today were discussed with you and documented in the Visit Summary left with you at the conclusion of this visit. The following violations were observed: Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. In Space #3a, on December 1, 2025, one child enrolled on August 25, 2025, departure time was not documented. 10A NCAC 09 .0302(d)(4) 852 Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. In Space #1a, one child two years of age that was enrolled on September 3, 2025, had missing information that was not documented on the November 11, 2025, incident report. The missing information included the type of injury and the cause of injury. .0802 (e) Comments: Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before December 16, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. Technical Assistance was provided on the following violation observed during today's visit: -Documenting Arrival and Departure Times: This was a repeated violation. All children must be signed in and out each day for before and after school care as well as full day care. If the parent fails to sign the child in or out, it is the staff member’s responsibility to document the child’s arrival and departure times. I suggested that you designate one staff member to monitor the arrival and departure records at designated times each day to ensure all children have been signed in and signed out. -Required Documentation on Incident Reports: We discussed the information required by rule for incident reports and suggested that you review incident report requirements with all staff. I suggested that you review all incident reports once received to ensure all required information is obtained. This will allow you to gather any missing information from staff to be recorded prior to filing. Child care rule .0802(e) states the child care provider shall complete an incident report each time a child is injured as a result of an incident occurring while the child is in care. This incident report shall include: (1) facility identifying information; (2) the child's name; (3) date and time of the incident; (4) witness to the incident; (5) time the parent is notified of the incident and by whom; (6) piece of equipment involved, if applicable; (7) cause of injury, if applicable; (8) type of injury, if applicable; (9) body part injured, if applicable; (10) where the child received medical treatment, if applicable; (11) description of how and where the incident occurred, and the First Aid received; and (12) steps taken to prevent reoccurrence. This report shall be signed by the person completing it and by the parent, a copy given to the parent or a parent signature declining a copy and the report maintained in the child's file. Inspections/Emergency Drills: Fire Inspection: The last fire inspection was completed on January 24, 2024. The next fire inspection is due by January 24, 2025. Sanitation Inspection: The last sanitation inspection was completed on December 2, 2024. A “superior” classification was issued with zero (0) demerits noted on the grade card. The next sanitation inspection needs to be completed by December 31, 2025. Emergency Drills/Playground Inspections: The last shelter in place emergency drill was completed on October 30, 2024, at 9:40am and recorded. The last playground inspection was completed on December 12, 2024. The last fire drill was completed on December 12, 2024, at 10:15am and recorded. A complete walkthrough of licensed indoor and outdoor spaces was conducted. A checklist was used to note the requirements I monitored today. Files Reviewed: Children’s files, program files, and staff files were monitored today. Four (4) children's files were reviewed during today’s visit. One staff file was reviewed and three (3) new staff files were reviewed today. I discussed the Staff and Training Worksheet with you today. The form revised and verified during the visit. The following violation was observed during the visit today. I discussed the violation with you in detail at the end of the visit. In Space #3a on December 10, 2024, eight children’s departure times were not documented. 10A NCAC 09 .0302(d)(4) Technical Assistance regarding the violations observed during today’s visit: -Arrival/Departure Times As a reminder, daily sign-in and sign-out times must be recorded when children arrive and depart the child care facility each day. I suggested creating a system and review with staff of the procedures of recording arrival and departure times. I also suggested as part of the classroom mid-day and closing procedures that staff check the arrival and departure sheets to ensure accurate records are documented. Compliance History: You must maintain at least 75% Compliance History for each 18-month period as required. Prior to today’s visit your compliance history was eighty-six percent (86%). Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to a recommended administrative action. Compliance Letter: You must correct the violations found during today's visit immediately. Please send me a letter verifying compliance by December 30, 2024. Please include in that letter each item number and explain in detail how you corrected each violation and what plan will be implemented to prevent these violations from occurring again. You, Tracy Mintz, Owner/Administrator, assisted me with the visit today. The Secretary of State website was checked on August 12, 2024, and your business Brothers in Christ Ministry, Inc. is active and in good standing. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me, Tiffani Sims, at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. The facility currently operates with a five-star rated license issued on October 14, 2019, earning six (6) points in staff education, seven (7) points in the program standards, and one (1) quality point. During today’s visit, the children were observed in art, story time, music and movement activities. Inspections/Drills: The most recent sanitation inspection for your facility was conducted on April 11, 2024. A superior sanitation classification was issued with four (4) demerits noted on the grade card. The next sanitation inspection is due by April 30, 2025. The last fire inspection was completed on January 24, 2024. The next fire inspection is due by January 24, 2025. The most recent fire drill was conducted on July 9, 2024, at 11:00amam and recorded. The most recent shelter in place emergency drill was conducted on July 10, 2024, at 9:05am am and recorded. The most recent playground safety inspection was conducted on July 10, 2024. You stated one new staff has been hired since your most recent annual compliance visit conducted on January10, 2024. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. The following violations of child care requirements were observed and documented during today’s visit. One staff member employed on March 7, 2024, did not have documentation on file available for review of reviewing the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy prior to the first day caring for children. The documentation available for review was documented of being completed on March 28, 2024. .0608(d)(1-4) Technical Assistance regarding the violations observed during today’s visit: -Arrival/Departure Times As a reminder, daily sign-in and sign-out times must be recorded when children arrive and depart the child care facility each day. I suggested creating a system and review with staff of the procedures of recording arrival and departure times. I also suggested as part of the classroom mid-day and closing procedures that staff check the arrival and departure sheets to ensure accurate records are documented. -Prevention of Shaken Baby Syndrome and Abusive Head Trauma Policy: As a reminder, documentation of review for the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy must be kept in the staff’s file and be available for review prior to providing care with children. I suggested using the DCDEE staff file checklist and highlight when specific due dates of when documentation is required to be completed in the staff file. I also suggested writing on a calendar when specific staff documentation is due such as prior to employment, on the first day, by sixty days, and so forth. Compliance Letter Documentation: The violations documented above must be corrected immediately. Send me a letter of compliance describing in detail specifically how each violation was corrected and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit and license ID number in the letter. I must receive your compliance letter by August 27, 2024. In addition to the updates to this rule, 10A NCAC 41C .1001 through .1007 and 15A NCAC 18A .2816 require all child care facilities (homes and centers) to test for lead in drinking, water, identify lead-based paint hazard, and mitigate or restrict access to identified hazards. The Clean Classrooms for Carolina Kids program is designed to identify and eliminate exposure to lead and asbestos hazards in building infrastructure, while providing child care facilities with the support needed to meet all rule requirements. The last playground inspection was completed on January 5, 2024. The last fire drill was completed on January 4, 202, at 9:45am and recorded. A complete walkthrough of licensed indoor and outdoor spaces was conducted. A checklist was used to note the requirements I monitored today. Files Reviewed: Children’s files, program files, and staff files were monitored today. Five (5) children's files were reviewed during today’s visit. Three (3) staff files were reviewed, and two (2) new staff files were reviewed today. I discussed the Staff and Training Worksheet and the Children’s Record form with you today. Both forms were completed and verified during the visit. The following violations were observed during the visit today. I discussed these violations with you in detail at the end of the visit. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff member employed on September 5, 2023, did not have documentation on file for review of a current First Aid training. The documentation for review for First Aid training was needed to be completed by December 5, 2023. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff member employed on September 5, 2023, did not have documentation on file for review of a current CPR training. I suggested creating a system and review with staff the procedures of recording sign-in and sign-out times daily to ensure accurate records are kept. Documentation for Review of EPR Plan and Emergency Medical Care Plan: I suggested creating a schedule to ensure the required documents are completed, reviewed, and placed in the staff’s files and made available for review. I also suggested you use the DCDEE file checklist for Staff to either set up staff files and/or to update staff files with specific dates on when staff need to review and complete specific forms and documents. Documentation of Training (CPR/First Aid/Recognizing and Responding to Suspicions of Child Maltreatment): We discussed reviewing all staff files to ensure the required training documentation is on file and available for review. I suggested creating a calendar of when staff need to complete trainings and/or highlight on the staff file checklist when specific training must be completed. Compliance History: You must maintain at least 75% Compliance History for each 18-month period as required. Prior to today’s visit your compliance history was ninety-three (93) %. Please note any violations cited during future visits will negatively impact your compliance history. You, Tracy Mintz, Owner/Administrator, and Leslie Mintz, NC Pre K Lead Teacher, assisted me with the visit today. The Secretary of State website was checked on January 9, 2024, and your business Brothers in Christ Ministry, Inc. is active and in good standing. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me, Tiffani Sims, at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. The facility currently operates with a five-star rated license issued on October 14, 2019, earning six (6) points in staff education, seven (7) points in the program standards, and one (1) quality point. The program's license restrictions include the following: daytime care, meets enhance space, and meets enhance ratios minus 1. During today’s visit, the children were observed in indoor free play, breakfast, nap time, and music. Inspections/Emergency Drills: Fire Inspection: The last fire inspection was completed on February 6, 2023. Sanitation Inspection: The last sanitation inspection was completed on September 29, 2023. A “superior” classification was issued with four (4) demerits noted on the grade card. Emergency Drills/Playground Inspections: The last shelter in place drill emergency drill was completed on January 4, 2024, at 10:00am and recorded. The last playground inspection was completed on January 5, 2024. The last fire drill was completed on January 4, 202, at 9:45am and recorded. A complete walkthrough of licensed indoor and outdoor spaces was conducted. A checklist was used to note the requirements I monitored today. Files Reviewed: Children’s files, program files, and staff files were monitored today. Five (5) children's files were reviewed during today’s visit. Three (3) staff files were reviewed, and two (2) new staff files were reviewed today. I discussed the Staff and Training Worksheet and the Children’s Record form with you today. Both forms were completed and verified during the visit. The following violations were observed during the visit today. I discussed these violations with you in detail at the end of the visit. Violation Number Comment Rule 125 Daily records of arrival and departure times for children enrolled at the center were not maintained as children arrive and depart and/or were not made available for review. In Space #1a, on December 20, 2023, one child’s departure times was not documented. In Space #2, on December 15, 2023, one child’s departure time was not documented. In Space #3a, on December 11, 2023, one child’s departure time was not documented. In Space #3c, on December 11, 2023, five children’s departure times were not documented. 10A NCAC 09 .0302(d)(4) 862 The EMC plan was not reviewed with all staff annually and whenever the plan was revised. Three staff members employed on June 1, 2007, June 30, 2007, and November 17, 2015, did not on file for review documentation of review of the emergency medical care plan. The last documentation available for review was dated February 1, 2022. 10A NCAC 09 .0802(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff member employed on September 5, 2023, did not have documentation on file for review of a current First Aid training. The documentation for review for First Aid training was needed to be completed by December 5, 2023. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff member employed on September 5, 2023, did not have documentation on file for review of a current CPR training. The documentation for review for CPR training needed to be completed by December 5, 2023. .1102(d) 1825 All staff did not review the center's EPR Plan during orientation and/or on an annual basis with the trained staff. Documentation of the review was not maintained on file. Three staff members employed on June 1, 2007, June 30, 2007, and November 17, 2015, did not have documentation on file for review of the annual Emergency Preparedness and Response plan. The last documentation available for review was dated February 1, 2022. .0607(f) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One staff member employed on September 5, 2023, did not have available for review training documentation for completing the Recognizing and Responding to Suspicions of Child Maltreatment by the required due date of December 5, 2023. .1102(g) The following Technical Assistance regarding the violations observed during today’s visit was provided: -Arrival/Departure Times As a reminder, daily sign-in and sign-out times must be recorded when children arrive and depart the child care facility to ensure accurate records are kept. I suggested creating a system and review with staff the procedures of recording sign-in and sign-out times daily to ensure accurate records are kept. Documentation for Review of EPR Plan and Emergency Medical Care Plan: I suggested creating a schedule to ensure the required documents are completed, reviewed, and placed in the staff’s files and made available for review. I also suggested you use the DCDEE file checklist for Staff to either set up staff files and/or to update staff files with specific dates on when staff need to review and complete specific forms and documents. Documentation of Training (CPR/First Aid/Recognizing and Responding to Suspicions of Child Maltreatment): We discussed reviewing all staff files to ensure the required training documentation is on file and available for review. I suggested creating a calendar of when staff need to complete trainings and/or highlight on the staff file checklist when specific training must be completed. Compliance History: You must maintain at least 75% Compliance History for each 18-month period as required. Prior to today’s visit your compliance history was ninety-three (93) %. Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to a recommended administrative action. Compliance Letter: You must correct the violations found during today's visit immediately. Please send me a letter verifying compliance by January 24, 2024. Please include in that letter each item number and explain in detail how you corrected each violation and what plan will be implemented to prevent these violations from occurring again. Restoration Learning Academy is in cohort #1 for resuming the star rated license reassessment. Your child care program’s prep year began on July 1, 2023, through June 30, 2024. Applicants who achieve a score of 80% or higher on the NC Early Childhood Equivalency Exam earn the new North Carolina Early Childhood Equivalency Certificate. You, Judy Thompson, Teacher assisted me with today's visit. Your program currently operates with a five-star license effective October 14, 2019. You have earned six (6) points in staff education, seven (7) points in program standards, and one (1) quality point. The program's license restrictions include the following: daytime care, meets enhanced space, and meets enhanced rations minus 1. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-eight percent (98%) as of July 24, 2023. The NC Secretary of State website was viewed on July 24, 2023, and Brothers in Christ Ministry, Inc. was current and active. A walk-through of the facility was completed today, and all indoor and outdoor areas were monitored. Staff and children were observed during free play and outdoor gross motor play. Inspections/Drills: The most recent sanitation inspection for your facility was conducted on March 27, 2023. A superior sanitation classification was issued with four (4) demerits noted on the grade card. The most recent fire inspection for your facility was conducted on February 6, 2023. The most recent fire drill was conducted on June 23, 2023, at 9:30am. The most recent emergency drill was conducted on May 19, 2023, at 10:00am. The most recent playground safety inspection was conducted on June 23, 2023. One new staff have been hired since your most recent annual compliance visit conducted on January 24, 2023. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Trainings, including Health and Safety Training, and Criminal Background Checks. The following violations of child care requirements were observed and documented during today’s visit. Violations and technical assistance were thoroughly reviewed with you, Ms. Thompson, and documented in the visit summary. At the end of the routine unannounced visit, the visit summary and staff and training worksheets were not able to be signed due to Mr. Mintz, Owner/Administrator, being out of town and no one present on-site was designated to be the legal designee for the child care facility. An electronic visit summary and staff and training worksheets were emailed to you, Mr. Mintz, requesting signatures on the first page of the visit summary and on all three pages of the staff and training worksheets. A legal designee form was also emailed to you, Mr. Mintz, to update information on who can sign documents during visits. One staff member employed on July 17, 2023, documentation of medical report, proof of tuberculosis screening, and health questionnaire were not maintained in a separate medical file from the staff’s personnel file. .0701(d) Technical Assistance regarding the violations observed during today’s visit: -Documentation of Training (CPR/First Aid/Special Trainings): I discussed with you, Mr. Mintz, Owner/Administrator, by telephone, and asked if the required documentation was available for review to verify the information on the staff’s training worksheets. I asked to review current CPR/First Aide and Health and Safety certificates. You, Mr. Mintz stated staff’s professional development information was located in your locked office and staff on-site at the child care facility did not have access to the requested information. I reminded you, Mr. Mintz, documentation of professional development trainings must be available for review during monitoring visits. You, Mr. Mintz, stated you would send documentation of trainings by the end of the week. Staff Medical Information Separate File: I, reviewed by telephone, with you, Tracy Mintz, Owner/Administrator, and you, Judy Thompson, Teacher, during the exit conference, child care rule 10A NCAC 09 .0701 HEALTH STANDARDS FOR CHILD CARE PROVIDERS, SUBSTITUTE PROVIDERS, VOLUNTEERS, AND UNCOMPENSATED PROVIDERS (d) which stated "staff medical statements, proof of a tuberculosis test or screening, and completed health questionnaires shall be included in the staff member's medical file, which must be maintained separately from the staff member's individual personnel file in the center". The period for background check qualification renewals was revised from three years to five years. 2. The provisional qualification period was reduced to forty-five days. EPR Plan: -I reminded you that the EPR Plan needs to be updated annually. We discussed the facility’s EPR plan needs to be reviewed with staff during orientation and annually. Documentation of reviewing the EPR plan with staff will need to be available for review. If no changes to the facility’s EPR plan are required, re-publish the EPR plan in the Emergency Management Portal and print the first page of the EPR plan with the updated review date.
