You, Missy Hamrick, Owner/Administrator, and Stacy McDaniel, Lead Teacher/Assistant Director, assisted me with the visit today. The facility currently operates with a 5-star rated license issued on April 17, 2026, earning seven (7) points in staff education, six (6) points in the program standards, and one (1) quality point. The program's license restrictions include the following: daytime care, enhanced ratios, meets enhanced space, Other: Building #1-40 children; Building #2-15; Building #3-42 children, Building #5-16 children; Building #6-21 children; Building #4-65 children The last annual compliance visit was conducted January 21, 2025. The most recent sanitation inspection for your facility was conducted on March 3, 2026. A superior sanitation classification was issued with eight (8) demerits noted on the grade card. The most recent approved fire inspection for your facility was conducted on July 25, 2025, for daytime care only. The most recent monthly fire drill was conducted on May 20, 2026, at 10:45am and recorded. The most recent quarterly shelter in place emergency drill was conducted on April 26, 2026, at _10:30am and recorded. The facilities EPR Plan was updated on October 27, 2025. The most recent monthly playground inspection was completed on _June 12, 2026, by Missy Hamrick. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-seven percent (97%) as of June 16, 2026. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me, Tiffani Sims, at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. I visited each indoor and outdoor space. A walk-through of the facility was completed today; all indoor and outdoor areas were monitored. I observed children in the indoor learning environments and found supervision and staff/child ratios to be in compliance with child care requirements. Children throughout the facility were participating in indoor free play, outdoor gross motor play, teacher directed activities, story time, bottle feeding, and naptime. A limited monitoring of staff files was conducted for the purpose of reviewing CPR/First Aid and special training. Supervision, staff/child ratio, storage of hazardous products, storage of medication, adequate/approved space, license posted, permit restrictions and program records were also monitored. You provided me with applicable program and staff records for review. The analysis date for the most recent lead water test was August 28, 2025. There is no requirement to restrict access or to mitigate lead-based paint that is in good condition per 10A NCAC 41C.1004. On October 11, 2024, The Clean Classrooms for Carolina Kids™ team determined that MAMA LISSA'S TINY TOT UNIVERSITY required an on-site visit to fulfill North Carolina rule requirements. An accredited asbestos professional did not identify asbestos hazards at the facility. There is no requirement to restrict access to or mitigate asbestos-containing materials that are in good condition per 10A NCAC 41C .1003. Rated License Information: We discussed your application packet for the Curriculum and Instructional Quality Pathway #2 option. The license is based on the QRIS Pathway to the Stars educational and programmatic requirements for Pathway #2. A five-star level is the goal that you would like to achieve for your facility. We discussed that your facility would need to follow the required reduced enhanced staff/child ratios. Your new Star License will be determined once all programmatic requirements and staff education have been verified. Once a new license has been issued by DCDEE, it will be mailed directly to you and must be posted upon receipt. At that time, return the old license to me as it remains the property of the State of North Carolina. I reviewed with you the Pathway #2 Classroom & Instructional Quality Start Star Level Assessment for Child Care Centers during today’s visit. The following QRIS Pathway #2 documents were reviewed during today’s visit. •Application for Assessment for a Rated License for Centers was updated during today’s visit to reflect the current curricula and formative assessments the facility is currently using. •Family and Community Engagement Standards plan was reviewed with you. We discussed your plan that you submitted during today’s visit. •Staff Information and Education Worksheet was completed and available for review. •Continuous Quality Improvement Plan for your facility was reviewed with you, and suggestions of possible goals were offered. •Continuous Quality Improvement Plan/Professional Development Plan for your individual staff was reviewed with you, and suggestions of possible goals were offered. The following violations were observed: Violation Number Comment Rule 895 Infants under the age of 12 months were not placed on their backs for sleeping unless the center had obtained the appropriate written waiver. In Building #1, Space #3, on May 18, 2026, at 10:15am an infant ten months of age and enrolled on March 2, 2026, first sleep position was documented on the sleep chart as side. In Space #3, on June 10, 2026, at 10:00am an infant six months of age and enrolled on March 6, 2026, first sleep position was documented on the sleep chart as tummy. .0606(a)(1)(A-B) Comments: Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. All staff were listed on the ABCMS Provider Portal roster for this facility. A sample of thirteen (13) children’s records were reviewed. I observed during lunch and the lunch menu consisted of spaghetti with meat sauce, corn, peas, wheat bread, and milk. Violations observed today were discussed with you and documented in the Visit Summary left with you at the conclusion of this visit. The following violations were observed: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The current activity plan available for review was dated for the week of January 19, 20026 and not for the required week of January 12, 2026. GS 110-91(12); .0508(a) 1115 First aid kit and/or fire extinguisher was not located in vehicle; kit and/or extinguisher was not mounted or secured if in passenger area. In the 2007 Chevy Express 3500 white bus vehicle used to for transportation, the first aid kit was missing. 10A NCAC 09 .1003(c) 1835 The medical action plan was not updated on an annual basis or when changes to the plan were made by the child's parent or health care professional. In Building #2 Space #5, the medical action plan for one child four years of age enrolled on January 6, 2025, had expired on January 6, 2026. .0801(b) Technical Assistance was provided on the following: Activity Plans: I suggested as a mid-morning check that you remind staff to make sure the correct activity plan is posted for the week. The violation was corrected during the visit. First Aid Kit in Vehicle: I suggested the drivers of the buses to conduct a monthly vehicle check to ensure all required documents, first aid kit, fire extinguisher are located in each vehicle used for transportation. The violation was corrected during the visit. Medical Action Plan Expired: I suggested that staff review monthly medical action plans for children enrolled in their classroom are current. The violation was corrected during the visit. T You, Missy Hamrick, Owner/Administrator, and Stacy McDaniel, Lead Teacher/Assistant Director, assisted me with the visit today. The facility currently operates with a 5-star rated license issued on December 2, 2019, earning seven (7) points in staff education, six (6) points in the program standards, and one (1) quality point. The program's license restrictions include the following: daytime care, enhanced ratios, meets enhanced space, building #1-40 children, building #2-15 children, building #1 and #2 -50 children and 6 adults, building #3-42 children and 6 adults, building 5-16 children, and building 6-21 children. The last annual compliance visit was conducted January 21, 2025. The center's compliance history was reviewed with the operator. The program’s compliance history was one hundred percent (100%) as of January 8, 2026. The most recent sanitation inspection for your facility was conducted on April 14, 2025. A superior sanitation classification was issued with four (4) demerits noted on the grade card. The most recent approved fire inspection for your facility was conducted on August 1, 2025, for daytime care only. The most recent fire drill was conducted on December 23, 2025, at 10:00am and recorded. The most recent shelter in place emergency drill was conducted on October 31, 2025, at 4:45pm and recorded. The most recent playground safety inspection was conducted on Januaryb2, 2026, by Stacy McDaniels. The Secretary of State website was checked on January 8, 2026, and your business Mama Lissa’s Tiny Tot University LLC is current and active. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me, Tiffani Sims, at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. I visited each indoor and outdoor space. A walk-through of the facility was completed today; all indoor and outdoor areas were monitored. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in indoor free play, story time, art activities, center time, diapering, lunch, and nap time. A checklist was used to note the requirements I monitored today. The analysis date for the most recent lead water test was August 28, 2025. Lead water testing must be completed every three years. On September 2, 2025, The Clean Classrooms for Carolina Kids™ team determined that MAMA LISSA'S TINY TOT UNIVERSITY required an on-site visit to fulfill North Carolina rule requirements. A certified lead risk assessor did not identify lead-based paint hazards at the facility. There is no requirement to restrict access or to mitigate lead-based paint that is in good condition per 10A NCAC 41C.1004. On October 11, 2024, The Clean Classrooms for Carolina Kids™ team determined that MAMA LISSA'S TINY TOT UNIVERSITY required an on-site visit to fulfill North Carolina rule requirements. An accredited asbestos professional did not identify asbestos hazards at the facility. There is no requirement to restrict access to or mitigate asbestos-containing materials that are in good condition per 10A NCAC 41C .1003. You provided me with applicable program, staff, and children’s records for review. Files for three (3) new staff, and three (3) existing staff, and applicable records for all staff were reviewed. All staff were listed on the ABCMS Provider Portal roster for this facility. A sample of thirteen (13) children’s records were reviewed. I observed during lunch and the lunch menu consisted of spaghetti with meat sauce, corn, peas, wheat bread, and milk. Violations observed today were discussed with you and documented in the Visit Summary left with you at the conclusion of this visit. The following violations were observed: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The current activity plan available for review was dated for the week of January 19, 20026 and not for the required week of January 12, 2026. GS 110-91(12); .0508(a) 1115 First aid kit and/or fire extinguisher was not located in vehicle; kit and/or extinguisher was not mounted or secured if in passenger area. In the 2007 Chevy Express 3500 white bus vehicle used to for transportation, the first aid kit was missing. 10A NCAC 09 .1003(c) 1835 The medical action plan was not updated on an annual basis or when changes to the plan were made by the child's parent or health care professional. In Building #2 Space #5, the medical action plan for one child four years of age enrolled on January 6, 2025, had expired on January 6, 2026. .0801(b) Technical Assistance was provided on the following: Activity Plans: I suggested as a mid-morning check that you remind staff to make sure the correct activity plan is posted for the week. The violation was corrected during the visit. First Aid Kit in Vehicle: I suggested the drivers of the buses to conduct a monthly vehicle check to ensure all required documents, first aid kit, fire extinguisher are located in each vehicle used for transportation. The violation was corrected during the visit. Medical Action Plan Expired: I suggested that staff review monthly medical action plans for children enrolled in their classroom are current. The violation was corrected during the visit. Consultation: We reviewed Pathway #2, Family and Community Engagement Foundational Practices, Continuous Quality Improvement, approved curriculum, formative assessment tools, and staff to child ratio options. During today’s visit, the children were observed in indoor free play, nap time, art, and small group time. Inspections/Emergency Drills: Fire Inspection: The last fire inspection was completed on September 11, 2023. Sanitation Inspection: The last sanitation inspection was completed on October 10, 2023, A “superior” classification was issued with four (4) demerits noted on the grade card. Emergency Drills/Playground Inspections: The last lockdown drill was completed on January 15, 2024, at 3:25pm and recorded. The last playground inspection was completed on January 5, 2024. The last fire drill was completed on January 10, 2024, at 3:00pm. A complete walkthrough of licensed indoor and outdoor spaces was conducted. A checklist was used to note the requirements I monitored today. Files Reviewed: Children’s files, program files, and staff files were monitored today. Ten (10) children's files were reviewed during today’s visit. Three (3) staff files were reviewed, and two (2) new staff files were reviewed today. I discussed the Staff and Training Worksheet and the Children’s Record form with you today. Both forms were completed and verified during the visit. The following violations were observed during the visit today. I discussed these violations with you in detail at the end of the visit. Violation Number Comment Rule 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. In Building #1 Space #4, on January 24, 204, the sleep positions were not documented for one child at 10:15am and 10:30am. .0606(g) 1128 For routine transport of children to and from the center, staff did not use the list to document attendance as children boarded and departed the vehicle. Attendance documentation, which documented children boarding and departing the vehicle, was not available for review for routine transport to and from the center. 10A NCAC 09 .1003(l) The following Technical Assistance regarding the violations observed during today’s visit was provided: -Safe Sleep Checks: As a reminder, review with staff on how to complete the safe sleep check chart and how to record the safe sleep positions. I also suggested creating a system where the safe sleep documentation is reviewed and checked for accuracy. -Documenting Attendance (Transporting Van) As a reminder, boarding and departing times must be documented on a roster daily. We discussed documenting specific time when children are boarding and departing the vans to ensure accurate records are kept. Compliance History: You must maintain at least 75% Compliance History for each 18-month period as required. Prior to today’s visit your compliance history was ninety-four percent (94%). Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to a recommended administrative action. Compliance Letter: You must correct the violations found during today's visit immediately. Please send me a letter verifying compliance by February 15, 2024. Please include in that letter each item number and explain in detail how you corrected each violation and what plan will be implemented to prevent these violations from occurring again.
