You, Twanette Davenport, Owner/Administrator, and Valerie Brown, Assistant Director, assisted me with the visit today. The facility currently operates with a 5-star rated license issued on May 23, 2022, earning five (5 points in staff education, seven (7) points in the program standards, and one (1) quality point. The program's license restrictions include the following: daytime care, 2nd shift, overnight, meets enhanced ratios, meets enhanced space, and meeting enhanced staff/child ratios minus 1. No children are enrolled during 2nd and 3rd shifts Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year -old children are enrolled. Creative Curriculum is the approved curriculum the facility is currently using. The last annual compliance visit was conducted on August 6, 2025. The most recent sanitation inspection for your facility was conducted on July 2, 2026. A superior sanitation classification was issued with zero (0) demerits noted on the grade card. The most recent approved fire inspection for your facility was conducted on July 30, 2025. The most recent monthly fire drill was conducted on July 2, 2026, at 1:30pm and recorded. The most recent quarterly shelter in place emergency drill was conducted on July 2, 2026, at 3:15pm and recorded. The most recent monthly playground inspection was completed on July 1, 2026, by Valerie Brown. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-three percent (93%) as of July 6, 2026. You visited each indoor and outdoor space with me. A walk-through of the facility was completed today; all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in outdoor gross motor play, lunch, hand washing, naptime, and indoor free play. The caregivers were interacting and meeting the developmental needs for each of the children. Lunch time was observed and children were served bologna and cheese sandwiches on wheat bread, tater tots, applesauce, and milk. We discussed the test results that were available for review on the Clean Classrooms for Carolina Kids website for lead testing water testing, lead-based paint testing, and asbestos testing for your facility. The lead water testing results were completed on August 2, 2023. I reminded you that lead-water testing must be completed every three years. I suggested contacting Clean Classrooms for Carolina Kids to discuss the process of getting the facility’s water retested. The Clean Classrooms for Carolina Kids™ team determined that LITTLE KINGS AND QUEENS CHILD DEVELOPMENT CENTER, required an on-site visit to fulfill North Carolina rule requirements. A certified lead risk assessor did not identify lead-based paint hazards at the facility. There is no requirement to restrict access or to mitigate lead-based paint that is in good condition per 10A NCAC 41C.1004. The Clean Classrooms for Carolina Kids™ team determined that LITTLE KINGS AND QUEENS CHILD DEVELOPMENT CENTER, required an on-site visit to fulfill North Carolina rule requirements. An accredited asbestos professional did not identify asbestos hazards at the facility. There is no requirement to restrict access to or mitigate asbestos-containing materials that are in good condition per 10A NCAC 41C .1003. Rated License Information: You submitted your application for re-assessment during today’s visit, choosing Curriculum and Instructional Quality Pathway #2 option during today’s visit. We discussed that your rated license packet is on hold because two of your staff have “return letters” from DCDEE WORKS. I discussed with you once staff’s education has been updated in the DCDEE WORKS portal and is available for review then the Pathway #2 rated license packet can be evaluated for the education star level. We also discussed that you and lead teachers have not completed the Funny Daffer formative assessment training. I reminded you once this task has been completed to submit the documentation for review. Your new Star License will be determined once all programmatic requirements and staff education have been verified. The following QRIS Pathway #2 documents were reviewed during today’s visit. •Application for Assessment for a Rated License for Centers was received and reviewed during today’s visits. •Family and Community Engagement Standards plan that will be implemented once your new license has been issued. I reviewed your plan with you during today’s visit and offered suggestions on possible goals pertaining to Pathway #2 requirements. •Staff Information and Education Worksheet was submitted during today’s visit. We discussed which staff needed to be listed on the forms. I also answered your questions concerning what information needed to be documented on the forms for each staff. I discussed with you that not all staff had information available for review on the DCDEE WORKS portal. I suggested visiting the DCDEE WORKS webpage for guidance on the two return status letters who are missing education information. •Continuous Quality Improvement Plan for your facility that will be implemented once your new license has been issued. I reviewed Facility’s CQI plan with you and offered suggestions related to Pathway #2. •Continuous Quality Improvement Plan/Professional Development Plan for your individual staff that will be implemented once your new license has been issued. I reviewed staff’s Individual CQI Plan and offered suggestions on possible goals related to Pathway #2. You provided me with applicable program, staff, and children’s records for review. Files for two existing staff, and applicable records for all staff were reviewed. All staff were listed on the ABCMS Provider Portal roster for this facility. A sample of four children’s records were reviewed. I discussed the Staff and Training Worksheet and the Children’s Record form with you today. Both forms were completed and verified during the visit. The following violations were observed during the visit today. I discussed these violations with you in detail at the end of the visit. A printed copy of the visit summary was reviewed, signed, and left with you at the end of the visit. Violation Number Comment Rule 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. In Space #2, on June 17, 2026, at 1:00pm and 1:15pm and on June 22, 2026, at 11:15am, an infant six months of age and enrolled on March 31, 2026, sleep position was not documented on the sleep chart. .0606(g) The following Technical Assistance regarding the violations observed during today’s visit was provided: Documenting Safe Sleep Checks: Documents that verify staff members’ compliance with visual checks on infants shall be maintained for a minimum of one month. Visually checking on sleeping infants every fifteen minutes, per your facilities safe sleep policy, is critical in protecting sleeping infants from sudden infant death syndrome. I suggested that you discuss requirements for safe sleep checks as well as sudden infant death syndrome with all staff, especially all staff who work in the classroom for infants. I suggested utilizing the Caring for Our Children resource book for more information pertaining to safe sleep practices and sudden infant death syndrome. I suggested determining how safe sleep checks will be turned in to administrative staff and create a plan to review safe sleep checks prior to filing. We discussed updating and/or creating an account on the DCDEE WORKS portal for staff’s education to be evaluated for the upcoming Pathways to the Stars process. An updated DCDEE WORKS account is required for all staff to process the education standard for your program. QRIS Education will be evaluated for administrators and staff who provide direct care to children. Provider Access to ABCMS On July 1, 2026, I reviewed the information available on the ABCMS Portal System for your facility. The information available for review showed that you created a business account for your facility and had linked current staff to your ABCMS Portal account. I reviewed the staff roster available on the ABCMS Portal to the staff and training worksheet with you for accuracy. I reminded you to update the ABCMS Portal if new staff are hired or if staff are terminated. -On-going Trainings: Please visit Moodle on the DCDEE website to obtain necessary on-going training hours as required annually. The Secretary of State website was checked on August 4, 2025, and your business Little Kings and Queens Child Development Center, is active and in good standing. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me, Tiffani Sims, at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. Inspections/Emergency Drills: Fire Inspection: The last fire inspection was completed on July 30, 2025. Sanitation Inspection: The last sanitation inspection was completed on July 15, 2025. A “superior” classification was issued with four (4) demerits noted on the grade card. Emergency Drills/Playground Inspections: The last shelter in place drill was completed on July 15, 2025, at 9:15am and recorded. The last playground inspection was completed on August 4, 2025. The last fire drill was completed on August 1, 2025, at 9am and recorded. A complete walkthrough of licensed indoor and outdoor spaces was conducted. A checklist was used to note the requirements I monitored today. We discussed if your facility provided transportation. You stated that at this time transportation services were not being provided. I reminded you that before children can be transported, I must make another visit to inspect the vehicle and review all required documents. Files Reviewed: Children’s files, program files, and staff files were monitored today. Five (5) children's files were reviewed during today’s visit. One (1) staff file was reviewed, and three (3) new staff files were reviewed today. I discussed the Staff and Training Worksheet and the Children’s Record form with you today. Both forms were completed and verified during the visit. The following violations were observed during the visit today. I discussed these violations with you in detail at the end of the visit. In Space #1, on August 5, 2025, four children departure times were not documented. On July 25, 2025, one child’s arrival time was not documented in Space #1. In Space # 3, on August 4, 2025, one child’s departure time was not documented. 10A NCAC 09 .0302(d)(4) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff member employed on May 6, 2025, did not have documentation on file not available for review of completing the required first aid training within ninety days of employment. The staff member needed to complete the required by August 6, 2025. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff member employed on May 6, 2025, did not have documentation on file not available for review of completing the required CPR training within ninety days of employment. The staff member needed to complete the CPR training by August 6, 2025. .1102(d) 1898 Staff did not complete the health and safety training within one year of employment. One staff member employed on July 12, 2024, did not have available on file for review of completing the Medication in Child Care Health and Safety training by the required due date of July 12, 2025. .1102(a) The following Technical Assistance regarding the violations observed during today’s visit was provided: -Arrival/Departure Times As a reminder, daily sign-in and sign-out times must be recorded when children arrive and depart the child care facility to ensure accurate records are kept. I suggested creating a system and review with staff of the procedures of recording sign-in and sign-out times. I also suggested that the closing teacher in each classroom checks the daily sign-in and sign-out documentation to ensure each child present has been checked in and out for the day. I suggested that you follow up with the closing staff to ensure accurate records are maintained. First Aid/CPR training I discussed with you that staff must have a current CPR and First Aid training to care for children. I suggested using the staff file checklist and highlighting when specific trainings are needed to be renewed. I also suggested reviewing the Staff and training worksheet to monitoring when specific trainings are needed to be renewed. I also discussed with you that if these training courses were not available in Cleveland County within the ninety days requirement that your staff can seek these training courses in another county. Health and Safety Training (1st year of employment): I discussed with you that the newly hired staff must complete all of the Health and Safety training within one year of their hired date. I suggested using either a calendar and write when specific trainings are due and also use the staff checklist and highlighting specific due dates to ensure that trainings are completed within the required due date. Compliance History: You must maintain at least 75% Compliance History for each 18-month period as required. Prior to today’s visit your compliance history was ninety-nine percent (99%). Please note any violations cited during future visits will negatively impact your compliance history. You, Twanette Davenport, Owner/Administrator, and Valerie Brown, Lead Teacher, assisted me with the visit today. The NC Secretary of State website was viewed on August 30, 2023, and Little Kings and Queens Child Development Center was listed as current and active. The facility currently operates with a 5-star rated license issued on May 23, 2022, earning five (5) points in staff education, seven (7) points in the program standards, and one (1) quality point. The program's license restrictions include the following: daytime care, 2nd shift, overnight care, enhanced ratios minus 1, and enhanced space. Inspections/Emergency Drills: Fire Inspection: The last fire inspection was completed on August 16, 2023. Sanitation Inspection: The last sanitation inspection was completed on April 26, 2023. A “superior” classification was issued with seven (7) demerits noted on the grade card. Emergency Drills/Playground Inspections: The last shelter in place drill was completed on July 10, 2023, at 10:30am and recorded. The last playground inspection was completed on August 2, 2023. The last fire drill was completed on August 2, 2023, at 9:10am and recorded. A complete walkthrough of licensed indoor and outdoor spaces was conducted. A checklist was used to note the requirements I monitored today. Staff and children were observed during outdoor gross motor play, indoor free play, art activities, and story time. Files Reviewed: Children’s files, program files, and staff files were monitored today. Five (5) children's files were reviewed during today’s visit. Two (2) staff files were reviewed and one (1) new staff file was reviewed today. I discussed the Staff and Training Worksheet and the Children’s Record form with you today. Both forms were completed and verified during the visit. The following violations were observed during the walk through today. I discussed these violations with you in detail at the end of the visit. One child enrolled on April 4, 2021, had a health assessment on file for review dated November 23, 2021. The health assessment needed to be on file for review by May 4, 2021. GS110-91(1) 1323 Each child was not immunized as per Article 6 of Chapter 130A and an immunization record was not on file before or within 30 days after admission. One child enrolled on April 4, 2021, had an immunization record on file for review that was dated November 23, 2021. The immunization recorded needed to be on file for review by May 4, 2021. One child enrolled on December 13, 2021, had an immunization record on file for review that was dated February 10, 2022. We discussed creating a schedule and putting specific dates on a calendar as a reminder when children’s health assessments and immunization records are due as a way to ensure accurate records are kept. I also suggested you use the children’s file checklist to document when required records are due. Compliance History: You must maintain at least 75% Compliance History for each 18-month period as required. Prior to today’s visit your compliance history was ninety-three (93) %. Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to a recommended administrative action. Compliance Letter: You must correct the violations found during today's visit immediately.
