The rated license visit was not conducted due to the facility needed additional time to complete the QRIS requirements. You, Michelle Chamber, Owner/Administrator, assisted me with the visit today. The facility currently operates with a 5-star rated license issued on March 2, 2023, earning seven (7) points in staff education, six (6) points in the program standards, and one (1) quality point. The program's license restrictions include the following: daytime care, 2nd shift, overnight, meets enhanced ratios, meets enhanced space, and no more than 80 children and staff in one day. No children were enrolled in second and/or third shifts. The last annual compliance visit was conducted on December 23, 2025. The most recent sanitation inspection for your facility was conducted on May 15, 2026. A superior sanitation classification was issued with nine (9) demerits noted on the grade card. The most recent approved fire inspection for your facility was conducted on February 24, 2026, for daytime care only. The most recent monthly fire drill was conducted on May 13, 2026, at 8:00am and recorded. The most recent quarterly lockdown emergency drill was conducted on April 30, 2026, at 11:00am and recorded. The facilities EPR Plan was reviewed on January 11, 2025. The most recent monthly playground inspection was completed on May 13, 2026, by Michelle Chambers. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-six percent (96%) as of June 8, 2026. The Secretary of State website was checked on June 8, 2026, and your business Kiddieland Child Development Center of Shelby Inc., is active and in good standing. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me, Tiffani Sims, at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. I visited each indoor and outdoor space. A walk-through of the facility was completed today; all indoor and outdoor areas were monitored. I observed children in the indoor learning environments and found supervision and staff/child ratios to be in compliance with child care requirements. Children throughout the facility were participating in outdoor gross motor, lunch, and naptime. A limited monitoring of staff files was conducted for the purpose of reviewing CPR/First Aid and special training. Supervision, staff/child ratio, storage of hazardous products, storage of medication, adequate/approved space, license posted, permit restrictions and program records were also monitored. You provided me with applicable program and staff records for review. No new staff has been hired since your last annual compliance visit conducted on December 23, 2025. Their file was reviewed during today’s visit. The analysis date for the most recent lead water test was June 21, 2024. Lead water testing must be completed every three years. As of June 8, 2026, The Clean Classrooms for Carolina Kids™ team determined that KIDDIELAND CHILD DEVELOPMENT CENTER required an on-site visit to fulfill North Carolina rule requirements. A certified lead risk assessor did not identify lead-based paint hazards at the facility. There is no requirement to restrict access or to mitigate lead-based paint that is in good condition per 10A NCAC 41C.1004. The Clean Classrooms for Carolina Kids™ team determined that KIDDIELAND CHILD DEVELOPMENT CENTER required an on-site visit to fulfill North Carolina rule requirements. An accredited asbestos professional did not identify asbestos hazards at the facility. There is no requirement to restrict access to or mitigate asbestos-containing materials that are in good condition per 10A NCAC 41C .1003. Rated License Information: We discussed your application packet for the Curriculum and Instructional Quality Pathway #2 option. We also discussed that staff’s education for one staff member needed to be updated on the DCDEE WORKS portal in order for education to be evaluated. The license is based on the QRIS Pathway to the Stars educational and programmatic requirements for Pathway #2. Your new Star License will be determined once all programmatic requirements and staff education have been verified. Once a new license will be mailed directly to you and must be posted upon receipt. At that time, return the old license to me as it remains the property of the State of North Carolina. I reviewed with you Pathway 2 Classroom & Instructional Quality Start Star Level Assessment for Child Care Centers during today’s visit and will email you to sign once all documentation has been sent and verified. We discussed that you needed additional time to complete your Pathway #2 packet. I discussed that your next annual compliance with rated license visit will be conducted in the Fall of 2026. The following QRIS Pathway #2 documents were reviewed during today’s visit. •Application for Assessment for a Rated License for Centers will need to be submitted once all documentation is completed. •Family and Community Engagement Standards plan that will be implemented once your new license has been issued. I reviewed with you your plan that you submitted during today’s visit. •Staff Information and Education Worksheet was not completed but was available for review. We discussed which staff needed to be listed on the forms. I also answered your questions concerning what information needed to be documented on the forms for each staff. I discussed with you that not all staff DCDEE WORKS account, did not have documentation available for review. I suggested contacting DCDEE WORKS to get a status update for missing staff’s education. •Continuous Quality Improvement Plan for your facility was reviewed with you, and suggestions of possible goals were reviewed with you. •Continuous Quality Improvement Plan/Professional Development Plan for your individual staff was reviewed with you, and suggestions of possible goals was offered. The following violations were observed: Violation Number Comment Rule 1812 The center did not complete an EPR Plan within four months of completing the EPR in Child Care training and/or the Plan was not completed on a template provided by the Division. The emergency preparedness plan was last updated on January 11, 2025 .0607(c) 1825 All staff did not review the center's EPR Plan during orientation and/or on an annual basis with the trained staff. Documentation of the review was not maintained on file. Staff last reviewed the emergency preparedness plan on January 2, 2026. .0607(f) Comments: Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before June 22, 2026. I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. The EPR plan must be updated on the Emergency Management Portal annually and reviewed with new staff during orientation and annually with current staff. Consultation: Updating DCDEE WORKS Account: We discussed updating and/or creating an account on the DCDEE WORKS portal for staff’s education to be evaluated for the upcoming Pathways to the Stars process. An updated DCDEE WORKS account is required for all staff to process the education standard for your program. Education will be evaluated for administrators and staff who provide direct care to children. ABCMS Portal: The ABCMS provider portal facility roster should be updated on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. I discussed this with you and staff during the visit today and offered suggestions on providing a safe environment during naptime. Violations observed today were discussed with you and documented in the Visit Summary left with you at the conclusion of this visit. The following violations were observed during today’s visit. Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In Space 2a, in unlocked storage cabinet located on the wall next to the entrance of the classroom there was one bottle of Stinky Bowl Citrus Scented Spray. The cabinet shelf where the spray bottle with a written warning keep out of reach of children was located was 3 feet and 11 inches from the ground was accessible to children. .2820(b) 1329 Application for enrollment did not include all required information. One child, two years and five months of age and enrolled on December 10, 2025, had missing information in the health care needs section of the child’s application. The missing information included list of the child’s fears and unique behavior characteristics, type of medication taken for health care needs, and other information to be shared that has a direct bearing on assuring safe medical treatment. .0801(a)(1-7) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. A shelter in place emergency drill was completed and recorded on December 11, 2025, at 9:30am and the previous shelter in place emergency drill was completed and recorded on August 19, 2025, at 10:40am. This violation was corrected during the visit. -Child’s application for enrollment: I suggested that you review all children’s applications upon enrollment to ensure all required information is recorded. If the information does not apply to the child, instruct parents to write “n/a” rather than leaving the information blank. -Emergency Drills: Shelter-in-place or lockdown drills must be practiced within every three months. I suggested that you make a note on a calendar or set a reminder on your computer to conduct emergency drills at least every three months. Shelter-in-place or lockdown drills are to be conducted within three months of the previous drill rather than once every quarter. Upon arrival, the purpose of today’s visit was reviewed with you, Michelle Chambers, Owner/Administrator. You, Michelle Chamber, Owner/Administrator assisted me with today’s visit. Your program currently operates with a five-star license effective March 2, 2023. The permit restrictions were in compliance including daytime care, 2nd shift care, overnight care, meets enhanced ratios, meets enhanced space, and no more than 80 children and staff in one day. The NC Secretary of State website was reviewed on August 14, 2023, and Kiddieland Child Development Center was listed as current/active. You reported no changes to the ownership of the facility. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. You conducted the walkthrough of the facility with me. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. The center's compliance history prior to today’s visit was reviewed with the operator. The program’s compliance history was ninety-seven percent (97%) as of August 14, 2023. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The most recent sanitation inspection for your facility was conducted on June 14, 2023. A Superior sanitation classification was issued with four (4) demerits. The most recent fire inspection for your facility was conducted on February 23, 2023. The most recent fire drill was conducted on August 10, 2023, at 2:30pm and recorded. The most recent lockdown emergency drill was conducted on May 21, 2023, at 11am and recorded. The most recent playground safety inspection was conducted on July 31, 2023. You stated no new staff have been hired since your most recent annual compliance visit conducted on February 1, 2023. The following violations were observed today: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The most recent annual fire inspection documentation on file for review was completed on February 23, 2023. The facility’s annual fire inspection was due by February 22, 2023. 10A NCAC 09 .0304(a) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In Space #3, an outlet located in the block center area did not have outlet safety covers in two of the outlets and an outlet in the art center area did not have an outlet safety cover in one of the outlets. Both areas were accessible to children. 10A NCAC 09 .0604(c) Compliance Letter: You must correct the violations found during today's visit immediately. Please send me a letter verifying compliance by August 29, 2023. Upon arrival, the purpose of today’s visit was reviewed with you, Michelle Chambers, Owner/Administrator. You, Michelle Chamber, Owner/Administrator assisted me with today’s visit. Your program currently operates with a five-star license effective March 2, 2023. The permit restrictions were in compliance including daytime care, 2nd shift care, overnight care, meets enhanced ratios, meets enhanced space, and no more than 80 children and staff in one day. The NC Secretary of State website was reviewed on August 14, 2023, and Kiddieland Child Development Center was listed as current/active. You reported no changes to the ownership of the facility. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. You conducted the walkthrough of the facility with me. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. The center's compliance history prior to today’s visit was reviewed with the operator. The program’s compliance history was ninety-seven percent (97%) as of August 14, 2023. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The most recent sanitation inspection for your facility was conducted on June 14, 2023. A Superior sanitation classification was issued with four (4) demerits. The most recent fire inspection for your facility was conducted on February 23, 2023. The most recent fire drill was conducted on August 10, 2023, at 2:30pm and recorded. The most recent lockdown emergency drill was conducted on May 21, 2023, at 11am and recorded. The most recent playground safety inspection was conducted on July 31, 2023. You stated no new staff have been hired since your most recent annual compliance visit conducted on February 1, 2023. The following violations were observed today: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection.
