You, Nicole Dobbins, Administrator, assisted me with the visit today. Your program currently operates with a five-star license effective March 13, 2025. You have earned seven (7) points in staff education, seven (7) points in program standards, and one (1) quality point. The program's license restrictions include the following: daytime care, meets enhanced space, and enhance ratios minus one (1). The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-seven percent (97%) as of January 23, 2026. Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to a recommended administrative action. You must maintain at least 75% Compliance History for each 18-month period as required. Your program was also monitored for compliance with implementing an approved curriculum Creative Curriculum: Teaching Strategies Gold as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted on January 28, 2025. The sanitation inspection was completed November 21, 2025, with a “Superior” classification and five (5) demerits. The last fire inspection was completed on October 13, 2025. The last lockdown emergency drill was completed on November 6, 2025, at 1:15pm and recorded. The last playground inspection was completed on December 19, 2025, by Harrietta Hall. The last fire drill was completed on December 19, 2025, at 2:50pm and recorded. Each indoor and outdoor space was monitored. Children enrolled were observed during outdoor gross motor play, indoor free play, snack time, group time, and lunch. Proper hand-washing techniques procedures were observed in the classrooms. Lunch was observed during today’s visit and included pepperoni and cheese pizza, chef salad with ranch dressing, potato wedges, apple, applesauce, and milk. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. The NC Pre-K Program Site Monitoring Tool was reviewed today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. Files Reviewed: The current NC Pre-K staff are lead teachers and teachers. A selection of files was monitored for completed health assessments and developmental screenings. Twelve (12) children’s files were reviewed during today’s visit. Two (2) staff files were reviewed, and two (2) new staff files were reviewed during today’s visit. I discussed the Staff and Training Worksheet and the Children’s Record form with you today. Both forms were completed and verified during the visit. You stated that you had completed the Brigance screenings for the children in your classrooms by September 30, 2025. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress. Checkpoint assessments are conducted three times per year: at the beginning in October 2025, middle in February 2026, and end of the school year in late April or May 2026. The first day of school was August 25, 2025. Staff also stated that additional resources are used in the classroom, such as Second Steps, Heggerty, Ready Rosie, and Finch. Classroom operating hours are from 7:40am – 2:30pm. Parent conferences are held twice per year. Two home visits are conducted two times per year. You stated that you use the Class Dojo (app) to communicate with parents, as well as in person, and through telephone calls. In addition, a folder is sent home daily with information such as daily reports, newsletters, second steps links are sent home to the parents which includes any information about the child’s day and any reminders. Documentation was reviewed of parent newsletters, monthly calendar with notes, parent contacts in Class Dojo, and observational notes of children. The environment rating scale score received on December 5th and 11th, 2025, and the scores were 6.22 and 5.36. NC Pre-K requirements were monitored today. The analysis date for the most recent lead water test was April 18, 2024. Lead water testing must be completed every three years. The violation observed today was discussed with you and documented in the Visit Summary left with you at the conclusion of this visit. Violation Number Comment Rule 1329 Application for enrollment did not include all required information. In Space #3, one child five years of age and enrolled on August 27, 2025, had missing information in the health care needs section of their application that was not available for review. In Space #4, one child four years of age and enrolled August 25, 2025, had missing information in the health care needs section of their application that was not available for review. .0801(a)(1-7) 1834 Application did not have a medical action plan attached for any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services. In Space #5, one child four years of age and enrolled on August 25, 2025, did not have available for review a current medical action plan. .0801(b) Technical Assistance was provided on the following Child’s application for enrollment- I suggested that you review all children’s applications upon enrollment to ensure all required information is recorded. If the information does not apply to the child, instruct parents to write “n/a” rather than leaving the information blank. Medical Action Plan: I suggested that you review the required documentation for medical action plan that must be included in a child’s application, file, and with the medication when a child has a diagnosed medical condition. I suggested that you check files upon enrollment and every 2 -3 months to ensure information is current.. Compliance Letter Documentation: The violations documented above must be corrected immediately. Send me a letter of compliance describing in detail specifically how each violation was corrected and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit and license ID number in the letter. I must receive your letter of compliance by February 6, 2026. An updated DCDEE WORKS account is required for all staff to process the education standard for your program. Education will be evaluated for administrators and staff who provide direct care to children. You, Trina Hord, Compliance and Licensure Coordinator, assisted me with the visit. Your program currently operates with a five-star license effective March 13, 2025. You have earned seven (7) points in staff education, seven (7) points in program standards, and one (1) quality point. The program's license restrictions include the following: daytime care, meets enhanced space, and enhance ratios minus one (1). A walk-through of the facility was completed today, and all indoor and outdoor areas were monitored. Staff and children were observed during snack time, outdoor and indoor free play, story time, group time, and music. Inspections/Drills: The most recent sanitation inspection for your facility was conducted on December 16, 2024. A superior sanitation classification was issued with four (4) demerits noted on the grade card. The last fire inspection was completed on October 17, 2024. The most recent fire drill was conducted on September 30, 2025, at 1:54pm and recorded. The most recent lockdown emergency drill was conducted on September 15, 2025, at 11:15am. The most recent playground safety inspection was conducted on September 30, 2025. You stated three (3) new staff have been hired since your most recent annual compliance visit conducted on February 5, 2025. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. The following violations of child care requirements were observed and documented during today’s visit. Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. In Space #5, on September 22, 23, and 24, 2025, thirteen children’s departure times were not documented. On October 2, 2025, sixteen children’s departure times were not documented. In Space #1, in an unlocked upper cabinet, one spray bottle of Clean Freak Multi-Purpose cleaner, Lysol Disinfecting Spray Can, and one container of Clorox wipes that were accessible to children. In Space #3, in an unlocked drawer at the teacher’s desk were one bottle of Pen+Gear Dry Erase Board Cleaning Spray and one bottle of BIC Cover-It Correction Fluid that were accessible to children. In Space #4, in an unlocked drawer at the teacher’s desk there was one bottle of BIC Cover-It Correction Fluid that was accessible to children. In Space #5, in an unlocked drawer at the teacher’s desk there was a tube of Blistex Lip Ointment that was accessible to children. In Space #6, in an unlocked drawer at the teacher’s desk there was one bottle of BIC Cover-It Correction Fluid that was accessible to children. In Space #7, in an unlocked cabinet, one container of Clorox Disinfecting wipes that was accessible to children. The hazardous materials had a written warning that stated to keep out of reach of children. .2820(b) Technical Assistance regarding the violations observed during today’s visit: -Arrival/Departure Times As a reminder, daily sign-in and sign-out times must be recorded when children arrive and depart the child care facility to ensure accurate records are kept. I suggested creating a system and review with staff of the procedures of recording sign-in and sign-out times. I also suggested that the closing teacher in each classroom checks the daily sign-in and sign-out documentation to ensure each child present has been checked in and out for the day. I suggested that you follow up with the closing staff to ensure accurate records are maintained. -Hazardous Materials/Plastic Bags with Written Warnings: We discussed that hazardous materials with written warnings keep out of reach of children must be kept in locked cabinets. I suggested reviewing with staff what is considered hazardous materials and conducted weekly checks in cabinets and teachers’ desks to ensure hazardous materials are locked. I reviewed with you the child care rules concerning safe environment and supervision. Child Care Rule References: 10A NCAC 09 .1801 SUPERVISION IN CHILD CARE CENTERS Children shall be adequately supervised at all times in child care centers. You, Trina Hord, Compliance and Licensure Coordinator, assisted me with the visit. Your program currently operates with a five-star license effective March 13, 2025. You have earned seven (7) points in staff education, seven (7) points in program standards, and one (1) quality point. The program's license restrictions include the following: daytime care, meets enhanced space, and enhance ratios minus one (1). A walk-through of the facility was completed today, and all indoor and outdoor areas were monitored. Staff and children were observed during snack time, outdoor and indoor free play, story time, group time, and music. Inspections/Drills: The most recent sanitation inspection for your facility was conducted on December 16, 2024. A superior sanitation classification was issued with four (4) demerits noted on the grade card. The last fire inspection was completed on October 17, 2024. The most recent fire drill was conducted on September 30, 2025, at 1:54pm and recorded. The most recent lockdown emergency drill was conducted on September 15, 2025, at 11:15am. The most recent playground safety inspection was conducted on September 30, 2025. You stated three (3) new staff have been hired since your most recent annual compliance visit conducted on February 5, 2025. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. The following violations of child care requirements were observed and documented during today’s visit. In Space #1, in an unlocked upper cabinet, one spray bottle of Clean Freak Multi-Purpose cleaner, Lysol Disinfecting Spray Can, and one container of Clorox wipes that were accessible to children. In Space #3, in an unlocked drawer at the teacher’s desk were one bottle of Pen+Gear Dry Erase Board Cleaning Spray and one bottle of BIC Cover-It Correction Fluid that were accessible to children. In Space #4, in an unlocked drawer at the teacher’s desk there was one bottle of BIC Cover-It Correction Fluid that was accessible to children. In Space #5, in an unlocked drawer at the teacher’s desk there was a tube of Blistex Lip Ointment that was accessible to children. In Space #6, in an unlocked drawer at the teacher’s desk there was one bottle of BIC Cover-It Correction Fluid that was accessible to children. In Space #7, in an unlocked cabinet, one container of Clorox Disinfecting wipes that was accessible to children. The hazardous materials had a written warning that stated to keep out of reach of children. .2820(b) Technical Assistance regarding the violations observed during today’s visit: -Arrival/Departure Times As a reminder, daily sign-in and sign-out times must be recorded when children arrive and depart the child care facility to ensure accurate records are kept. I suggested creating a system and review with staff of the procedures of recording sign-in and sign-out times. I also suggested that the closing teacher in each classroom checks the daily sign-in and sign-out documentation to ensure each child present has been checked in and out for the day. I suggested that you follow up with the closing staff to ensure accurate records are maintained. -Hazardous Materials/Plastic Bags with Written Warnings: We discussed that hazardous materials with written warnings keep out of reach of children must be kept in locked cabinets. I suggested reviewing with staff what is considered hazardous materials and conducted weekly checks in cabinets and teachers’ desks to ensure hazardous materials are locked. Compliance Letter Documentation: The violations documented above must be corrected immediately. Send me a letter of compliance describing in detail specifically how each violation was corrected and how compliance will be maintained in the future. I discussed with you and staff during the visit today that the naptime classrooms are not allowed to be pitch dark and that lighting should be provided in the classroom to allow staff to supervise and/or offer assistance to a child if needed. I also discussed that naptime music should not be so loud that staff are unable to hear children in case assistance is needed. I reviewed with you the child care rules concerning safe environment and supervision. You, Amanda Irvin, Administrator, assisted me with the visit today. On November 19, 2024, the following allegation was received: There is a concern that a child was not adequately supervised. The allegation was discussed with you and all staff members on December 3, 2024, and December 5, 2024. You and all staff members had an opportunity to ask questions, state perceptions of the situation and provide pertinent information in response to the allegation. On November 19, 2024, a self-report was reported to DCDEE concerning an incident that occurred on November 13, 2024. A child was placed on the wrong vehicle during departure by a substitute staff member. You stated that no checklist was used to document departure for November 13, 2024, for children being transported to James Love Elementary and to a day care van. You were made aware of the incident on November 14, 2024, and you sent an email to all staff with the following procedures to handle arrival and departure of children. The procedures included in the email were the new revised dismissal checklist, a link for the dismissal checklist, reminder to have staff check the tags on children’s backpacks detailing the vehicle each child is to be placed on, and communication with families to confirm no changes on the permission to transport form for each child was needed. You also stated that checklists were also given to the bus drivers to ensure that children were put on the correct vehicle during departure. You stated the procedures were effective November 18, 2024. You also stated that on November 21, 2024, you conducted a staff meeting to discuss and review arrival and departure procedures. On November 14, 2024, you stated you discussed the new revised departure procedures with parents. You provided a copy of the agenda to me and the email sent to staff. During staff interviews, it was reported that a departure checklist was unavailable, and children knew where to go. The program’s departure procedures were not followed by staff members. It was also reported that children were supervised the entire time during departure when the incident occurred. I observed supervision and departure of children, and no violations were observed. I also reviewed the emergency information for each children enrolled who rides the buses. Based on observations, staff interviews, and review of documentation, the allegation regarding supervision was unconfirmed, therefore unsubstantiated. Two violations unrelated to supervision was cited. The following violations were documented during today’s visit. Violation Number Comment Rule 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. The emergency contact information was not available for review on the James Love Elementary School bus for six out of seven children who use the bus. Four children were enrolled on August 28, 2024, one child was enrolled on August 26, 2024, and one child was enrolled on November 14, 2024. 10A NCAC 09 .1003(d) 1127 For routine transport of children to and from the center, staff did not have a list of the children being transported. On November 13, 2024, staff did not have a list of children being transported and did not follow their arrival and departure procedures. 10A NCAC 09 .1003(l) Technical Assistance I discussed with you to continue to use the new revised arrival and departure checklists, and review documentation daily to confirm that staff are following the arrival and departure procedures. I also suggested that you review the arrival and departure procedures with all staff at least annually and document the review. 10A NCAC 09 .1003 SAFE PROCEDURES for Transportation (l) For routine transport of children to and from the center, staff shall have a list of the children being transported. Staff members shall use this list to document attendance as children board the vehicle and as they depart the vehicle. A list of all children being transported shall also be available at the center. Fifteen (15) children's files were reviewed during today’s visit. One (1) staff file was reviewed and five (5) new staff files were reviewed today. I discussed the Staff and Training Worksheet and the Children’s Record form with you today. Both forms were completed and verified during the visit. The following violation was observed during the visit today. I discussed this violation with you in detail at the end of the visit.
