A checklist was used to note the requirements I monitored today. Staff and training worksheets were completed and monitored during the visit. You, Sarah Goodson, Administrator, were present when I arrived and you were notified of the purpose of today’s visit and you were available for consultation at the conclusion of the visit. This facility currently operates with a GS 110-106 Notice of Compliance issued March 2, 2016. The last annual compliance visit was conducted on May 13, 2025. The sanitation inspection was completed March 9, 2026, with an “Superior” classification. The last fire inspection was completed on June 3, 2024. Upon arrival, I observed an infant sitting in a bouncy seat in a space designated for child care. When asked, Ms. Goodson stated that the child was her child and she was waiting for the child to be picked up. It was further stated that there was a conflict with the child's grand parent that normally keeps the child; therefore, the child had to come with her to the facility. The teachers stated that the infant comes to the facility at various times and various days of the week. We reviewed that the age range on the permit for the facility is two (2) - five (5) years of age. Due to the age range on the permit, the infant can not attend the facility, even if you are caring for the infant. The other enrolled children present were observed interacting with assigned caregivers, having routine care needs met, playing in the outdoor environment, and playing with different activities indoors. Prior to today’s visit, the program’s compliance history was 97% over the current 18- month period. A copy of your compliance history report was left with you. The center is owned and operated by a non-profit corporation, Spring Creek Baptist Church, Inc. with a current active status based on today’s research on the North Carolina Secretary of State database. This corporation owns the center, which means a Current/Active status must always be maintained. Failure to maintain a Current/Active corporation status may warrant the issuance of an Administrative Action, including Suspension and/or Revocation of the Notice of Compliance. Prior to any changes with the corporation, please contact me to discuss change of ownership procedures. I used the Annual Compliance Monitoring Checklist for Child Care and the Child Care Center Item Number Listing (DCD 0357) as a basic monitoring tool to assess compliance with all applicable child care requirements. The computer-generated visit summary was reviewed with you at the conclusion of the visit and a copy of the visit summary was left with you. The office door was open upon arrival. Hazardous cleaning items in an aerosol can were being stored on a shelf. This area was accessible to children. .2820(b) 1043 All staff records, except financial records, were not made available for review. The following items were not available for staff member M. Lovelace: application, orientation, medical, health questionnaire, and shaken baby acknowledgement. Staff member S. Goodson does not have orientation documentation and the shaken baby acknowledgement. You, Sarah Goodson, Administrator, were present when I arrived and you were notified of the purpose of today’s visit and you were available for consultation at the conclusion of the visit. This facility currently operates with a GS 110-106 Notice of Compliance issued March 2, 2016. The last annual compliance visit was conducted on May 13, 2025. The sanitation inspection was completed March 9, 2026, with an “Superior” classification. The last fire inspection was completed on June 3, 2024. Upon arrival, I observed an infant sitting in a bouncy seat in a space designated for child care. When asked, Ms. Goodson stated that the child was her child and she was waiting for the child to be picked up. It was further stated that there was a conflict with the child's grand parent that normally keeps the child; therefore, the child had to come with her to the facility. The teachers stated that the infant comes to the facility at various times and various days of the week. We reviewed that the age range on the permit for the facility is two (2) - five (5) years of age. Due to the age range on the permit, the infant can not attend the facility, even if you are caring for the infant. The other enrolled children present were observed interacting with assigned caregivers, having routine care needs met, playing in the outdoor environment, and playing with different activities indoors. Prior to today’s visit, the program’s compliance history was 97% over the current 18- month period. A copy of your compliance history report was left with you. The center is owned and operated by a non-profit corporation, Spring Creek Baptist Church, Inc. with a current active status based on today’s research on the North Carolina Secretary of State database. This corporation owns the center, which means a Current/Active status must always be maintained. PROVIDER ACCESS TO THE BACKGROUND CHECK MANAGEMENT SYSTEM: Beginning February 1, child care administrators can access the Automated Background Check Management System (ABCMS) This access will allow providers to: • See the real-time background check status of staff members. • Run a printable report of the staff roster to assist with compliance visits. • See new background check applicants and add to staff roster. Access to the ABCMS requires the successful completion of a Moodle course consisting of a short video followed by a test.
